Project description:The main objective of this paper is to understand the role of the family on the sexual experiences of adolescents from urban, disadvantaged settings in Baltimore and Johannesburg. Data were collected as part of the WAVE study, a global study of disadvantaged youth in five cities. Qualitative data were based on key informant interviews, a Photovoice exercise, community mapping, focus groups and in-depth interviews with adolescents. Quantitative data were gathered from an ACASI survey that was administered to approximately 450-500 adolescents per site. Results from the qualitative data revealed that while parents were viewed as important sources of information for sexual and reproductive health, they were often not present in the adolescents' lives. This lack of parental presence was perceived to result in adolescents feeling an overall lack of adult support and guidance. The impact of parental presence and support on adolescent sexual experience was further examined from the quantitative data and revealed a complex picture. In both Baltimore and Johannesburg, female adolescents who were raised by other relatives were less likely to report having had sex compared to those raised by two biological parents, which was not observed for males. In Johannesburg, female adolescents who were paternal orphans were less likely to have had sex compared to non-orphans; the opposite was true among males. Finally, in both sites, female adolescents who had been exposed to violence were more likely to have had sex compared to those who had not; for males, there was no significant relationship. The study demonstrates the powerful influence of both context and gender for understanding the influences of the family on adolescent sexual behaviors. Programs aiming to reduce adolescent sexual risk behaviors the need to understand the complex influences on risk behaviors in different settings and in particular, the role of mothers and fathers. Prevention strategies need to also understand and incorporate gender-specific messages and interventions in order to address the high risk of sexual behaviors among adolescents in these settings.

Project description:Friendships form an important context in which adolescents initiate and establish alcohol use patterns, but not all adolescents may be equally affected by this context. Therefore, this study tests whether parenting practices (i.e., parental discipline, parental knowledge, unsupervised time with peers) and individual beliefs (i.e., alcohol descriptive norms, positive social expectations, moral approval of alcohol use) moderate friend selection and influence around alcohol use. Stochastic actor-based models were used to analyze longitudinal social network and survey data from 12,335 adolescents (aged 11 to 17, 51.3% female) who were participating in the PROSPER project. A separate model was estimated for each moderating variable. Adolescents who reported consistent parental discipline, less unsupervised time with peers, higher descriptive alcohol use norms, and less positive social expectations about alcohol use were less likely to select alcohol-using friends. Those who reported consistent parental discipline, better parental knowledge, lower descriptive alcohol use norms, and less positive social expectations were more influenced by their friends' level of alcohol use. Thus, adolescents with these characteristics whose friends frequently use alcohol are at greater risk whereas those whose friends do not use alcohol are at lower risk of using alcohol. The findings show that, although selection and influence processes are connected, they may function in different ways for different groups of adolescents. For some adolescents, it is particularly important to prevent them from selecting alcohol-using friends, because they are more susceptible to influence from such friends. These peer network dynamics might explain how proximal outcomes targeted by many prevention programs (i.e., parenting practices and individual beliefs) translate into changes in alcohol use.

Project description:The purpose of this study was to examine the direct and indirect ecological influences of teacher and friend social support on body weight and diet behaviors in African-American adolescent females. Using a quantitative, cross-sectional research design, a convenience sample of 182 urban African-American adolescent females (12-17 years old) completed a 39-item questionnaire. The questionnaire assessed perceived teacher social support, friend social support, nutrition self-efficacy, and diet behaviors (with internal reliability values of scale items: alpha?=?0.74, 0.81, 0.77, and 0.69 respectively). Anthropometric assessments were conducted to measure height and weight to compute BMI. Majority of the participants were in middle or early high school (65 %) and were overweight or obese (57.7 %). Both teacher social support and friend social support demonstrated a positive, indirect influence on child weight status through nutrition self-efficacy and diet behaviors following two different and specific paths of influence. Diet behaviors, in turn, demonstrated a positive, direct effect on child weight status. In the structural model, teacher social support had the greatest effect on diet behaviors, demonstrating a direct, positive influence on diet behaviors (B?=?0.421, p?<?0.05), but its direct effect on nutrition self-efficacy was not significant. Friend social support demonstrated a positive, direct effect on nutrition self-efficacy (B?=?0.227, p?<?0.05), but its direct effect on diet behaviors was not statistically significant. The study's findings call for actively addressing the childhood obesity epidemic in the school environment by implementing health behavior change strategies at various social and ecological environmental levels.

Project description:ImportanceParent-adolescent sexual communication has received considerable attention as a factor that can positively affect safer sex behavior among youth; however, the evidence linking such communication to youth contraceptive and condom use has not been empirically synthesized.ObjectivesTo examine the effect of parent-adolescent sexual communication on safer sex behavior among youth and explore potential moderators of this association.Data sourcesA systematic search of studies published from database inception through June 30, 2014, using the MEDLINE, PsycINFO, and Communication & Mass Media Complete databases and relevant review articles yielded 5098 studies, of which 52 studies with 25,314 adolescents met the study eligibility criteria. Analysis was conducted from July 1, 2014, to July 27, 2015.Study selectionStudies were included if they sampled adolescents (mean sample age ≤18 years), included an adolescent report of sexual communication with one or both parents, measured safer sex behavior, and were published in English.Data extraction and synthesisCorrelation coefficients (r) and 95% CIs were computed from studies and meta-analyzed using random-effects models.Main outcomes and measuresSafer sex behavior, including use of contraceptives or condoms.ResultsFifty-two articles, including 71 independent effects representing more than 3 decades of research on 25,314 adolescents (weighted mean age, 15.2 years) were synthesized. Across studies, there was a significant weighted mean effect (r = 0.10; 95% CI, 0.08-0.13) linking parent-adolescent sexual communication with safer sex behavior, which was statistically heterogeneous (Q = 203.50, P < .001, I2 = 65.60). Moderation analyses revealed larger effects for communication with girls (r = 0.12) than boys (r = 0.04) and among youth who discussed sex with their mothers (r = 0.14) compared with their fathers (r = 0.03). Effects did not differ for contraceptive vs condom use or among longitudinal vs cross-sectional studies, indicating that parent sexual communication had a similar effect across study designs and outcomes. Several methodological issues were identified in the literature; future studies can improve on these issues by measuring parent-adolescent communication with robust, multi-item measures, clearly specifying the target parent, and applying multimethod longitudinal designs.Conclusions and relevanceSexual communication with parents, particularly mothers, plays a small protective role in safer sex behavior among adolescents; this protective effect is more pronounced for girls than boys. We discuss the implications for practice and make suggestions for future research on parent-adolescent sexual communication.

Project description:PURPOSE:Evidence suggests that female adolescents and those living in urban environments may have lower physical activity (PA) levels compared with their peers. Yet few studies report PA for urban adolescent females, and there is no consensus regarding potential causes for low PA in this subgroup. We examined PA levels, in a large, diverse cohort of 14- to 17-yr-old urban-dwelling females and assessed the effect of socioeconomic, personal, and neighborhood/environmental factors on PA. METHODS:One week of time-stamped step count data were collected on 926 girls from the Pittsburgh Girls Study at four annual visits. Valid recordings (worn at least 10 h on 3+ d) were examined and compared with normalized step count values from a U.S. population-representative sample. Relationships between important covariates and average steps per day were examined with regression models. RESULTS:Adjusted mean ± SD step counts per day at baseline were 5614 ± 2434 after controlling for important covariates with less than 6% of girls achieving at least 10,000 steps per day. The girls from the Pittsburgh Girls Study accrued ~45% of their steps during school hours. Age-specific median step counts per day for study participants were similar to the 25th percentile of U.S. population normalized values and did not significantly change during follow-up. Non-Hispanic African American race/ethnicity was associated with higher average step counts per day; obesity and a recent childbirth were associated with lower average step counts per day. CONCLUSIONS:Step counts in this cohort of urban adolescent girls were considerably lower than expected for U.S. adolescent females. Targeted efforts to improve PA levels in urban youth should consider the importance of school-based activity while increasing PA opportunities outside of school.

Project description:ObjectivesFood insecurity contributes to racial/ethnic disparities in health. This is the first study to examine associations among food insecurity, sleep, and cardiometabolic outcomes in urban American Indian/Alaska Native (AI/AN) youth.DesignParticipants were 142 urban AI/AN youth (mean age = 14 years, 58% female). Food insecurity and self-reported sleep disturbance were measured using validated surveys. A multi-dimensional sleep health composite was derived using questionnaires (ie, satisfaction, alertness) and actigraphy-derived indices (ie, duration, efficiency, regularity, timing). Cardiometabolic measures included body mass index, blood pressure, glycosylated hemoglobin, waist circumference, cholesterol, and triglycerides. Covariates were sex, age, and single-parent household.ResultsGreater food insecurity was significantly associated with greater body mass index (b = 0.12, p = .015), higher systolic blood pressure (b = 0.93, p = .03), and greater sleep disturbance (b = 1.49, p < .001), and marginally associated with lower sleep health composite scores (b = -0.09, p = .08). There was a significant indirect path from greater food insecurity to greater waist circumference through poorer sleep health (0.11, 95% bootstrapping CI: [0.01, 0.30]).ConclusionFood insecurity is an important correlate of sleep and cardiometabolic health among urban AI/AN youth and should be addressed to reduce emerging health risks during this important developmental period. Policies to reduce food insecurity and increase access to healthy foods as well as sleep interventions for these youth could help, as preliminary findings suggest that sleep health may mediate the negative impact of food insecurity on cardiometabolic risks.

Project description:Although voluminous research has linked nonresident fatherhood to riskier sexual behavior in adolescence, including earlier sexual debut, neither the causality of that link nor the mechanism accounting for it has been well-established. Using data from the National Longitudinal Survey of Youth, 1979-the Young Adult Survey (CNLSY-YA), the present study addresses both questions by comparing the sexual development of siblings discordant for age at father departure from the home and examining results across behavioral (age at first intercourse), biological (pubertal timing), and cognitive (attitudes about childbearing and marriage) sexual outcomes (N = 5,542). Findings indicate that nonresident fatherhood, beginning either at birth or during middle childhood, leads to an earlier sexual debut for girls, but not for boys, an effect likely explained by weak parental monitoring rather than an accelerated reproductive strategy.

Project description:For adolescents, normative development encompasses learning to negotiate challenges of sexual situations; of special importance are skills to prevent early pregnancy, HIV, and other sexually transmitted diseases. Disparities in sexual risk among American Indian youth point to the importance of intervening to attenuate this risk. This study explored the impact of Circle of Life (COL), an HIV prevention intervention based on social cognitive theory, on trajectories of self-efficacy (refusing sex, avoiding sexual situations) among 635 students from 13 middle schools on one American Indian reservation. COL countered a normative decline of refusal self-efficacy among girls receiving the intervention by age 13, while girls participating at age 14 or older, girls in the comparison group, and all boys showed continuing declines.

Project description:Failure to acknowledge that one has been the victim of sexual violence is an important, yet understudied, barrier that prevents women from seeking appropriate support following sexual violence. Drawing from a literature of demonstrating the benefits of self-distancing when evaluating emotionally charged personal information, the effects of self-distancing on acknowledgment of sexual assault were tested. Four experimental studies (Ntotal = 1,609) manipulated perspective-taking, either by asking women to imagine a series of hypothetical sexual encounters as experiences that happened to themselves or to their friends, or by asking women to describe a sexual experience from a first- or third-person perspective. Findings from the studies suggest that taking another person's perspective can help women to label ambiguous sexual experiences as more inappropriate and coercive. Notably, this did not seem to stem from women downplaying or dismissing experiences when they imagined themselves, as they reported anticipating more negative and less positive emotions in the scenarios where they imagined themselves compared to a friend. Nonetheless, in spite of the stronger anticipated negative emotional response when imagining themselves, women were less open to information about resources associated with sexual assault and support when they imagined themselves compared to a friend. This pattern of findings replicated for own, past sexual experiences but only to the extent that women spontaneously engaged in distanced perspective-taking themselves. This research suggests in addition to using contextual information to disambiguate and determine whether a sexual experience was inappropriate, taking a distanced perspective might provide a route through which women can come to terms with the experience and open up to the use of community-based services and sexual assault resources.

Project description:ObjectiveThe purpose of this study was to examine the relationships between individual parent stressors (financial, legal, career, relationships, home safety, community safety, medical, housing, authority, and prejudice) and adolescent obesity in African American adolescents.MethodsData were from a cross section convenience sample of 273 African American parent-child dyads (ages 11-19) from Washtenaw County, Michigan. A subset of 122 dyads who completed parent and child questionnaires were included in this analysis. Parent stressors were assessed using the Crisis in Family Systems Revised (CRISYS-R) questionnaire. Height, weight, and waist circumference were measured by trained staff; height and weight were converted to BMI. Multivariate linear regression models were used to examine the relationships between individual parent stressors and adolescent BMI and waist circumference.ResultsParental exposure to stressors related to safety in the community was positively associated with adolescent BMI (β = 1.20(0.47), p=0.01) and waist circumference (β = 2.86(1.18), p=0.02). Parental appraisal of stressors related to safety in the community as "difficult to get through" was positively associated with adolescent BMI (β = 0.39(0.14), p=0.006) and waist circumference (β = 1.00(0.35), p=0.005). These relationships remained significant when adjusting for behavioral and psychosocial covariates. There were no significant relationships observed between other parent stressors and adolescent BMI or waist circumference.ConclusionThese findings suggest parents' exposure and appraisal of stressors related to community safety are associated with increased adolescent obesity in African American youth. Longitudinal, larger-scale studies are needed to better understand the mechanisms by which community safety may increase obesity risk in this ethnic minority pediatric population.
