Project description:This case describes a 57-year-old morbidly obese woman with complications of a permanent Bard inferior vena cava (IVC) filter (BD Bard, Covington, Ga) placed in 2005. She complained of recurrent abdominal pain for 6 years associated with constipation, nausea, and bloating. Computed tomography venography showed an IVC filter with malposition, strut fracture, and extrusion, and a fractured strut protruded outside of the vena cava, adjacent to the distal right common iliac artery. The da Vinci Xi Surgical System (Intuitive Surgical, Sunnyvale, Calif) was used to remove the IVC filter and its displaced fractured strut. The operation time was 189 minutes, with an estimated blood loss of 200 mL. She was discharged home on postoperative day 2 with no complications. She had complete resolution of abdominal pain at her follow-up appointment.

Project description:BackgroundThis article describes a rare case of inferior vena cava (IVC) filter perforation into the duodenum in a patient presenting with abdominal pain.Case reportA 55 year old woman presented with abdominal pain four years after an IVC filter placement. Workup demonstrated an IVC filter strut perforating the duodenum. The filter was removed via laparotomy, the duodenum was closed primarily, and the IVC was repaired. The patient was discharged home on post-operative day five and is doing well.ConclusionsMost extraluminal perforations of IVC filter struts are asymptomatic. Rare filter associated duodenal perforations may present with non-specific abdominal symptoms. If no other diagnosis can be attributed to the patient's presentation, direct removal of the filter and repair of the duodenum are indicated.

Project description:BackgroundInferior vena cava thrombosis is a rare blunt abdominal trauma complication often associated with severe liver injury. We present two cases of inferior vena cava thrombosis due to mild liver injuries.Case presentationCase 1 was a 25-year-old woman taking oral contraceptives for dysmenorrhea who was injured in a motorcycle accident. Contrast-enhanced computed tomography revealed hepatic contusion of the sixth segment. At 1 week after the accident, inferior vena cava thrombosis was detected. Case 2 was a 58-year-old man injured in a motorcycle accident. Contrast-enhanced computed tomography showed traumatic subarachnoid hemorrhage, right hemothorax, and liver injury with hepatic contusion of the sixth segment. At 1 week after the accident, inferior vena cava thrombosis was observed.ConclusionInferior vena cava thrombosis can occur following liver injury, regardless of damage severity. When there are thrombogenic factors and damage near the inferior vena cava, follow-up examinations should be carried out.

Project description:PurposeTo compare filter tilt and filter jumping during Option inferior vena cava (IVC) filter deployment with 3 different wires techniques using a 3-dimensional (3D) printing vena cava phantom.Materials and methodsAn IVC 3D printed vena cava phantom was made from a healthy young male's computed tomographic data. Option IVC filters were deployed with 3 different wires: i) original push wire, ii) hydrophilic stiff wire, and iii) bent stiff wire. Right internal jugular and right femoral access were used 5 times with each wire. Filter tilt angle, tilt ratio, jumping, and tip abutment to the IVC wall were analyzed.ResultsThe transfemoral approach with original push wire had significantly higher tilt angle than did the transjugular approach (6.1˚ ± 1.9 vs. 3.5˚ ± 1.3, p = 0.04). Mean tilt ratio was significantly lower with the bent wire with transfemoral access (0.49 ± 0.13 vs. 0.78 ± 0.18 [original push-wire] and 0.67 ± 0.08 [stiff wire], p = 0.019). The ratio was lower also with original push wire with transjugular access (0.34 ± 0.19 vs. 0.57 ±0.11 [stiff wire] and 0.58 ±0.17 [bent wire], p = 0.045). Filter jumping occurred more often with the transjugular approach with original push wire than with stiff or bent-wire delivery. Filter tip abutment to the IVC wall occurred only with the transfemoral approach.ConclusionsBent wire with transfemoral access and original push wire with transjugular access had lower filter tilt ratio at Option IVC filter deployment. However, filter jumping was common using the original push wire with transjugular access.

Project description:Annually, approximately 65,000 inferior vena cava (IVC) filters are placed in the United States (Ahmed et al., J Am Coll Radiol 15:1553-1557, 2018). Approximately 35% of filters are eventually retrieved (Angel et al., J Vasc Interv Radiol 22: 1522-1530 e1523, 2011). Complications during filter retrieval depend heavily on technique and filter position. In this paper, we review risk factors and incidence of complications during IVC filter removal. We also discuss ways these complications could be avoided and the appropriate management if they occur.

Project description:Background: Previous observational studies suggest that inferior vena cava filter placement in pulmonary embolism patients complicated with congestive heart failure, mechanical ventilation, and shock may have a mortality benefit. We sought to analyze the survival benefits of inferior vena cava filter in pulmonary embolism patients complicated with acute myocardial infarction, acute respiratory failure, shock, or requiring treatment with thrombolytics. Methods: This retrospective observational study used hospital discharge data from the National Inpatient Sample Data (NIS). ICD-9-CM coding was used to identify complicated pulmonary embolism patients (N = 254,465) in NIS from 2002 to 2014, including the subgroups of acute myocardial infarction, acute respiratory failure, shock, and thrombolytics. Inferior vena cava filter recipients were 1:1 propensity score-matched on age, sex, race, deep vein thrombosis, Elixhauser comorbidities, and other pulmonary embolism comorbidities (45 covariates) to non-inferior vena cava filter recipients in complicated pulmonary embolism patients and separately in each subgroup. Clinical outcomes were compared between the inferior vena cava filter group and the non-inferior vena cava filter group. Results: Mortality rate in complicated pulmonary embolism patients with inferior vena cava filter placement was lower (20.9% vs. 33%; NNT = 8.28, 95% confidence interval (CI) 7.91-8.69, E-value = 2.53) and in the subgroups; acute myocardial infarction (17.9% vs. 30.1%; NNT = 8.19, 95% CI 7.52-8.92, E-value = 2.76), acute respiratory failure (19.5% vs. 29.7%; NNT = 9.76, 95% CI 8.67-11.16, E-value = 2.38), shock (30.7% vs. 47.1%; NNT = 6.08, 95% CI 5.73-6.47, E-value = 2.43), and with the use of thrombolytics (7% vs. 12.9 %; NNT 17.1, 95% CI 14.88-20.12, E-value = 3.01) (p < 0.001 for all). Conclusion: Inferior vena cava filter placement in pulmonary embolism complicated with acute myocardial infarction, acute respiratory failure, shock, or requiring thrombolytic therapy was associated with reduced mortality.

Project description:A 45-year-old man, who was implanted with an inferior vena cava (IVC) filter in his infrarenal IVC, had a complication of deep vein thrombosis (DVT) propagation from the IVC, beyond the IVC filter, to the right calf and left external iliac veins. The entire IVC filter was covered with a massive thrombus. We first decided to retrieve the IVC filter itself, which was suspected of causing metallic allergy. The thrombotic IVC filter was successfully retrieved using multi-step catheter intervention. To our knowledge, this is the first case report to describe successful multi-step catheter intervention for retrieval of an IVC filter covered with a massive thrombus. <Learning objectives: Inferior vena cava (IVC) filter thrombosis is one of the most serious IVC filter complications. It is difficult to retrieve the IVC filter covered with a massive thrombus. Multi-step catheter intervention technique may be useful to retrieve an IVC filter with thrombotic occlusion.>.

Despite declared effectiveness in prevention of pulmonary embolism, certain IVC filter-related complications have been described. This case report deals with successful endovascular retrieval of an IVC filter penetrating into the aorta.Learning objectiveThe use of inferior vena cava (IVC) filters has been associated with controversy in recent years, largely owing to concerns about the overuse. The perforation of IVC wall and further penetration of IVC filter struts into surrounding tissues belong to the most severe complication. The purpose of this report is to highlight potential severe complications associated with the use of IVC filter and to present the reader how the IVC filter-related complications can be successfully managed by endovascular treatment.

Project description:Inferior vena cava thrombosis (IVCT) is a rare complication of deep vein thrombosis (DVT) in the legs. The etiology of IVC thrombosis can be divided into genetic versus nongenetic (environmental/acquired) etiological factors found in Virchow's triad of stasis, endothelial injury, and hypercoagulability. Hyperhomocysteinemia is a rare risk factor for venous thromboembolism and atherosclerotic disease. Hence, very few cases have been described to date to knowledge. A case of a 78-year-old hypertensive man who presented with hyperhomocysteinemia-induced DVT of the left leg extending toward an anatomically normal IVC is discussed in the report.

Project description:BackgroundInferior vena cava filter (IVCF) thrombosis is an uncommon complication of IVCF utilization. The aims of this study were to investigate inferior vena cava (IVC) venography before filter retrieval to determine the incidence relative to filter dwell time and risk factors of IVCF thrombosis based on the clinical data and imaging findings in patients with lower extremity deep vein thrombosis (LEDVT).MethodsThe clinical data from a multicenter randomized trial conducted between October 2017 and March 2019 were reviewed to determine the incidence of IVCF thrombosis in preretrieval venography and the associated risk factors. The correlation between filter dwell times (within 90 days) and incidence was assessed. Baseline demographics, LEDVT presentation, laboratory examination, thrombus characteristics, concurrent pulmonary embolism (PE), comorbidities and risk factors for LEDVT, and IVCF-relevant information were analyzed using the independent samples t-test, chi-squared test, Fisher exact test, and regression analysis to determine the univariable and multivariable associations in assessing the risk factors of IVCF thrombosis.ResultsA total of 178 eligible patients were included, of whom 58 were in the IVCF thrombosis group and 120 were in the IVCF nonthrombosis group, and the mean filter dwell time was 22.07±27.91 days (range, 4-190 days). The overall incidence of IVCF thrombosis in patients with LEDVT who received IVCFs was 32.58% (58/178). The incidence of IVCF thrombosis was 35.25% (49/139) in the first 30 days after the IVCF placement and decreased to 22.73% (5/22) between 30 to 60 days of dwell time and to 18.18% (2/11) between 60 and 90 days of dwell time, indicating a decreasing trend within the first 90 days. The risk factors for the occurrence of IVCF thrombosis were concurrent PE [odds ratio (OR) =2.59; 95% confidence interval (CI): 1.27-5.28; P=0.01], rheumatic diseases of the immune system (OR =14.42; 95% CI: 1.52-136.41; P=0.02), IVC:filter radial ratio >0.587 (OR =0.25; 95% CI: 0.10-0.65; P<0.01), and percutaneous angioplasty (PTA) (OR =2.50; 95% CI: 1.09-5.70; P=0.03).ConclusionsThe incidence of IVCF thrombosis at the time of filter retrieval appears to decrease with dwell time within 90 days. Concurrent PE, rheumatic diseases of the immune system, and PTA were taken into account as risk factors. An IVC:filter radial ratio of 0.587 was a protective factor against developing IVCF thrombosis. These findings require further validation in a well-designed study since the present study lacked a close follow-up.
