Project description:We demonstrate an extended-imaging-range anterior-segment optical coherence tomography (eAS-OCT) system for the biometric assessment of full AS in human eye. This newly developed eAS-OCT operating at 1340-nm wavelength band is simultaneously capable of an imaging speed of 120 kHz A-line scan rate, an axial resolution of 7.2 ?m, and an extended imaging range of up to 16 mm in air. Imaging results from three healthy subjects and one subject with a narrow-angle demonstrate the instrument's utility. With this system, it can provide anatomical dimensions of AS, including central corneal thickness, anterior chamber width, anterior chamber depth, crystalline lens vault, crystalline lens thickness, angle opening distance (AOD500/AOD750), and the area described by the trabecular-iris space (TISA500/TISA750) at 500/750???m. We also use eAS-OCT to image and quantify dynamic functional changes of the AS in response to a light stimulus that induces physiological pupillary changes as well as accommodative efforts that induce lens changes. The results show that the described eAS-OCT is able to provide full anatomical biometry for AS and is useful for the studies where the dynamic response of AS compartment to certain stimulus is required.

Project description:We describe a high-speed long-depth range optical frequency domain imaging (OFDI) system employing a long-coherence length tunable source and demonstrate dynamic full-range imaging of the anterior segment of the eye including from the cornea surface to the posterior capsule of the crystalline lens with a depth range of 12 mm without removing complex conjugate image ambiguity. The tunable source spanned from 1260 to 1360 nm with an average output power of 15.8 mW. The fast A-scan rate of 20,000 per second provided dynamic OFDI and dependence of the whole anterior segment change on time following abrupt relaxation from the accommodated to the relaxed status, which was measured for a healthy eye and that with an intraocular lens.

Project description:PurposeTo measure by ultralong scan depth optical coherence tomography (UL-OCT) dimensional changes in the anterior segment of human eyes during accommodation.DesignEvaluation of diagnostic test or technology.ParticipantsForty-one right eyes of healthy subjects with a mean age of 34 years (range, 22-41 years) and a mean refraction of -2.5 ± 2.6 diopters were imaged in 2 repeated measurements at minimal and maximal accommodations.MethodsA specially adapted and designed UL-OCT instrument was used to image from the front surface of the cornea to the back surface of the crystalline lens. Custom software corrected the optical distortion of the images and yielded the biometric measurements. The coefficient of repeatability and the intraclass correlation coefficient were calculated to evaluate the repeatability and reliability.Main outcome measuresAnterior segment parameters and associated repeatability and reliability upon accommodation. The dimensional results included central corneal thickness (CCT), anterior chamber depth and width (ACD, ACW), pupil diameter (PD), lens thickness (LT), anterior segment length (ASL = ACD + LT), lens central position (LCP = ACD + 1/2LT), and horizontal radii of the lens anterior and posterior surface curvatures (LAC, LPC).ResultsRepeated measurements of each variable within each accommodative state did not differ significantly (P>0.05). The coefficients of repeatability (CORs) and intraclass correlation coefficients for CCT, ACW, ACD, LT, LCP, and ASL were excellent (1.2%- 3.59% and 0.998-0.877, respectively). They were higher for PD (18.90%-21.63% and 0.880-0.874, respectively) and moderate for LAC and LPC (34.86%-42.72% and 0.669-0.251, respectively) in the 2 accommodative states. Compared with minimal accommodation, PD, ACD, LAC, LPC, and LCP decreased and LT and ASL increased significantly at maximal accommodation (P<0.05), whereas CCT and ACW did not change (P>0.05).ConclusionsThe UL-OCT measured changes in anterior segment dimensions during accommodation with good repeatability and reliability. Project description:We report a newly developed multifunctional 1050 nm spectral domain optical coherence tomography (SD-OCT) system working at 147 kHz A-scan rate for posterior eye imaging. It is demonstrated through in-vivo experiments that this system delivers not only superior performance of posterior eye structural imaging but also detailed visualization of microcirculation network in retina. The choroid of the eye with either myopic or normal conditions can clearly be visualized through the entire scanning volume. These results indicate great potential in applying this new system for clinical studies.

Project description:Purpose:To develop high-speed, extended-range, ultrahigh-resolution spectral-domain optical coherence tomography (UHR SD-OCT) and demonstrate scan protocols for clinical retinal imaging. Methods:A UHR SD-OCT operating at 840-nm with 150-nm bandwidths was developed. The axial imaging range was extended by dynamically matching reference arm length to the retinal contour during acquisition. Two scan protocols were demonstrated for imaging healthy participants and patients with dry age-related macular degeneration. A high-definition raster protocol with intra-B-scan reference arm length matching (ReALM) was used for high-quality cross-sectional imaging. A cube volume scan using horizontal and vertical rasters with inter-B-scan ReALM and software motion correction was used for en face and cross-sectional imaging. Linear OCT signal display enhanced visualization of outer retinal features. Results:UHR SD-OCT was demonstrated at 128- and 250-kHz A-scan rates with 2.7 µm axial resolution and a 1.2-mm, 6-dB imaging range in the eye. Dynamic ReALM was used to maintain the retina within the 6-dB imaging range over wider fields of view. Outer retinal features, including the rod and cone interdigitation zones, retinal pigment epithelium, and Bruch's membrane were visualized and alterations observed in age-related macular degeneration eyes. Conclusions:Technological advances and dynamic ReALM improve the imaging performance and clinical usability of UHR SD-OCT. Translational Relevance:These advances should simplify clinical imaging workflow, reduce imaging session times, and improve yield of high quality images. Improved visualization of photoreceptors, retinal pigment epithelium, and Bruch's membrane may facilitate diagnosis and monitoring of age-related macular degeneration and other retinal diseases.

Project description:The purpose of this study was to measure central macular thickness in an unprecedented number of cynomolgus monkeys. Macular thickness was measured with Heidelberg spectral-domain OCT in 320 eyes of healthy and treatment-naïve cynomolgus monkeys (80 males and 80 females). The macula was successfully measured in all 320 eyes. Macular thickness was not significantly different between the sexes. The mean central macular thickness was 244 μm (+/- 21 μm). Macular thicknesses in the quadrants were 327 +/-17 μm (temporal inner), 339 +/- 17 μm (inferior inner), 341 +/- 14 μm (superior inner), 341 +/-18 μm (nasal inner), and 299 +/- 20 μm (temporal outer), 320 +/- 16 μm (superior outer), 332 +/-23 μm (inferior outer), and 337 +/-18 μm (nasal outer). Highly significant differences between the nasal and temporal quadrants were detected. This study successfully demonstrated the feasibility of retinal thickness measurements in healthy cynomolgus monkeys. The present findings indicate that the macula is thicker in cynomolgus monkeys than in humans and provide important normative data for future studies.

Project description:Iridocorneal angle (ICA) details particularly the trabecular meshwork (TM), Schlemm's canal (SC), and collector channels (CCs) play crucial roles in the regulation of the aqueous outflow in the eyes and are closely associated with glaucoma. Current clinical gonioscopy imaging provides no depth information, and studies of 3D high-resolution optical coherence tomography (OCT) imaging of these structures are limited. We developed a custom-built spectral-domain (SD-) OCT imaging system to fully characterize the angle details. Imaging of a human cadaver eye reveals the visibility of details in the TM/SC/CC region via a 'crossline' scanning and a series of image processing. This shows that ICA imaging can be used for preoperative glaucoma inspections in the clinical setting with the proposed prototype.

Project description:BackgroundRetinal ganglion cell (RGC) death is a common cause of loss of vision during glaucoma. Pattern electroretinogram (PERG) is an objective measure of the central retinal function that correlates with macular GCL thickness. The aim of this study is to determine possible relationships between the N95 amplitude of pattern electroretinogram (PERGamp) and macular ganglion cell/inner plexiform layer thickness (GCIPLT).Methods and findingsThis was a retrospective and comparative study including 74 glaucoma patients (44 early stage and 30 advanced stage cases) and 66 normal control subjects. Macular GCIPLT was measured using Cirrus spectral domain-optical coherence tomography. Standard automated perimetry and pattern ERGs were used in all patient examinations. Three types of regression analysis (broken stick, linear regression, and quadratic regression) were used to evaluate possible relationships between PERGamp and GCIPLT. Correlations between visual field parameters and GCIPLT were evaluated according to glaucoma severity. The best fit model for the relationship between PERGamp and GCIPLT was the linear regression model (r2 = 0.22; P < 0.001). The best-fit model for the relationship between visual field parameters and GCIPLT was the broken stick model. During early glaucoma, macular GCIPLT was positively correlated with PERGamp, but not with visual field loss. In advanced glaucoma, macular GCIPLT was positively correlated with both PERGamp and visual field loss.ConclusionsPERGamp was significantly correlated with macular GCIPT in early glaucoma patients, while visual field performance showed no correlation with GCIPLT. PERGamp can therefore assist clinicians in making an early decision regarding the most suitable treatment plan, especially when GCIPLT is thinning with no change in visual field performance.

Project description:PurposeTo report retinal findings for healthy newborn infants imaged with handheld spectral-domain optical coherence tomography (SD OCT).DesignProspective, observational case series.MethodsThirty-nine full-term newborn infants underwent dilated retinal examinations by indirect ophthalmoscopy and retinal imaging by handheld SD OCT, without sedation, at the Duke Birthing Center.ResultsOf the 39 infants imaged, 44% (17/39) were male. Race and ethnicity composition was 56% white, 38% black, 3% Asian, and 3% Hispanic. Median gestational age was 39 weeks (range, 36 to 41 weeks). Six (15%) of the 39 infants had bilateral subfoveal fluid on SD OCT not seen by indirect ophthalmoscopy. Eight infants (21%) had retinal hemorrhages noted on dilated retinal examination, 1 of which had subretinal fluid on SD OCT. Subretinal fluid was noted on follow-up examination to have resolved on SD OCT 1 to 4 months later. Infants with bilateral subretinal fluid had an older gestational age compared with infants without subretinal fluid (median, 40.4 vs 39.1 weeks, respectively; P = .03) and were more likely to have had mothers with diabetes (2/6 vs 0/33, respectively; P = .02). Vaginal versus Caesarian section delivery was not significantly different between the 2 groups.ConclusionsSome healthy full-term infants have bilateral subfoveal fluid not obvious on dilated retinal examination. This fluid resolves within several months. The visual significance of this finding is unknown, but clinicians should be aware that it is common when evaluating newborn infants for retinal pathologic features using SD OCT.

Project description:Real-time display of processed en-face spectral domain optical coherence tomography (SD-OCT) images is important for diagnosis. However, due to many steps of data processing requirements, such as Fast Fourier transformation (FFT), data re-sampling, spectral shaping, apodization, zero padding, followed by software cut of the 3D volume acquired to produce an en-face slice, conventional high-speed SD-OCT cannot render an en-face OCT image in real time. Recently we demonstrated a Master/Slave (MS)-OCT method that is highly parallelizable, as it provides reflectivity values of points at depth within an A-scan in parallel. This allows direct production of en-face images. In addition, the MS-OCT method does not require data linearization, which further simplifies the processing. The computation in our previous paper was however time consuming. In this paper we present an optimized algorithm that can be used to provide en-face MS-OCT images much quicker. Using such an algorithm we demonstrate around 10 times faster production of sets of en-face OCT images than previously obtained as well as simultaneous real-time display of up to 4 en-face OCT images of 200 × 200 pixels(2) from the fovea and the optic nerve of a volunteer. We also demonstrate 3D and B-scan OCT images obtained from sets of MS-OCT C-scans, i.e. with no FFT and no intermediate step of generation of A-scans.
