Immediate loading of one-piece implants in conjunction with a modified technique of inferior alveolar nerve lateralization: 10 years follow-up.

ABSTRACT: This report describes a treatment modification for a patient presented with severely resorbed bilateral edentulous posterior mandible and mobility of the anterior teeth. There was less than 8 mm of bone between the crest of the alveolar ridge and the mandibular canal as revealed by radiographic examination. A modified technique for inferior alveolar nerve lateralization (IANL) in conjunction with ridge expansion was performed using threaded bone expanders, which allowed for better primary stability and placing longer implants. A total of four postextraction implants were in the anterior region of the mandible. The mandible received a total of nine one-piece implants to allow for immediate nonfunctional loading. The definitive ceramometallic prosthesis was delivered 3 months postoperatively. The 10 years clinical and radiographic assessment showed minimal bone resorption around osseointegrated implants. One-piece implants showed great success rate and minimal bone resorption following the modified technique of IANL and immediate implantation.

Immediate functional loading of single implants: a multicenter study with 4 years of follow-up.

Project description:Background. In the current scientific literature there are only few studies on the immediate functional loading of single implants. The aim of the present present study was to evaluate the 4-year survival rate, complication rate and peri-implant marginal bone loss (PIMBL) of immediately loaded single implants inserted in healed ridges and fresh post-extraction sites. Methods. Six centers were involved in this prospective study. The surgical and prosthetic protocol was defined in detail, before the start of recruiting patients. Recruitment of patients and performance of surgeries took place between February 2012 and February 2013. Criteria for inclusion were single-tooth gaps in healed ridges and fresh post-extraction sockets. All the fixtures (Anyridge®, Megagen Corporation, Gyeongbuk, South Korea) were functionally loaded immediately after insertion and followed for a period of 4 years. Outcome measures were implant survival, complications and PIMBL. Results. Forty-six patients (18‒73 years of age) were selected. In total, 57 fixtures were placed (10 in fresh post-extraction sockets). After 4 years of functional loading, only one fixture was lost; therefore, high survival rates (97.6% patient-based; 98.1% implant-based) were reported. In addition, a limited incidence of biologic (4.8% patient-based; 3.8% implant-based) and prosthetic (9.7% patient-based; 7.6% implant-based) complications was reported. The overall 4-year PIMBL amounted to 0.38±0.21 mm (healed ridges: 0.4±0.21 mm; fresh post-extraction sockets: 0.33±0.20 mm). Conclusion. Loading single implants immediately seems to be a highly successful treatment modality. However, long-term data are needed to confirm these positive outcomes.

Immediate Loading of Short Implants in Posterior Maxillae: Case Series.

Project description:There is an increasing number of scientific evidence supporting the clinical use of short dental implants. However, few studies have evaluated the long-term outcomes of immediate loading protocols in short implants. This study aims to evaluate immediate loading of short dental implants in the posterior regions of the maxillae.Patients having short implants in maxillae posterior areas inserted before December, 2010 and immediately loaded were selected. The following data were gathered regarding patients' age and gender, implant dimensions, anatomical location, and prosthodontic treatment. The outcome variables were peri-implant bone remodeling and the survival rates of the short implants and the prosthesis.A 10 short implants were placed in 8 patients. The mean follow-up time was 4.6 ± 1.21 years after loading and 1 implant failed. One prosthetic complication occurred. No prostheses failed resulting in a survival rate of 100%.The immediate loading of short implants in maxillae posterior areas could save time, cost and could be regarded as a successful treatment.

Randomized controlled clinical study comparing two types of two-piece dental implants supporting fixed restorations-Results at 8 years of loading.

Project description:ObjectivesTo assess clinical, technical, biological, and radiographic outcomes of implants supporting fixed restorations using two types of dental implants with non-matching implant-abutment junctions at 8 years.Materials and methodsSixty-four patients were randomly assigned to receive one of two implant systems (S1 or S2) and eventually fixed restorations. Patients were examined at loading (TL ), one (T1 ), three (T3 ), five (T5 ), and eight years (T8 ). Outcome measures included implant and restoration survival, technical and biological complications, and radiographic bone levels. All data were analyzed on the implant and patient level.ResultsNinety-eight implants were inserted in 64 patients and loaded with fixed restorations. At 8 years, 49 patients with 42 (S1) and 36 (S2) implants (25 in group S1 and 24 in group S2 on the patient level) were re-examined. The survival rates on the patient level were 97.6% (S1) and 97.2% (S2). The marginal bone levels (the primary endpoint) amounted to a gain of 0.21 mm (Q1: -0.11 mm; Q3: 0.5 mm) (S1) (p = .007) and to a loss of 0.24 mm (Q1: -0.79 mm; Q3: 0.05 mm) (S2) (p = .001) between baseline (TL ) and T8 (intergroup p < .001). The technical complication rates were 28% (S1) and 12.5% (S2) (intergroup p = .289). Peri-implant mucositis was observed in 24% (S1) and 50% (S2) of the implants on the patient level (intergroup p = .792). The respective figures for peri-implantitis were 0% (S1) and 12.5% (S2) (intergroup p = .11).ConclusionsDental implants with non-matching implant-abutment junctions supporting fixed restorations resulted in high survival rates independent of the system used. Differences, mainly observed in terms of technical complications (in favor of S2), biological complications (in favor of S1), and marginal bone-level changes (in favor of group S1), appear to be clinically negligible.

Bone-implant contact around crestal and subcrestal dental implants submitted to immediate and conventional loading.

Project description:The present study aims to evaluate the influence of apicocoronal position and immediate and conventional loading in the percentage of bone-implant contact (BIC). Thus, 36 implants were inserted in the edentulous mandible from six dogs. Three implants were installed in each hemimandible, in different positions in relation to the ridge: Bone Level (at crestal bone level), Minus 1 (one millimeter apical to crestal bone), and Minus 2 (two millimeters apical to crestal bone). In addition, each hemimandible was submitted to a loading protocol: immediate (prosthesis installed 24 hours after implantation) or conventional (prosthesis installed 120 days after implantation). Ninety days after, animals were killed, and implant and adjacent tissues were prepared for histometric analysis. BIC values from immediate loaded implants were 58.7%, 57.7%, and 51.1%, respectively, while conventional loaded implants were 61.8%, 53.8%, and 68.4%. Differences statistically significant were not observed among groups (P = 0.10, ANOVA test). These findings suggest that different apicocoronal positioning and loading protocols evaluated did not interfere in the percentage of bone-implant contact, suggesting that these procedures did not jeopardize osseointegration.

Delayed versus immediate loading of implants: survival analysis and risk factors for dental implant failure.

Project description:The purpose of this study was to estimate 1-year survival for delayed versus immediately loaded implants and identify risk factors for implant failure.This was a retrospective cohort study, consisting of a sample of subjects who had greater than or equal to 1 Bicon dental implant (Bicon, Boston, MA) placed over a 13-year period. The primary predictor variable was method of implant loading: delayed (3 to 6 months after placement) or immediately after insertion. Secondary predictor variables were classified as demographic, anatomic, implant/abutment, and reconstructive. The outcome variable was implant failure, defined as removal of the implant, and was recorded as months of survival. Descriptive, Kaplan-Meier, and univariate Cox proportional hazards statistics were computed. Univariate associations with P <or= .15 and biologically relevant variables (eg, age, gender) were included in a marginal multiple Cox regression model. In the multiple model, a P value of <or= .05 was considered statistically significant.The study sample consisted of 677 subjects who had 2,349 delayed-loaded dental implants and 178 patients who had 477 immediate-loaded implants. The unadjusted 1-year survival estimates for the delayed and immediate loading groups were 95.5% and 90.3%, respectively (P < .01). In the marginal multiple Cox regression model, immediate loading, current tobacco use, maxillary implants, and shorter implants were associated with failure (P <or= .05).In this study, implants loaded immediately were 2.7 times (after adjusting) more likely to fail at 1 year compared with delayed-loaded implants.

Mini Inferior Turbinoplasty-Tunneling Technique.

Project description:Techniques for inferior turbinate reduction vary with various surgical methods, which differ in the approach of preservation of tissue from total turbinectomy to limited submucosal cauterization. Our preferred method to address hypertrophic inferior turbinate by mini turbinoplasty-tunneling technique are presented. Critical steps include creation of window in the inferior turbinate with a 4 mm microdebrider blade and removal of both inferior turbinate mucosal hypertrophy and bony component to convert a convex looking inferior turbinate to concave shape. This allows proper debulking of the entire medial aspect of the inferior turbinate and widening of the nasal valve area. Mini turbinoplasty-tunneling technique for hypertrophic inferior turbinates, is a safe method in achieving turbinate size reduction with minimal morbidity and long-term relief of nasal obstructive symptoms without added risk of complications.

Clinical efficacy of early loading versus conventional loading of dental implants.

Project description:The aim of this study was to determine the clinical differences between early and conventional loading protocols for dental implants. A comprehensive search of the Medline, Embase, and OVID databases for studies published through January 10, 2015 was conducted. Fourteen studies were included in our analysis. We found that early loading imposed a significantly higher risk of implant failure than did conventional loading (risk ratio = 2.09, 95% confidence interval [CI] [1.18, 3.69], P = 0.01), while no significant differences between the methods were found with regards to the marginal bone loss (weighted mean differences [WMD]?= 0.11, 95% CI [-0.07, 0.28], P = 0.23), periotest value (WMD?= 0.02, 95% CI [-0.83, 0.87], P = 0.96), or implant stability quotient (WMD?= 0.79, 95% CI [-0.03, 1.62], P = 0.06). As for the health status of the peri-implant tissue, conventionally loaded implants demonstrated better performance than did early loaded implants. Subgroup analyses demonstrated that the sample size, time of publication, loading definition, implant position, extent, and restoration type influenced the results. Although early implant loading is convenient and comfortable for patients, this method still cannot achieve the same clinical outcomes as the conventional loading method.

Project description:"Pinhead sperm," or "acephalic sperm," a type of human teratozoospermia, refers to the condition in which ejaculate contains mostly sperm flagella without heads. Family clustering and homogeneity of this syndrome suggests a genetic basis, but the causative genes remain largely unknown. Here we report that Spata6, an evolutionarily conserved testis-specific gene, encodes a protein required for formation of the segmented columns and the capitulum, two major structures of the sperm connecting piece essential for linking the developing flagellum to the head during late spermiogenesis. Inactivation of Spata6 in mice leads to acephalic spermatozoa and male sterility. Our proteomic analyses reveal that SPATA6 is involved in myosin-based microfilament transport through interaction with myosin subunits (e.g., MYL6).

Neurosensory disturbances after immediate loading of implants in the anterior mandible: an initial questionnaire approach followed by a psychophysical assessment.

Project description:The aim of the study was to assess past and present neurosensory disturbances using a questionnaire and a psychophysical approach in patients treated with immediate loaded implants in the edentulous anterior mandible. A group of 65 patients (age range 30-84 years, mean 58 years, 30 women) was enrolled. All were treated by means of three immediately loaded implants (Branemark Novum System). A self-designed questionnaire was used for data collection. The response rate was 89%. Of the 58 responders, 33% (n=19) reported neurosensory disturbances after implant surgery. Nine of these patients (mean age 56 years, seven women) participated in an objective evaluation and were subjected to a psychological and several psychophysical tests. At the moment of the evaluation none of the nine patients still had clinical complaints. Psychological testing revealed no statistical differences between the patients, who had previously experienced subjective complaints, and the control group. Two-point discrimination and thermal sensation tests revealed no sensory lesions. The light touch sensation test at the lower lip indicated a more frequent reduction of tactility for the test group (p<or=0.03). Neurosensory disturbances can occur in the anterior region of the mandible after implant surgery.

Immediate Function of Anodically Oxidized Surface Implants (TiUnite™) for Fixed Prosthetic Rehabilitation: Retrospective Study with 10 Years of Follow-Up.

Project description:Purpose. To report the long-term outcome at 10 years of fixed prosthetic rehabilitation supported by dental implants with anodically oxidized surfaces in immediate function. Materials and Methods. This retrospective cohort study included 75 consecutive patients (44 females and 31 males; 14 bruxers; 21 smokers; 14 systemic compromised), with average age of 60 years, rehabilitated with 264 implants. Outcome measures were implant cumulative survival rates (calculated through life tables) and marginal bone level at 10 years. Results. Twenty-one patients with 66 implants (25%) were lost to follow-up. Six patients lost 12 implants (MkIII implants: n = 9; MkIV implants: n = 3). The overall implant cumulative survival rate at 10 years was 95.2% (maxilla: 95.6%; mandible: 94.7%). The average (standard deviation) marginal bone level at 10 years was 1.96?mm (1.50?mm), with 1.92?mm (1.31?mm) for the maxilla and 2.00?mm (1.71?mm) for the mandible, with a significant difference between nonsmokers (average = 1.60?mm) and smokers (average = 2.95?mm). Conclusions. Within the limitations of this study, it can be concluded that fixed prosthetic rehabilitation supported by implants with anodically oxidized surface in immediate function is a viable and safe treatment option for both jaws.
