Follicular Thyroid Carcinoma Presenting as Large Solitary Vertebral Metastasis: Report of Two Unusual Cases Treated with Radiotherapy.

ABSTRACT: Solitary spinal metastasis with cord compression as the presenting feature of follicular thyroid carcinoma (FTC) is extremely unusual with <10 cases reported in world literature. We hereby present two such cases in a 39-year-old male with lytic lesion left sacral bone with biopsy showing metastatic carcinoma with morphology and immunophenotype of thyroid gland and a 35-year-old female with thoracic vertebral lesions suggestive of metastatic deposit of FTC. Subsequently, both patients were found to have a solitary nodule in the thyroid lobe. They were treated with local radiotherapy (RT) with significant symptomatic relief. The present cases highlight the rarity of FTC to present as upfront solitary vertebral metastases with significant morbidity in young individuals although a slow indolent course with metastases in late stages of disease is more common, debilitating the effect of metastatic lesion requiring RT for pain palliation and the role of supportive nursing care for patient rehabilitation.

Solitary pulmonary metastasis from occult papillary thyroid carcinoma.

Project description:Pulmonary metastases from thyroid carcinoma typically cause a micronodular or miliary pattern throughout both lungs. Metastasis consisting of a solitary pulmonary nodule measuring 20 mm in diameter is rare. Here we report a case of a 66-year-old woman without a history of papillary thyroid carcinoma who presented with a pulmonary nodule measuring 20 mm in diameter, found on chest computed tomography during a health checkup. The patient underwent a right lobectomy. Microscopic examination showed well-differentiated papillary adenocarcinoma. Immunohistochemical findings were consistent with a diagnosis of pulmonary metastasis from papillary thyroid carcinoma. Solitary metastasis to the lung from occult thyroid carcinoma is quite rare, but if a pulmonary nodule is encountered in a patient without a history of thyroid carcinoma, the possibility must be considered.

Invasive follicular variant of papillary thyroid cancer harboring the NRAS mutation Q61K and presenting with bone metastasis-A case report.

Project description:IntroductionThe follicular variant of papillary thyroid cancer (FVPTC) can be noninvasive or invasive. The invasive form of FVPTC commonly harbors BRAF mutations whereas RAS mutations are more often associated with noninvasive FVPTC and a favorable clinical outcome.Case reportA 47-year-old man presented with a metastasis to his right iliac bone as the initial manifestation of a 1.6cm invasive FVPTC. After total thyroidectomy, the patient underwent additional treatment, including thyroid hormone suppressive treatment to non-detectable TSH levels, repeated courses of radioiodine treatment, external beam radiation, and treatment with the tyrosine kinase inhibitor sorafenib. Despite these therapeutic efforts, the disease progressed with growth of the iliac mass and additional metastatic spread to cervical and lumbar vertebrae causing increasing pain and disability. The patient succumbed to the disease four years after presentation. Retrospective next-generation sequencing of the primary tumor using a pan-cancer targeted mutation and gene fusion panel revealed NRAS Q61K mutation and no other oncogenic alterations.DiscussionThe study challenges the concept that thyroid neoplasms with isolated RAS mutations are often associated with favorable clinical behavior and may be candidates for conservative management.ConclusionAn isolated RAS mutation in invasive FVPTC may be associated with an aggressive clinical behavior.

Radiomics Based on Thyroid Ultrasound Can Predict Distant Metastasis of Follicular Thyroid Carcinoma.

Project description:We aimed to evaluate whether radiomics analysis based on gray-scale ultrasound (US) can predict distant metastasis of follicular thyroid cancer (FTC). We retrospectively included 35 consecutive FTCs with distant metastases and 134 FTCs without distant metastasis. We extracted a total of 60 radiomics features derived from the first order, shape, gray-level cooccurrence matrix, and gray-level size zone matrix features using US imaging. A radiomics signature was generated using the least absolute shrinkage and selection operator and was used to train a support vector machine (SVM) classifier in five-fold cross-validation. The SVM classifier showed an area under the curve (AUC) of 0.90 on average on the test folds. Age, size, widely invasive histology, extrathyroidal extension, lymph node metastases on pathology, nodule-in-nodule appearance, marked hypoechogenicity, and rim calcification on the US were significantly more frequent among FTCs with distant metastasis compared to those without metastasis (p < 0.05). Radiomics signature and widely invasive histology were significantly associated with distant metastasis on multivariate analysis (p < 0.01 and p = 0.003). The classifier using the results of the multivariate analysis showed an AUC of 0.93. The radiomics signature from thyroid ultrasound is an independent biomarker for noninvasively predicting distant metastasis of FTC.

Intrathyroidal parathyroid carcinoma presenting as thyroid nodule: Case report of unusual location.

Project description:IntroductionParathyroid carcinoma (PC) is considered a rare and uncommon malignancy. Its prevalence is about 0.005% of all cancers. Intrathyroidal location is rare, rendering preoperative diagnosis tedious. Until now, around 700 cases of PC have been documented, reportedly, among them, less than 21 cases of intrathyroidal parathyroid carcinoma have been described in the literature. We report a case of intrathyroidal PC that was taken for a suspicious thyroid nodule, with a literature review.PRESENTATION OF THE CASE: Our case is an asymptomatic intrathyroidal PC imitating a suspicious thyroid nodule, in a 54-year-old woman. A literature review was performed about clinical, radiological features, histopathological findings, and therapeutic options.DiscussionThe diagnosis of asymptomatic intrathyroidal parathyroid carcinoma, similar to our case report, is even more difficult, our patient had no symptoms of hypercalcemia. Surgery is the cornerstone of the treatment. A better chance to cure this disease is conditioned by complete surgical resection with negative margins microscopically, that was the case of our patient with a very good clinical course after 12 months of follow-up.

Anterior mediastinal tumor as a solitary lymph node metastasis of occult thyroid carcinoma.

Project description:Solitary metastasis of occult thyroid carcinoma to the anterior mediastinum is very rare. A 65-year-old woman was examined for anterior mediastinal tumor based on the FDG accumulation on PET. We resected the tumor by video-assisted thoracic surgery. A pathological examination revealed that the tumor was lymph node metastasis of papillary thyroid carcinoma. The postoperative examination showed that the tumor was a solitary lymph node metastasis of occult thyroid carcinoma. Primary thyroid carcinoma has not appeared in 2 years since the surgery, and careful follow-up has been continued.

Long-term follow-up of papillary and follicular thyroid carcinomas with bone metastasis.

Project description:The aims of this study were to investigate papillary and follicular thyroid carcinomas with bone metastasis in various clinical presentations and to determine the prognostic factors after multimodality treatment. A retrospective analysis was performed of 3,120 patients with papillary and follicular thyroid carcinoma. Of these patients, 131 (including 97 women, 71.8%) were diagnosed with bone metastasis and underwent follow-up at the Chang Gung Medical Center. Patients with bone metastasis were categorized into two groups. Group A was comprised of patients who were diagnosed with bone metastasis either before thyroidectomy or within 6 months of the initial thyroidectomy (90 patients, 68.7%). Group B was comprised of patients with bone metastasis who received a diagnosis 6 months post-thyroidectomy in the follow-up period (41 patients, 31.3%). After a mean follow-up period of 8.4 ± 7.0 years, there were 88 deaths (67.2%) attributed to thyroid cancer and 13 patients (9.9%) achieved disease-free status. A multivariate analysis showed that older age, early diagnosis, and brain metastasis were each associated with a poor prognosis. The difference in disease-specific mortality rates between groups A and B was significant (p < 0.0001). In conclusion, papillary and follicular thyroid cancers with bone metastasis have a high rate of mortality. Despite this high mortality, 9.9% patients still had an excellent response to treatment.

Brain Metastasis of Follicular Thyroid Carcinoma in Pregnancy: A Case Report and Literature Review.

Project description:Brain metastases (BMs) related to cancer are quite common and represent the most common brain cancer. We present a rare case of a 32-year-old female, 36 weeks pregnant, admitted to the emergency with complaints of severe headache, vomiting, and left hemiparesis associated with drowsiness. Cranial tomography showed an image suggestive of an expansive lesion in the right front-temporo-insular region with an important mass effect. The result of biopsy with immunohistochemistry was compatible with metastasis of follicular thyroid carcinoma (FTC). The knowledge of neurological characteristics in the clinical analysis of patients with thyroid carcinoma must be highly valued, both in the correct interpretation of the signs and in the early investigation through skull imaging exams.

Solitary sclerosis presenting as isolated spontaneous paroxysmal dysarthria.

Project description:Paroxysmal dysarthria and ataxia (PDA) is a rare syndrome characterized by brief, stereotyped episodes of slurred speech, clumsiness with extremities, or vertigo. It is usually observed in young patients suffering from multiple sclerosis with numerous lesions. PDA is challenging to identify in those presenting with atypical patterns. Here, a non-ataxic variant of PDA in an otherwise neurologically healthy elderly man is presented who had a single midbrain lesion. A broad diagnostic workup illustrates the challenges of identifying PDA. Teaching points emphasize the significance of the midbrain lesion and response to anti-epileptic medication.

Extraneural Glioblastoma Multiforme Vertebral Metastasis.

Project description:Glioblastoma multiforme (GBM) is the most common malignant central nervous system tumor; however, extraneural metastasis is uncommon. Of those that metastasize extraneurally, metastases to the vertebral bodies represent a significant proportion. We present a review of 28 cases from the published literature of GBM metastasis to the vertebra. The mean age at presentation was 38.4 years with an average overall survival of 26 months. Patients were either asymptomatic with metastasis discovered at autopsy or presented with varying degrees of pain, weakness of the extremities, or other neurologic deficits. Of the cases that included the time to spinal metastasis, the average time was 26.4 months with a reported survival of 10 months after diagnosis of vertebral metastasis. A significant number of patients had no treatments for their spinal metastasis, although the intracranial lesions were treated extensively with surgery and/or adjuvant therapy. With increasing incremental gains in the survival of patients with GBM, clinicians will encounter patients with extracranial metastasis. As such, this review presents timely information concerning the presentation and outcomes of patients with vertebral metastasis.
