The mis-measurement of extreme global poverty: A case study in the Pacific Islands.

ABSTRACT: Debate over the measurement of global poverty in low- and middle-income countries continues unabated. There is considerable controversy surrounding the 'dollar a day' measure used to monitor progress against the Millennium Development Goals. This article shines fresh light on the debate with new empirical analyses of poverty (including child poverty), inequality and deprivation levels in the Pacific island state of Vanuatu. The study focuses not only on economic and monetary metrics and measures, but also the measures of deprivation derived from sociology in relation to shelter, sanitation, water, information, nutrition, health and education. Until recently, there had been few, if any, attempts to study poverty and deprivation disparities among children in this part of the world. Different measures yield strikingly different estimates of poverty. The article, therefore, attempts to situate the study findings in the broader international context of poverty measurement and discusses their implications for future research and the post-2015 development agenda.

Project description:BACKGROUND:From late 2014 multiple atolls in Kiribati reported an unusual and sometimes fatal illness. We conducted an investigation to identify the etiology of the outbreak on the most severely affected atoll, Kuria, and identified thiamine deficiency disease as the cause. Thiamine deficiency disease has not been reported in the Pacific islands for >5 decades. We present the epidemiological, clinical, and laboratory findings of the investigation. METHODOLOGY/PRINCIPAL FINDINGS:We initially conducted detailed interviews and examinations on previously identified cases to characterize the unknown illness and develop a case definition. Active and passive surveillance was then conducted to identify additional cases. A questionnaire to identify potential risk factors and blood samples to assay biochemical indices were collected from cases and asymptomatic controls. Thiamine hydrochloride treatment was implemented and the response to treatment was systematically monitored using a five-point visual analogue scale and by assessing resolution of previously abnormal neurological examination findings. Risk factors and biochemical results were assessed by univariate and multivariate analyses. 69 cases were identified on Kuria (7% attack rate) including 34 confirmed and 35 unconfirmed. Most were adults (median age 28 years [range 0-62]) and 83% were male. Seven adult males and two infants died (13% case fatality rate). Resolution of objective clinical signs (78%) or symptoms (94%) were identified within one week of starting treatment. Risk factors included having a friend with thiamine deficiency disease and drinking kava; drinking yeast alcohol reduced the risk of disease. Higher chromium (p<0·001) but not thiamine deficiency (p = 0·66) or other biochemical indices were associated with disease by univariate analyses. Chromium (p<0·001) and thiamine deficiency (p = 0·02) were associated with disease by multivariate analysis. CONCLUSIONS/SIGNIFICANCE:An outbreak of thiamine deficiency disease (beriberi) in Kiribati signals the re-emergence of a classic nutritional disease in the Pacific islands after five decades. Although treatment is safe and effective, the underlying reason for the re-emergence remains unknown. Chromium was highly and positively correlated with disease in this study raising questions about the potential role of factors other than thiamine in the biochemistry and pathophysiology of clinical disease.

Project description:BackgroundPacific Islanders, including those residing in the US Affiliated Pacific Islands (USAPI), experience some of the highest mortality rates resulting from non-communicable diseases (NCDs) worldwide. The Pacific Island Health Officers' Association declared a Regional State of Health Emergency in 2010 due to the epidemic of NCDs in the USAPI. Obesity, a known risk factor for NCDs, has become an epidemic among both children and adults in Micronesia and other parts of the USAPI. There is some recent information about overweight and obesity (OWOB) among young children in the USAPI, but there is no data looking at the relationship between children and their biological parents. The Pacific Islands Cohort on Cardiometabolic Health (PICCAH) Study aims to collect data on NCD lifestyle factors from two generations of families (n = 600 child-parent dyads or 1,200 participants) living in Guam, Pohnpei, and Palau.MethodsThe PICCAH Study is an epidemiological study using community-based convenience sampling to recruit participants in USAPI of Guam, Palau, and Pohnpei. The goal is to recruit participant dyads consisting of 1 child plus their biological parent in Guam (500 dyads or 1,000 participants), Pohnpei (50 dyads or 100 participants), and Palau (50 dyads or 100 participants). All participants are having the following information collected: demographic, health, and lifestyle information; anthropometry; diet; physical activity; sleep; acanthosis nigricans; blood pressure; and serum levels of fasting plasma glucose, fasting insulin, glycated hemoglobin, total cholesterol, triglycerides, LDL, and HDL.DiscussionThe PICCAH Study is designed to establish the baseline of a generational epidemiologic cohort with an emphasis on cardiometabolic risk, and to better understand the extent of DM and CVD conditions and related risk factors of those living in the USAPI jurisdictions of Guam, Pohnpei, and Palau. This study also serves to further build research capacity in the underserved USAPI Region.

Project description:BackgroundTrachoma is widely considered a disease of poverty. Although there are many epidemiological studies linking trachoma to factors normally associated with poverty, formal quantitative data linking trachoma to household economic poverty within endemic communities is very limited.Methodology/principal findingsTwo hundred people with trachomatous trichiasis were recruited through community-based screening in Amhara Region, Ethiopia. These were individually matched by age and gender to 200 controls without trichiasis, selected randomly from the same sub-village as the case. Household economic poverty was measured through (a) A broad set of asset-based wealth indicators and relative household economic poverty determined by principal component analysis (PCA, (b) Self-rated wealth, and (c) Peer-rated wealth. Activity participation data were collected using a modified 'Stylised Activity List' developed for the World Bank's Living Standards Measurement Survey. Trichiasis cases were more likely to belong to poorer households by all measures: asset-based analysis (OR = 2.79; 95%CI: 2.06-3.78; p<0.0001), self-rated wealth (OR, 4.41, 95%CI, 2.75-7.07; p<0.0001) and peer-rated wealth (OR, 8.22, 95% CI, 4.59-14.72; p<0.0001). Cases had less access to latrines (57% v 76.5%, p = <0.0001) and higher person-to-room density (4.0 v 3.31; P = 0.0204) than the controls. Compared to controls, cases were significantly less likely to participate in economically productive activities regardless of visual impairment and other health problems, more likely to report difficulty in performing activities and more likely to receive assistance in performing productive activities.Conclusions/significanceThis study demonstrated a strong association between trachomatous trichiasis and relative poverty, suggesting a bidirectional causative relationship possibly may exist between poverty and trachoma. Implementation of the full SAFE strategy in the context of general improvements might lead to a virtuous cycle of improving health and wealth. Trachoma is a good proxy of inequality within communities and it could be used to target and evaluate interventions for health and poverty alleviation.

Project description:Protecting key hotspots of marine biodiversity is essential to maintain ecosystem services at large spatial scales. Protected areas serve not only as sources of propagules colonizing other habitats, but also as receptors, thus acting as protected nurseries. To quantify the geographical extent and the temporal persistence of ecological benefits resulting from protection, we investigate larval connectivity within a remote archipelago, characterized by a strong spatial gradient of human impact from pristine to heavily exploited: the Northern Line Islands (NLIs), including part of the Pacific Remote Islands Marine National Monument (PRI-MNM). Larvae are described as passive Lagrangian particles transported by oceanic currents obtained from a oceanographic reanalysis. We compare different simulation schemes and compute connectivity measures (larval exchange probabilities and minimum/average larval dispersal distances from target islands). To explore the role of PRI-MNM in protecting marine organisms with pelagic larval stages, we drive millions of individual-based simulations for various Pelagic Larval Durations (PLDs), in all release seasons, and over a two-decades time horizon (1991-2010). We find that connectivity in the NLIs is spatially asymmetric and displays significant intra- and inter-annual variations. The islands belonging to PRI-MNM act more as sinks than sources of larvae, and connectivity is higher during the winter-spring period. In multi-annual analyses, yearly averaged southward connectivity significantly and negatively correlates with climatological anomalies (El Niño). This points out a possible system fragility and susceptibility to global warming. Quantitative assessments of large-scale, long-term marine connectivity patterns help understand region-specific, ecologically relevant interactions between islands. This is fundamental for devising scientifically-based protection strategies, which must be space- and time-varying to cope with the challenges posed by the concurrent pressures of human exploitation and global climate change.
