Project description:IntroductionCecal duplication cysts occur only in 0.4% of all the gastrointestinal tract duplication cysts. More than 80% cases present in the first two years of life. However, asymptomatic individuals may also present in adult life.Presentation of caseA female patient of 42 years presented with generalized abdominal pain and multiple episodes of vomiting from one day. A vague tender mass was palpable in the left lumber region, firm in consistency with ill-defined borders and not moving with respiration. Ultrasound shows mild free fluid with internal debrinous echoic area noted in lower abdomen and pelvis along with fatty hepatomegaly. CeCT scan of the abdomen and pelvis shows twisted appearance of the gut and mesentery in right sub-hepatic region. Complete resection and Ileocolic anastomosis was done along with right hemicolectomy. Based on radiological, surgical and pathological findings, the final diagnosis was enteric duplication cyst.DiscussionBased on their location, terminal ileum and ileocecal junction are the most common sites (53 %) with colonic duplication second to it (13%). However, cecal duplication cysts remain the least common with incidence of 0.4 % only. Females are more common than males. However, their exact cause is not known. Possible causes are defective recanalization, fusion of embryonal longitudinal folds, persistant diverticulae of embryonic life and uterine vascular anomalies.ConclusionEnteric duplication cysts most commonly presenting with palpable abdominal mass, pain mimicking appendicitis & bleeding per-rectum. The treatment of choice is resection and ileocolic anastomosis with overall good prognosis. The delay in the diagnosis can lead to high mortality.

Project description:IntroductionParagonimiasis, lung fluke disease caused by infection with Paragonimus species, is a food-borne parasitic zoonosis. The overriding symptoms of Paragonimus westermani infection include chronic cough, shortness of breath, and pleuritic pain. Extrapulmonary paragonimiasis caused by aberrant parasitic migration is known to occur in a variety of sites such as the brain, abdominal wall, and intraperitoneal cavity. Ectopic paragonimiasis is an uncommon disease that presents with a few clinical manifestations, which makes it difficult to diagnose and treat.Case presentationA 47-year-old man with an unremarkable medical and surgical history presented with a peritoneal lesion that was discovered incidentally on abdominal computed tomography during routine health screening. The patient did not exhibit any associated symptoms such as abdominal pain. The radiologic diagnosis was a gastric duplication cyst and we performed laparoscopic excision of the peritoneal mass. Histopathological examination revealed paragonimiasis, and the result of the skin test for paragonimiasis was positive. The patient was treated with praziquantel.Clinical discussionThe diagnosis of ectopic peritoneal paragonimiasis remains challenging due to inexperience, misdiagnosis, and its rarity. Clinicians should bear in mind that an intra-abdominal mass may be related to a parasitic infection. The detection of the ova of Paragonimus parasites in sputum and biopsy specimens may be difficult due to an insufficient amount.ConclusionClinicians need to thoroughly take the patient's history and clinically suspect parasitic infections. Laparoscopic resection of this rare mass is safe, feasible, and allows for rapid recovery.

Project description:IntroductionGastric duplication cyst (GDC) is a rare congenital anomaly of the gastrointestinal tract. Though GDC is often misdiagnosed, misidentification as an adrenal cyst has rarely been reported. Herein, we report a case of GDC in a young female mimicking an adrenal cyst.Case presentationA 17-year-old female presented with chronic epigastric pain, nausea, and intermittent vomiting. Physical examinations revealed mild tenderness in the epigastric region. Esophagogastroduodenoscopy showed no abnormality. Ultrasound, contrast-enhanced computed tomography scan, and MRI of the abdomen and pelvis showed an oval-shaped left adrenal cystic lesion measuring 33 × 26 mm. Preoperative blood investigations and hormonal assessments were normal. Laparoscopy showed that the cyst originated from the greater curvature of the stomach. The left adrenal gland was normal. After an intraoperative consultation with a gastrointestinal surgeon, a wedge resection of the cyst was performed. Histopathology confirmed the gastric duplication cyst.ConclusionGDCs are rare congenital malformations that may become symptomatic during adulthood. They can mimic adrenal cysts and lead to misdiagnosis.

Project description:BackgroundIntracranial arachnoid cysts are usually benign congenital findings of neuroimaging modalities, sometimes however, leading to focal neurological and psychiatric comorbidities. Whether primarily clinically silent cysts may become causally involved in cognitive decline in old age is neither well examined nor understood.Case presentationA 66-year old caucasian man presenting with a giant left-hemispheric frontotemporal cyst without progression of size, presented with slowly progressive cognitive decline. Neuropsychological assessment revealed an amnestic mild cognitive impairment (MCI) without further neurological or psychiatric symptoms. The patient showed mild medio-temporal lobe atrophy on structural MRI. Diffusion tensor and functional magnetic resonance imaging depicted a rather sustained function of the strongly suppressed left hemisphere. Amyloid-PET imaging was positive for increased amyloid burden and he was homozygous for the APOEε3-gene. A diagnosis of MCI due to Alzheimer's disease was given and a co-morbidity with a silent arachnoid cyst was assumed. To investigate, if a potentially reduced CSF flow due to the giant arachnoid cyst contributed to the early manifestation of AD, we reviewed 15 case series of subjects with frontotemporal arachnoid cysts and cognitive decline. However, no increased manifestation of neurodegenerative disorders was reported.ConclusionsWith this case report, we illustrate the necessity of a systematic work-up for neurodegenerative disorders in patients with arachnoid cysts and emerging cognitive decline. We finally propose a modus operandi for the stratification and management of patients with arachnoid cysts potentially susceptive for cognitive dysfunction.

Project description:Mesothelial cyst of the spleen is a developmental disorder. Previous reports have elaborated on the radiological features of splenic mesothelial cysts as being unilocular with a smooth and well-defined margin. However, due to its rarity, it is unclear whether these characteristics are representative and specific for the diagnosis of mesothelial cysts. Herein, this case report presents an atypical case of splenic mesothelial cyst mimicking a malignant tumour, especially splenic metastasis in a 66-year-old woman with ascending colon cancer. Due to an overlapping imaging finding of hypodense splenic lesions, and considering the clinical history, a mesothelial cyst that developed as a multilocular hypodense mass from an ill-defined small nodule was inevitably misdiagnosed as metastasis. Although rare, it is important to consider the possibility of mesothelial cyst in a patient with multilocular hypodense lesions of the spleen.

Project description:BackgroundPericardial cysts are rare congenital mediastinal cysts. They are typically asymptomatic and are often discovered incidentally, although some patients may present with chest pain and dyspnoea. Asymptomatic patients are managed conservatively with multiple modalities, with surgical resection often recommended for symptomatic patients only. The frequency of follow-up imaging has yet to be established.Case summaryWe report a case of a 59-year-old female with a gradually increasing pericardial cyst, first noted 10 years prior as an abnormal cardiac silhouette on routine chest radiography. Further evaluation confirmed the presence of a pericardial cyst compressing the left ventricle with new-onset atrial fibrillation. The patient underwent successful thoracoscopic excision of the pericardial cyst under general anaesthesia. The patient's post-operative course was uneventful, and she was ultimately discharged in stable condition.DiscussionPericardial cysts are typically benign, but complications may arise in the case of compression of adjacent cardiac structures, inflammation, haemorrhage, or rupture of the cyst. Magnetic resonance imaging is considered the better modality for both diagnosis and follow-up of pericardial cysts. This case illustrates the need for long-term clinical follow-up in order to optimize the time for treatment.

Project description:BackgroundFat necrosis is a common sequelae of breast trauma. Its many presentations have been described in the literature. Oil cyst is one of the less common presentations of fat necrosis. Giant oil cyst is rare and may take many years to mature in an unstable environment of the women's breast. To our best knowledge, rupture of a giant oil cyst has never been reported. Yet, it should be included on the list of differential diagnoses along with hematoma, infection, or neoplasm. When a patient presents with an expanding breast lump, clinicians should be aware of the potential for oil cyst rupture and its features.Case descriptionA 51-year-old woman presented to a breast clinic with concern of an expanding painless left breast lump for the last 4 months. The lump first appeared 12 years ago shortly after a car accident and was stable for many years. The initial left breast ultrasound was inconclusive, but diagnostic mammogram revealed a giant ruptured calcified oil cyst. Due to the availability of prior diagnostic images, we were able to retrospectively follow the unique sequelae of fat necrosis from the development of giant oil cyst shortly after the breast trauma and up to the final outcome of ruptured calcified oil cyst.ConclusionsTrauma to a woman's breast can lead to the formation of a large oil cyst which can remain present for years, peripherally calcify, and is susceptible to rupture. Clinicians should be aware of this potential complication when presented with the case of expanding breast lump. Diagnostic mammogram is a study of choice and demonstrates benign pathognomonic features of an oil cyst.

Project description:Schwannomas are benign tumors in 95% of cases and very rarely occur in the retroperitoneum. We report the cases of a 35-year-old man with abdominal discomfort and a 50-year-old asymptomatic woman with large retroperitoneal masses. Both underwent multivisceral surgery to exclude an adrenal carcinoma, and the pathologic diagnosis showed schwannomas in both cases. Despite morphological imaging, it was not possible to get a clear diagnosis preoperatively.

Project description:Introduction A nasolabial cyst is an ectodermal development cyst. It presents as a fullness of canine fossa, nasal ala, or vestibule of the nose. It is rare and usually small. Treatment consists of complete surgical excision or transnasal endoscopic marsupialization. Objective To describe a giant nasolabial cyst case treated using Neumann incision. Case Report A 37-year-old man was referred to the otolaryngology department with nasal obstruction and nasal deformity. Computed tomography showed a nasal cystic lesion 4 × 4.5 × 5 cm wide. Surgical excision using Neumann incision was performed. Discussion Neumann incision provides wide access to the nasal cavity and may be useful in nasolabial cyst treatment.

Project description:Smooth muscle neoplasms of the vulva are rare, and when localized in the Bartholin's gland, it can be mistaken as a benign lesion leading to a delay in diagnosis. We report a case of leiomyosarcoma of the Bartholin's gland in a 63-year-old postmenopausal woman, which clinically mimicked a chronic Bartholin's gland cyst. This case report emphasizes the importance of suspecting malignancy in any vulvar lesion in women of postmenopausal age, despite its location and asymptomatic appearance. A prompt, careful evaluation with adequate clinical judgment will help to manage such cases efficiently.
