Project description:Four in 10 young rural Chinese children are 'left behind' by parents migrating for economic opportunities. Left-behind children do as well academically and imagine as many possible futures for themselves as their peers, implying that they must compensate in some ways for loss of everyday contact with their parents. Three studies test and find support for the prediction that compensation entails self-expansion to include a caregiving grandmother rather than one's mother in self-concept, as is typical in Chinese culture. We measured self-expansion with feeling, function and neurophysiological variables. Twelve-year-old middle school left-behind children (Study 1, N?=?66) and 20-year-old formerly left-behind children (now in college, Studies 2 and 3, N?=?162) felt closer to their grandmothers and not as close to their mothers as their peers. Self-expansion had functional consequence (spontaneous depth-of-processing) and left a neurophysiological trace (event-related potential, Study 3). Left-behind participants had enhanced recall for information incidentally connected to grandmothers (Studies 1 and 3, not Study 2). Our results provide important insights into how left-behind children cope with the loss of parental presence: they include their grandmother in their sense of self. Future studies are needed to test downstream consequences for emotional and motivational resilience.

Project description:BackgroundBullying is one of the most important factors associated with child abuse. However, robust tests supporting the assumption that being bullied can contribute to child sexual abuse (CSA) among left-behind children (LBC) remain sparse. This study aims to investigate the association of bullying victimization with CSA among LBC in China.MethodsA cross-sectional study was conducted in six middle schools of Sichuan and Anhui province in 2015. The bullying victimization was assessed by seven items from the Revised Olweus Bully/Victim Questionnaire. The experience of CSA was measured by ten items CSA scale with good consistency.ResultsA total of 1,030 children met the sampling criteria, including 284 LBC and 746 non-LBC. The prevalence of CSA was 22.89% in LBC and 20.19% in non-LBC (p > 0.05). Bullying victimization was related to CSA among both LBC (adjusted Odds Ratio [aOR] = 2.52, 95% CI [1.34-4.73]) and non-LBC (aOR = 2.35, 95% CI [1.58-3.53]). The association between bullying victimization and CSA was much higher among left-behind girls (left-behind girls: aOR = 7.36, 95% CI [2.16-24.99]; non-left-behind girls: aOR = 2.38, 95% CI [1.08-5.27]). Also, LBC of a young age (11-15), children with siblings, living in rural areas and non-traditional family structure who were bullied were more likely to suffer CSA than their non-LBC peers.ConclusionsBullying victimization is associated with a significant increase in CSA among both LBC and non-LBC. Anti-bullying programs should target vulnerable populations including female LBC and LBC with siblings to reduce the risk of CSA.

Project description:To estimate the prevalence of suicide attempts and explore the shared and unique factors influencing suicide risk in left-behind children (LBC) and non-left-behind children (NLBC) in rural China, this study collected data using a multi-stage cluster random sampling method from 13,952 children including 6,034 LBC and 7,918 NLBC. Sociodemographic characteristics, suicide attempts, neglect and physical abuse, negative life events, and loneliness were measured by self-reported questionnaires. Data were analyzed using logistic regression models. Gender and mother's education level were unique influential factors for NLBC while family structure type was a unique influential factor for LBC. The study provides two novel findings regarding NLBC specifically: 1. Children with optimal family socioeconomic status are more likely to report suicide attempts (odds ratio OR = 1)than are those in the general children population, OR 0.52 (95% CI: 0.39-0.70), and 2. Children with higher mother's education level are subject to higher suicide rates in high school, OR 1.67 (95% CI: 1.13-2.46), and post-secondary education, OR 2.14 (95% CI: 1.37-3.37). The unique characteristics of LBC and NLBC in China suggest that investigating risk factors and determining the factors that might be targeted in intervention programs are urgently needed currently.

Project description:The United Nations' Sustainable Development Agenda calls for targeted attention to the needs and rights of the most vulnerable populations to ensure a life of dignity and human security for all. In this paper, we argue that persons with disabilities are in a disproportionately vulnerable situation in public health emergencies. By using the example of Coronavirus disease 2019 (Covid-19), we explain why that is and call for the systematic consideration of the needs and rights of persons with disabilities during the response to the outbreak and during the recovery phase. Otherwise, equity will continue to be merely an aspiration during this COVID-19 emergency - as it will in future health emergencies.

Project description:The left-behind phenomenon, caused by parent out-migration, has become a common social issue and might lead to long-term and potential risks for children in rural areas of China. It is important to investigate the effect of social interaction on prefrontal activation of left-behind children in China because of possible effects of parent out-migration on children's social cognition. We recruited 81 rural Chinese preschoolers aged 52-76 months (mean = 64.98 ± 6.321 months) preschoolers with three different statuses of parental out-migration (including non-, partially, and completely left-behind children). Using functional Near-Infrared Spectroscopy (fNIRS), we compared behavior and brain activation and in three groups (non-, partially-, completely-left-behind children) under two different social interaction conditions (child-teacher and child-stranger situation). Results revealed that initiating joint attention (IJA) may evoke higher brain activation than responding to joint attention (RJA) in the prefrontal cortex (PFC), especially in the case of initiating joint attention with the stranger. In addition, the activation of joint attention was positively correlated with children's language score, cognitive flexibility, and facial expression recognition. More importantly, partially-left-behind children evoked higher brain activation in the IJA condition and presented a higher language level than completely/non-left-behind children. The current study provides insight into the neural basis of left-behind children's development and revealed for the first time that family economic level and left-behind status may contribute to the lower social cognition.

Project description:BackgroundWith the reorganization of primary care into Patient Aligned Care Teams (PACT) teams, the Veteran Affairs Health System (VA) aims to ensure all patients receive care based on patient-centered medical home (PCMH) principles. However, some patients receive the preponderance of care from specialty rather than primary care clinics because of the special nature of their clinical conditions. We examined seven VA (HIV) clinics as a model to test the extent to which such patients receive PCMH-principled care.ObjectiveTo examine the extent to which HIV specialty care in VA conforms to PCMH principles.DesignQualitative study.ParticipantsForty-one HIV providers from seven HIV clinics and 20 patients from four of these clinics.ApproachWe conducted semi-structured interviews with HIV clinic providers and patients about care practices and adherence to PCMH principles. Using an iterative approach, data was analyzed using both a content analysis and an a priori, PCMH-principled coding strategy.Key resultsPatients with HIV receive varying levels of PCMH-principled care across a range of VA HIV clinic structures. The more PCMH-principled HIV clinics largely functioned as PCMHs; patients received integrated, coordinated, comprehensive primary care within a dedicated HIV clinic. In contrast, some clinics were unable to meet the criteria of being a patient's medical home, and instead functioned primarily as a place to receive HIV-related services with limited care coordination. Patients from the less PCMH-principled clinics reported less satisfaction with their care.ConclusionsEven in a large, integrated healthcare system, there is wide variation in patients' receipt of PCMH-principled care in specialty care settings. In order to meet the goal of having all patients receiving PCMH-principled care, there needs to be careful consideration of where primary and specialty care services are delivered and coordinated. The best mechanisms for ensuring that patients with complex medical conditions receive PCMH-principled care may need to be tailored to different specialty care contexts.

Project description:Checkpoint inhibitors, monoclonal antibodies that inhibit PD-1 or CTLA-4, have revolutionized the treatment of multiple cancers. Despite the enthusiasm for the clinical successes of checkpoint inhibitors, and immunotherapy, in general, only a minority of patients with specific tumor types actually benefit from treatment. Emerging evidence implicates epigenetic alterations as a mechanism of clinical resistance to immunotherapy. This review presents evidence for that association, summarizes the epi-based mechanisms by which tumors evade immunogenic cell death, discusses epigenetic modulation as a component of an integrated strategy to boost anticancer T cell effector function in relation to a tumor immunosuppression cycle and, finally, makes the case that the success of this no-patient-left-behind strategy critically depends on the toxicity profile of the epigenetic agent(s).

Project description:An unprecedented number of new tuberculosis (TB) medications are currently in development, and there will be great pressure to deploy these new drugs among all populations after their efficacy is demonstrated. People living with HIV experience a large burden of TB and have a particularly pressing need for TB treatments that are shorter and less toxic. In addition, all people living with HIV now require antiretroviral therapy during TB treatment. A roadmap of the research, programmatic, and regulatory considerations includes the following: (1) inclusion of people living with HIV early in clinical trials for treatment and prevention using new TB medications, (2) prioritization of key studies of HIV-TB drug interactions and interactions between new TB agents, and (3) optimization of clinical trial infrastructure, laboratory capacity, and drug susceptibility testing.

Project description:BackgroundWith great economic development and rapid urbanization in China, left-behind children whose parents migrate to big cities for job has become a large special population which requires more attention. The present study aims to explore the specific influence of migrant mothers on early child development, especially on social-emotional problems.MethodsThe data of this study was obtained from a cross-sectional study in 8 counties of central and western rural China. Development status of 1880 children aged <60 months were assessed by Ages & Stages Questionnaire-Chinese Edition (ASQ) and the Ages and Stages Questionnaire: Social Emotional-Chinese Edition (ASQ: SE). Multivariate logistic regressions were used to analyze the association between being left behand by migrant mothers and developmental problems in various domains, while adjusting socio-demographic, socio-economic and perinatal co-variates, and effect modification analysis were conducted to explore the effect of age, gender and birth order.ResultsChildren left behind by migrant mothers were more likely to have overall suspected developmental delay (odds ratio (OR)?=?1.24, 95% confidence interval (CI)?=?1.13-1.35), developmental delay in personal social domain (OR?=?1.55, 95% CI?=?1.17-2.04) and socio-emotional delay compared with those living with their own mothers (OR?=?1.49, 95% CI?=?1.11-2.00) after adjusting for potential confounders. Additionally, girls increased the odds of social-emotional problems among children being left behind by migrating mother (P for interaction?=?0.037).ConclusionsThe study concluded that children left behind by migrant mothers were more likely to have suspected developmental delay compared with their peers living with mothers, especially on social emotional development. Future intervention is needed for this special population and should pay more attention to girls.

Project description:Parental migration has caused millions of children left behind, especially in China and India. Left-behind children (LBC) have a high risk of mental disorders and may present negative life outcomes in the future. However, little is known whether there are cerebral structural alterations in LBC in relative to those with parents. This study is to explore the effect of parental migration on brain maturation by comparing gray matter volume (GMV) and fractional anisotropy (FA) of LBC with well-matched non-LBC. Thirty-eight LBC (21 boys, age = 9.60 ± 1.8 years) and 30 non-LBC (19 boys, age = 10.00 ± 1.95 years) were recruited and underwent brain scans in 3.0 T MR. Intelligence quotient and other factors including family income, guardians' educational level and separation time were also acquired. GMV and FA were measured for each participant and compared between groups using 2-sample t-tests with atlas-based analysis. Compared to non-LBC, LBC exhibited greater GMV in emotional and cortico-striato-thalamo-cortical circuits, and altered FA in bilateral superior occipitofrontal fasciculi and right medial lemniscus (p < 0.05, Cohen's d > 0.89, corrected for false-discovery rate). Other factors including family income, guardians' educational level and separation time were not associated with these brain changes. Our study provides empirical evidence of altered brain structure in LBC compared to non-LBC, responsible for emotion regulation and processing, which may account for mental disorders and negative life outcome of LBC. Our study suggests that absence of direct biological parental care may impact children's brain development. Therefore, public health efforts may be needed to provide additional academic and social/emotional supports to LBC when their parents migrate to seeking better economic circumstances, which has become increasingly common in developing countries.
