Economic considerations for social distancing and behavioral based policies during an epidemic.

ABSTRACT: Public policies intended to induce behavioral change, specifically incentives to reduce interpersonal contacts or to "social distance," increasingly play a prominent role in public disease response strategies as governments plan for and respond to major epidemics. I compare social distancing incentives and outcomes under decentralized, full control social planner, and constrained social planner, without health class specific control, decision making scenarios. Constrained social planner decision making, based on non-health class specific controls, can in some instances make society worse off than decentralized decision making (i.e. no intervention). The oft neglected behavior of recovered and immune individuals is important for welfare and health outcomes.

Social acceptability of standard and behavioral economic inspired policies designed to reduce and prevent obesity.

Project description:The obesity epidemic is a significant public policy issue facing the international community, resulting in substantial costs to individuals and society. Various policies have been suggested to reduce and prevent obesity, including those informed by standard economics (a key feature of which is the assumption that individuals are rational) and behavioral economics (which identifies and harness deviations from rationality). It is not known which policy interventions taxpayers find acceptable and would prefer to fund via taxation. We provide evidence from a discrete choice experiment on an Australian sample of 996 individuals to investigate social acceptability of eight policies: mass media campaign; traffic light nutritional labeling; taxing sugar sweetened beverages; prepaid cards to purchase healthy food; financial incentives to exercise; improved built environment for physical activity; bans on advertising unhealthy food and drink to children; and improved nutritional quality of food sold in public institutions. Latent class analysis revealed three classes differing in preferences and key respondent characteristics including capacity to benefit. Social acceptability of the eight policies at realistic levels of tax increases was explored using post-estimation analysis. Overall, 78% of the sample were predicted to choose a new policy, varying from 99% in those most likely to benefit from obesity interventions to 19% of those least likely to benefit. A policy informed by standard economics, traffic light labeling was the most popular policy, followed by policies involving regulation: bans on junk food advertising to children and improvement of food quality in public institutions. The least popular policies were behaviorally informed: prepaid cards for the purchase of only healthy foods, and financial incentives to exercise.

Disease-dependent interaction policies to support health and economic outcomes during the COVID-19 epidemic.

Project description:Lockdowns and stay-at-home orders have partially mitigated the spread of Covid-19. However, the indiscriminate nature of mitigation - applying to all individuals irrespective of disease status - has come with substantial socioeconomic costs. Here, we explore how to leverage the increasing reliability and scale of both molecular and serological tests to balance transmission risks with economic costs involved in responding to Covid-19 epidemics. First, we introduce an optimal control approach that identifies personalized interaction rates according to an individual's test status; such that infected individuals isolate, recovered individuals can elevate their interactions, and activity of susceptible individuals varies over time. Critically, the extent to which susceptible individuals can return to work depends strongly on isolation efficiency. As we show, optimal control policies can yield mitigation policies with similar infection rates to total shutdown but lower socioeconomic costs. However, optimal control policies can be fragile given mis-specification of parameters or mis-estimation of the current disease state. Hence, we leverage insights from the optimal control solutions and propose a feedback control approach based on monitoring of the epidemic state. We utilize genetic algorithms to identify a 'switching' policy such that susceptible individuals (both PCR and serological test negative) return to work after lockdowns insofar as recovered fraction is much higher than the circulating infected prevalence. This feedback control policy exhibits similar performance results to optimal control, but with greater robustness to uncertainty. Overall, our analysis shows that test-driven improvements in isolation efficiency of infectious individuals can inform disease-dependent interaction policies that mitigate transmission while enhancing the return of individuals to pre-pandemic economic activity.

In this paper, we demonstrate how individualized policies based on disease status can reduce transmission risk while minimizing impacts on economic outcomes. We design feedback control policies informed by optimal control solutions to modulate interaction rates of individuals based on the epidemic state. We identify personalized interaction rates such that recovered/immune individuals elevate their interactions and susceptible individuals remain at home before returning to pre-lockdown levels. As we show, feedback control policies can yield similar population-wide infection rates to total shutdown but with significantly lower economic costs and with greater robustness to uncertainty compared to optimal control policies. Our analysis shows that test-driven improvements in isolation efficiency of infectious individuals can inform disease-dependent interaction policies that mitigate transmission while enhancing the return of individuals to pre-pandemic economic activity.

Considerations for remote delivery of behavioral economic interventions for substance use disorder during COVID-19 and beyond.

Project description:The response to the COVID-19 crisis has created direct pressure on health care providers to deliver virtual care, and has created the opportunity to develop innovations in remote treatment for people with substance use disorders. Remote treatments provide an intervention delivery framework that capitalizes on technological innovations in remote monitoring of behaviors and can efficiently use information collected from people and their environment to provide personalized treatments as needed. Interventions informed by behavioral economic theories can help to harness the largely untapped potential of virtual care in substance use treatment. Behavioral economic treatments, such as contingency management, the substance-free activity session, and episodic future thinking, are positioned to leverage remote monitoring of substance use and to use personalized medicine frameworks to deliver remote interventions in the COVID-19 era and beyond.

Behavioral Economic Approaches for Population-Based Colorectal Cancer Screening

Project description:This is a 3-year pragmatic, randomized clinical trial among average-risk patients at diverse primary care practices who are overdue for colorectal (CRC) screening. This project aims to evaluate the effect of a centralized program that includes direct outreach to patients and visit-based, clinician directed nudges facilitated by the electronic health record (EHR) with follow-up text messaging on the uptake of CRC screening. The primary outcome is CRC screening completion at 3 years. Patient and clinician factors impacting the experience and effectiveness of the intervention will be explored through surveys and qualitative interviews.

Economic considerations of macular edema therapies.

Project description:PurposeTo relate costs and treatment benefits for diabetic macular edema (DME), branch retinal vein occlusion (BRVO), and central retinal vein occlusion (CRVO).DesignA model of resource use, outcomes, and cost-effectiveness and utility.ParticipantsNone.MethodsResults from published clinical trials (index studies) of laser, intravitreal corticosteroids, intravitreal anti-vascular endothelial growth factor (VEGF) agents, and vitrectomy trials were used to ascertain visual benefit and clinical protocols. Calculations followed from the costs of 1 year of treatment for each treatment modality and the visual benefits as ascertained.Main outcome measuresVisual acuity (VA) saved, cost of therapy, cost per line saved, cost per line-year saved, and costs per quality-adjusted life years (QALYs).ResultsThe lines saved for DME (0.26-2.02), BRVO (0.74-4.92), and CRVO (1.2-3.75) yielded calculations of costs/line of saved VA for DME ($1329-$11,609), BRVO ($494-$13,039), and CRVO ($704-$7611); costs/line-year for DME ($60-$561), BRVO ($25-$754), and CRVO ($45-$473); and costs/QALY ($824 to $25,566).ConclusionsRelative costs and benefits should be considered in perspective when applying and developing treatment strategies.

The detection of the epidemic phase of COVID-19 and the timing of social distancing policies in Korea.

Project description:ObjectivesObserving cumulative and new daily confirmed cases of COVID-19, disease control authorities respond to a surge in cases with social distancing measures or economic lockdown. The question in this article is whether we can gather more useful information from a readily available time series data set of day-to-day changes in confirmed cases of COVID-19.Study designTime-series data analysis was done using a hidden Markov model.MethodsDay-to-day differences in confirmed cases of COVID-19 in Korea from February 19, 2020, to July 13, 2021, were modeled via a hidden Markov model. The results from the model were compared with the effective reproduction number and the Korean government's response.ResultsThe model reports that Korea was in an epidemic phase from August 2020 and from mid-November 2020, the second and third epidemic waves. The government's response, represented by the Government Response Stringency Index, was not timely during the epidemic phases. The results from the model may also be more helpful to detect the onset of the epidemic phase of an infectious disease than the effective reproduction number.ConclusionsThe model can reveal a hidden epidemic phase and help disease control authorities to respond more promptly and effectively.

Combination of economic policies: how the perfect storm wrecked the Brazilian economic growth.

Project description:This study uses a counterfactual analysis to investigate, from the Brazilian experience, the "perfect storm" resultant from the combination of economic policies on economic growth. Specifically, we analyze whether the combination of economic policies that neglect fiscal balance and low and stable inflation with the adoption of strategies to stimulate economic growth without considering the side effects on the economy harmed economic growth. Our findings, robust to several placebo tests, show Brazil's growth rate is approximately 2.8 pp below the "synthetic Brazil" growth rate. Furthermore, comprehending the great shocks in the period under investigation, the complementary empirical analysis supports the view that the "perfect storm" is the main factor explaining the underperformance of the Brazilian economic growth.Supplementary informationThe online version contains supplementary material available at 10.1007/s00181-021-02167-4.

Psychological distress during the COVID-19 epidemic in Chile: The role of economic uncertainty.

Project description:Previous research has shown that the COVID-19 outbreak, social distancing, and lockdown can affect people's psychological well-being. The aims of this study were (1) to estimate the extent to which perceptions and expectations regarding the social, economic, and domestic effects of the COVID-19 outbreak are associated with psychological distress and (2) to identify some demographic, psychosocial, and economic factors associated with increased vulnerability to psychological distress during the COVID-19 outbreak in Chile. 1078 people participated in a telephone survey between May 30 and June 10, 2020. The sample is representative of the Chilean adult population. Psychological distress was assessed through a questionnaire of anxious and depressive symptoms (Patient Health Questionnaire-4). We analyzed the data set using ordinary least-squares regression models, first estimating models for the entire sample, and then stratifying the sample into different groups to explore differences by gender and age. 19.2% of participants displayed significant psychological distress (PHQ-4 ≥ 6), with moderate to severe anxiety-depression symptoms being more prevalent in women than in men (23.9% vs 14.1%, χ2 16.78, p<0.001). The results of this study suggest that being a woman, feeling lonely and isolated, living in the areas hit hardest by the pandemic and lockdown, expecting a lack of income due to having to stop working as a consequence of the pandemic, and having a history of diagnosed mental disorders are significantly associated with psychological distress (p<0.05). The results of this study highlight the need to implement psychosocial programs to guard people's psychological well-being and social policies to address economic uncertainty during the current COVID-19 outbreak in Chile.
