Ictal laughter and crying: Should they be classified as automatisms?

ABSTRACT: Gelastic seizures (GS) describe ictal laughter and are associated with hypothalamic lesions, as well as other cortical areas. Dacrystic seizures (DS), characterized by ictal crying, also have been reported in hypothalamic lesions and focal epilepsy. We describe a young girl with drug resistant focal dyscognitive seizures associated with gelastic and dacrystic features. However, neither laughter nor crying was correlated with a stereotyped electroencephalographic (EEG) pattern or involvement of a particular brain region. Additionally, based on the variety of epileptogenic foci associated with GS and DS in the literature, laughter and crying appear to represent ictal or peri-ictal automatisms.

Ictal asystole and ictal syncope: insights into clinical management.

Project description:Ictal asystole is a rare, serious, and often treatable cause of syncope. There are currently limited data to guide management. Characterization of ictal syncope predictors may aid in the selection of high-risk patients for treatments such as pacemakers.We searched our epilepsy monitoring unit database from October 2003 to July 2013 for all patients with ictal asystole events. Clinical, electroencephalogram, and ECG data for each of their seizures were examined for their relationships with ictal syncope events. In 10 patients with ictal asystole, we observed 76 clinical seizures with 26 ictal asystole episodes, 15 of which led to syncope. No seizure with asystole duration?6 s led to syncope, whereas 94% (15/16) of seizures with asystole duration>6 s led to syncope (P=0.02). During ictal asystole events, 4 patients had left temporal seizure onset, 4 patients had right temporal seizure onset, and 2 patients had both. Syncope was more common with left temporal (40%) than with right temporal seizures (10%; P=0.002). Treatment options included antiepileptic drug changes, epilepsy surgery, and pacemaker implantation. Eight patients received pacemakers. During follow-up of 72±95 months, all patients remained syncope free.Ictal asystole>6 s is strongly associated with ictal syncope. Ictal syncope is more common in left than in right temporal seizures. A permanent pacemaker should be considered in patients with ictal syncope if they are not considered good candidates for epilepsy surgery.

Non ictal onset zone: A window to ictal dynamics.

Project description:The focal and network concepts of epilepsy present different aspects of electroclinical phenomenon of seizures. Here, we present a 23-year-old man undergoing surgical evaluation with left fronto-temporal electrocorticography (ECoG) and microelectrode-array (MEA) in the middle temporal gyrus (MTG). We compare action-potential (AP) and local field potentials (LFP) recorded from MEA with ECoG. Seizure onset in the mesial-temporal lobe was characterized by changes in the pattern of AP-firing without clear changes in LFP or ECoG in MTG. This suggests simultaneous analysis of neuronal activity in differing spatial scales and frequency ranges provide complementary insights into how focal and network neurophysiological activity contribute to ictal activity.

Fetal homologue of infant crying.

Project description:Four behavioural states are recognised in the human fetus and are comparable to those of the neonate: 1F (quiet sleep), 2F (active state), 3F (quiet awake), and 4F (active awake). State 5, or crying, is not considered to have a fetal correlate. In a study assessing the effects of exposure to tobacco and cocaine during pregnancy on fetal response and habituation to vibroacoustic stimulation, what appears to be the fetal homologue of crying was observed. These behaviours were seen on ultrasound, and have been captured on video recordings and include: an initial exhalation movement associated with mouth opening and tongue depression, followed by a series of three augmented breaths, the last breath ending in an inspiratory pause followed by an expiration and settling. This is the first report/video documenting these behaviours and suggests the possibility of a state 5F.

The crying sign: the winking umbilical cord.

Project description:A preterm baby girl, born at 34?weeks gestation, with features of Beckwith-Wiedemann syndrome was noted to have a relatively large umbilical stump. No fetal abnormalities had been detected on anatomy scan at 28?weeks and only mild polyhydramnios and macrosomia were noted on a 32-week ultrasound scan. Although there was no obvious omphalocoele, clinical assessment of the umbilical cord revealed an abdominal wall defect through which bowel would protrude into the umbilicus when the infant was crying. In keeping with an abdominal wall defect ?-fetoprotein was found to be elevated. Surgical consultation advised conservative management. Subsequently, detachment of the umbilical cord occurred 1?week postdischarge and a large umbilical hernia persists. Genetic analysis confirmed a diagnosis of Beckwith-Wiedemann syndrome.

Limbic-visual attenuation to crying faces underlies neglectful mothering.

Project description:Neglectful mothering is one of the most common forms of childhood maltreatment, involving a severe disregard of the child's needs, yet little is known about its neural substrate. A child's needs are usually conveyed by signals of distress revealed by crying faces. We tested whether infant and adult crying faces are processed differently in two sociodemographically similar groups of Neglectful (NM) and non-neglectful Control Mothers (CM). We used functional brain imaging to analyze the BOLD response from 43 mothers (23 neglectful and 20 control) while viewing faces from infants and adults (crying and neutral). In NM as compared to CM, the BOLD responses to both infant and adult crying faces were significantly reduced in the cerebellum, lingual, fusiform, amygdala, hippocampus, parahippocampus, and inferior frontal gyrus. The reduced BOLD was also modulated by comorbid psychiatric symptoms. In the CM, frontal activation to infant versus adult crying faces was enhanced, whereas in the NM activation in the anterior cingulate cortex to infant crying was reduced compared to adult crying. The altered neural response to crying faces in NM, showing generic face and infant-specific face processing deficits, could underlie their characteristic poor social abilities as well as their poor response to infant needs, both affecting the caregiving role.

Crying Therapy Intervention for Breast Cancer Survivors: Development and Effects.

Project description:BACKGROUND:crying therapy is currently being applied in some countries to treat cancer patients, manage pain, and promote mental health. However, little nursing and medical research on the effects of crying therapy has been conducted in other parts of the world. This study aimed to develop a crying therapy program for breast cancer survivors and assess its effects. Interventions/method: data from 27 breast cancer survivors in South Korea were analyzed. The intervention, employing a single group, pre-post-test quasi-experimental design, was divided into three phases, and effects were verified for emotional (distress, fatigue, and mood conditions) and physiological (cortisol, immunoglobulin G, and blood pressure) variables. RESULTS:there were significant changes in distress, mood changes, and immunoglobulin G and smaller changes in blood pressure postintervention. Fatigue and cortisol showed no significant changes. CONCLUSIONS:this study demonstrated the effectiveness of a short-term crying therapy program that can induce positive emotional changes and physiological effects in breast cancer survivors. This intervention can improve quality of life, indicating its value as a self-care program for cancer survivors.

Esophageal Atresia and Thenar Hypoplasia Associated with Asymmetric Crying Face.

Project description:Asymmetric crying face (ACF) is a minor congenital anomaly that is often associated with a high rate of major malformations and may be considered an indication of a syndromic clinical presentation. Here, we report a 21-month-old male presenting with left- sided ACF, thenar hypoplasia, and esophageal atresia. Ultrasonographic images of the volar surface of the left hand evidenced the absence of muscle tissue around the thenar prominence at the level of the first metacarpal bone. No pathogenic copy number variation was detected on array-comparative genomic hybridization analysis (CGH). The association of esophageal atresia, thenar hypoplasia, and ACF has not been reported before. We discuss the possibility of a distinct association or of a sequence of anomalies associated with ACF.

Brainstem Correlates of Pathological Laughter and Crying Frequency in ALS.

Project description:Pseudobulbar affect is a disorder of emotional expression commonly observed in amyotrophic lateral sclerosis (ALS), presenting as episodes of involuntary laughter, or crying. The objective of the current study was to determine the association between frequency of pathological laughter and crying (PLC) episodes with clinical features, cognitive impairment, and brainstem pathology. Thirty-five sporadic ALS patients underwent neuropsychological assessment, with a subset also undergoing brain imaging. The Center for Neurological Study Lability Scale (CNS-LS) was used to screen for presence and severity of pseudobulbar affect (CNS-LS ≥ 13) and frequency of PLC episodes. Presence of pseudobulbar affect was significantly higher in bulbar onset ALS (p = 0.02). Frequency of PLC episodes was differentially associated with cognitive performance and brainstem integrity. Notably pathological laughter frequency, but not crying, showed a significant positive association with executive dysfunction on the Trail Making Test B-A (R 2 = 0.14, p = 0.04). Similarly, only pathological laughter frequency demonstrated a significant negative correlation with gray matter volume of the brainstem (R 2 = 0.46, p < 0.01), and mean fractional anisotropy of the superior cerebellar peduncles (left: R 2 = 0.44, p < 0.01; right: R 2 = 0.44, p < 0.01). Hierarchical regression indicated brainstem imaging in combination with site of symptom onset explained 73% of the variance in pathological laughter frequency in ALS. The current findings suggest emotional lability is underpinned by degeneration across distinct neural circuits, with brainstem integrity critical in the emergence of pathological laughter.

The neural basis of authenticity recognition in laughter and crying.

Project description:Deciding whether others' emotions are genuine is essential for successful communication and social relationships. While previous fMRI studies suggested that differentiation between authentic and acted emotional expressions involves higher-order brain areas, the time course of authenticity discrimination is still unknown. To address this gap, we tested the impact of authenticity discrimination on event-related potentials (ERPs) related to emotion, motivational salience, and higher-order cognitive processing (N100, P200 and late positive complex, the LPC), using vocalised non-verbal expressions of sadness (crying) and happiness (laughter) in a 32-participant, within-subject study. Using a repeated measures 2-factor (authenticity, emotion) ANOVA, we show that N100's amplitude was larger in response to authentic than acted vocalisations, particularly in cries, while P200's was larger in response to acted vocalisations, particularly in laughs. We suggest these results point to two different mechanisms: (1) a larger N100 in response to authentic vocalisations is consistent with its link to emotional content and arousal (putatively larger amplitude for genuine emotional expressions); (2) a larger P200 in response to acted ones is in line with evidence relating it to motivational salience (putatively larger for ambiguous emotional expressions). Complementarily, a significant main effect of emotion was found on P200 and LPC amplitudes, in that the two were larger for laughs than cries, regardless of authenticity. Overall, we provide the first electroencephalographic examination of authenticity discrimination and propose that authenticity processing of others' vocalisations is initiated early, along that of their emotional content or category, attesting for its evolutionary relevance for trust and bond formation.

Diagonistic [corrected] Apraxia and Ictal Alien Hand.

Project description:Alien hand is a strangely behaving hand and occurs in disease of medial frontal lobe and also corpus callosum. When it is with frontal lobe disease, it manifests as groping and hooking with either hand, but callosal lesions produce left-hand abnormalities of varying degrees. Occasionally this phenomenon can be transient as an ictal manifestation. Case 1 is a 20-year-old female who was on treatment for panic and anxiety for 4 years and later noticed strange action by the left hand pushing food plate and notebook when she is using them with the right hand. Case 2 is a 14-year-old boy who presented with episodes of strange situation where his left hand was pushing his right hand into his mouth, and he had no control over it. He bit his right-hand fingers, and on one episode, he was bleeding from his injured fingers, yet he felt someone is making his left hand do this. Both patients had a demonstrable structural lesion. Patients, when they have stereotyped strange behavior with limbs, need to be investigated for organic disease and initiate treatment accordingly.
