Project description:Conjunctival amyloidosis is a very rare disease, and its presence may be a sign of systemic amyloidosis. We present our ocular and systemic findings in a patient with conjunctival amyloidosis. A 43-year-old man had repeated subconjunctival hemorrhages (SCHs) for two years and was referred to the Chiba University Hospital. He had comprehensive ophthalmological and systemic examinations to determine the cause of the SCHs. His visual acuities were 1.2 OU, and the intraocular pressures were 13-14 mmHg OU. Magnetic resonance imaging was normal. Initially, the SCH was the only abnormality. After 3 months, the SCH had partially cleared, and a pink mass was detected in the superior area of the subconjunctiva. Partial biopsy and histopathological examinations showed a greenish birefringence and dichroism under polarized light illumination. The birefringence was located in amyloid fibers. Immunofixation electrophoresis detected λ-light chain abnormality in the ocular biopsy specimen but systemic examinations did not find any lesions. Multiple myeloma was ruled out, and the patient is being followed closely to detect any early signs of systemic amyloidosis. Because repeated SCHs might be initial signs of systemic amyloidosis, patients with conjunctival amyloidosis should be comprehensively examined for systemic amyloidosis because of its poor life prognosis.

Project description:Intratumoral hemorrhage in vestibular schwannomas is rare. Symptoms often have an acute onset and include headache, nausea, vomiting, vertigo, and depressed consciousness. Intratumoral hemorrhage is probably caused by vascular fragility associated with tumor characteristics and growth. With hemorrhage in VS being rare, repeated hemorrhage has only been reported twice, and on clinical grounds only. The present report details the case of acute neurological deterioration in a patient with repeated intratumoral hemorrhage inside a vestibular schwannoma with computed tomography and magnetic resonance imaging confirmation. To our knowledge, repeated hemorrhage in vestibular schwannoma with radiological confirmation has not been reported before.

Project description:As one of the common neurological diseases, pediatric middle fossa arachnoid cysts(MFACs) can develop intracystic hemorrhage and subdural hematoma. Risk factors for pediatric arachnoid cyst rupture/hemorrhage is very complicated in mechanism. Although surgery is the first choice for children with MFACs and subdural hematoma, the rate of recurrence of the subdural hematoma is very high after 1 or more surgeries. Atorvastatin has proven to be a bold and safe choice in the management of subdural hematoma with mild symptoms. The present study has described a 7-year-old child with a recurrent rupture of arachnoid cyst develops into a subdural hematoma. We demonstrate that atorvastatin is safe and effective in pediatric patient who has failed surgical treatment of middle fossa arachnoid cyst and subdural hematoma. The patient received atorvastatin monotherapy, once daily for the first week, with an initial dose of 5 mg, followed by 10 mg once daily for 7 weeks. In the third month after the initial treatment, the neurological function recovered, and the hematoma completely resolved. This case report supports the concept that atorvastatin can promote the absorption of subdural hematoma.

Project description:Hydatid infection of the heart is rare and there is always the lethal hazard of cyst perforation. We present an 18-year-old male from Kashmir valley who was admitted to the emergency department of our hospital with fever and chest pain for the last 4 days. Using echocardiography and cardiac tomography (CT), cardiac Echinococcosis was diagnosed. The results of surgical treatment of cardiac Echinococcosis were better than the conservative strategy. Surgical excision was performed. The patient had an uneventful recovery.

Project description:This article explores how brucellosis became a racialized disease in Israel, where almost all patients are Palestinians. Informed by legal and historical research, the article demonstrates how colonial and settler-colonial policies have targeted Palestinians and their goats and contributed to the distribution of brucellosis along ethno-national lines. Goats, once ubiquitous to the landscape, became enemies of the Israeli state and were blamed for the "destruction" of nature. Under Israeli rule, legal policies not only seized and confiscated Palestinian land but also targeted goat grazing and led to a steep reduction in the number of goats. The resulting depeasantization and concentration of Palestinians in dense poor townships shaped goat grazing as a backyard practice with lack of trust in the hostile state and its brucellosis eradication campaigns. We argue that state policies of organized violence and organized abandonment have shaped the current ecology of brucellosis as a racialized disease. IMPORTANCE The importance of this article is the novelty in combining public health, colonial studies, and legal research to understand the ecology of human brucellosis. This approach allows us to move from a "snap-shot" reading of diseases and cultural practices toward a reading of bacteria, animals, and humans within their political and historical context. The article uses a settler colonial lens to examine the racialized distribution of human brucellosis in Israel and traces colonial policies toward Palestinians and goats-both seen as unwanted intruders to the newly established Israeli nation state. We place these policies in a context of organized violence and organized abandonment, building on the work of Ruth Wilson Gilmore to read the power hierarchies of humans, animals, and diseases and how they shape practices and disease.

Project description:Testicular epidermoid cysts are a rare cause of testicular pathology. No cases of recurrence or metastasis have been reported in the literature. As a result, inguinal partial orchiectomy with frozen section has recently become standard treatment. A 43-year-old male presented with right testicular discomfort and the presence of mass. Right inguinal partial orchiectomy with frozen section was performed, and the right testicle was preserved. The final pathology report confirmed the diagnosis of an epidermoid cyst. The importance of accurate diagnosis of this benign lesion is crucial for the prevention of unnecessary radical orchiectomy.

Project description:IntroductionNon-parasitic spleen cyst (NSC) is a relatively rare and difficult to diagnose disease, which rapture due to a traumatic impact to the spleen, can lead to the peritonitis.Case presentationWe present the case of a 30-year-old woman with a 7.5 × 7.5 × 5 cm NSC, who underwent the microwave ablation (MWA) of the splenic cyst.ResultsThe procedure was performed under intravenous anesthesia. MWA of the cysts was performed using a probe placed in the cavity of the cyst, with the frequency of 902-928 MHz delivered during 15 minutes. The postoperative period was uneventful, and the patient was discharged after 2 days.ConclusionWe demonstrated that MWA can be utilized as a novel, minimally-invasive, and cost-effective approach in NSC treatment.

Project description:IntroductionThe cervical lymphoepithelial or branchial cleft cyst are benign dysembryologic cystic tumors developing in the anterolateral region of the neck. They are relatively uncommon anomalies.The aims of this study are to analyze the anatomoclinical features and to discuss the modalities of care for the management of this disease.Presentation of caseWe report a case of a 70 years-old woman who was admitted to our department with a complaint of painless mass in the right supraclavicular region. Clinical examination and radiological investigations found a mass compatible with cervical cyst. Treatment consisted of the complete resection of the cyst. histopathological examination found a Cervical lymphoepithelial cyst.ConclusionThe cervical lymphoepithelial can be easily misdiagnosed. It is imperative that clinicians make an accurate diagnosis for appropriate treatment (that is, surgical excision).

Project description:BackgroundNeurenteric cysts are rare congenital lesions of endodermal origin which result from the failure of the neurenteric canal to close during embryogenesis. The majority of neurenteric cysts occur in the spinal cord, though in rare instances can occur intracranially, typically in the posterior fossa anterior to the pontomedullary junction (80%) or in the supratentorial region adjacent to the frontal lobes (20%).Case descriptionWe present the case of a 75-year-old woman with an extra-axial cystic lesion centered in the premedullary cistern causing brainstem compression. The lesion was later histopathologically confirmed to be a neurenteric cyst. She presented initially with a 4-month history of worsening headache, dizziness, and unsteady gait. We performed a left retrosigmoid craniotomy for cyst fenestration/biopsy with the aid of operating microscope and stealth neuronavigation. Following the procedure, the patient recovered without complications or residual deficits.ConclusionThis case illustrates the successful fenestration of an intracranial neurenteric cyst with good clinical outcome. We present the pre- and post-operative imaging findings, a technical video of the procedure, histopathological confirmation, and a brief review of the relevant clinical literature on the topic.

Project description:Hepatic cystadenoma is an urgent problem due to the high risk of malignant transformation. There are both radical and minimally invasive methods of treatment. We present a clinical case describing the successful use of microwave ablation (MWA) in a 72-year-old woman with hepatic cystadenoma. The patient was admitted to the clinic with abdominal discomfort, dull pain in the right hypochondrium, and weight loss of 10 kg during the previous year. The patient had a past medical history of liver cyst in segment VII. Ultrasound scanning, computed tomography (CT) of abdominal organs detected an increase in the size of the cyst, heterogeneity of its structure; the contrast enhancement was noted in the cyst wall. We suspected hepatic cystadenoma in segment VII and performed cyst puncture under ultrasound control - the obtained fluid revealed the presence of cylindrical epithelial cells, mucin, and macrophages in large quantities, high telomerase activity, CA 19-9 levels were greater than 1000 U/mL. Surgery was accomplished using the MWA catheter. Biological and cytological examination of the fluid confirmed the presence of signs of liver cystadenoma with a high malignancy risk. On the 2nd day after surgery ultrasound imaging of the abdomen revealed the residual cavity of 2 × 1 cm in segment VII. The patient was discharged with recommendations to conduct ultrasound examinations every six months. The control CT scan in 2020 showed no focal or cystic formations in the liver. In what way, MWA under control of ultrasound is a promising method of biliary cystadenoma treatment with high malignancy risk.
