Project description:BackgroundHome-based renal replacement therapy (RRT) [peritoneal dialysis (PD) and home hemodialysis (HHD)] offers independent quality of life and clinical advantages compared to conventional in-center hemodialysis. However, follow-up may be less complete for home dialysis patients following a change in care settings such as post hospitalization. We aim to implement a Home Dialysis Virtual Ward (HDVW) strategy, which is targeted to minimize gaps of care.Methods/designThe HDVW Pilot Study will enroll consecutive PD and HHD patients who fulfilled any one of our inclusion criteria: 1. following discharge from hospital, 2. after interventional procedure(s), 3. prescription of anti-microbial agents, or 4. following completion of home dialysis training. Clinician-led telephone interviews are performed weekly for 2 weeks until VW discharge. Case-mix (modified Charlson Comorbidity Index), symptoms (the modified Edmonton Symptom Assessment Scale) and patient satisfaction are assessed serially. The number of VW interventions relating to eight pre-specified domains will be measured. Adverse events such as re-hospitalization and health-services utilization will be ascertained through telephone follow-up after discharge from the VW at 2, 4, 12 weeks. The VW re-hospitalization rate will be compared with a contemporary cohort (matched for age, gender, renal replacement therapy and co-morbidities). Our protocol has been approved by research ethics board (UHN: 12-5397-AE). Written informed consent for participation in the study will be obtained from participants.DiscussionThis report serves as a blueprint for the design and implementation of a novel health service delivery model for home dialysis patients. The major goal of the HDVW initiative is to provide appropriate and effective supports to medically complex patients in a targeted window of vulnerability.Trial registration(NCT01912001).

Project description:Bayesian optimization (BO) can accelerate material design requiring time-consuming experiments. However, although most material designs require tuning of multiple properties, the efficiency of multi-objective (MO) BO in time-consuming experimental material design remains unclear, due to the complexity of handling multiple objectives. This study introduces MO BO method that efficiently achieves predefined goals and shows that by focusing on achieving the goals, BO can efficiently accelerate realistic MO design problems with small efforts. Benchmarks showed that the proposed BO method dramatically reduced the number of experiments needed to achieve goals relative to a baseline method. Virtual MO inverse design experiments with realistic material design problems were also performed, during which the proposed method could achieve goals within only around ten experiments in average and showed over 1000-fold acceleration relative to the random sampling for the most difficult case. The introduction of goal-oriented BO will precede real-world application of BO.

Project description:BACKGROUND:Renovation of a general medical ward provided an opportunity to study health care facility design as a factor for preventing hospital-acquired infections. OBJECTIVE:To determine whether a hospital ward designed with predominantly single rooms was associated with lower event rates of hospital-acquired infection and colonization. METHODS:A prospective controlled trial with patient allocation incorporating randomness was designed with outcomes on multiple 'historic design' wards (mainly four-bed rooms with shared bathrooms) compared with outcomes on a newly renovated 'new design' ward (predominantly single rooms with private bathrooms). RESULTS:Using Poisson regression analysis and adjusting for time at risk, there were no differences (P=0.18) in the primary outcome (2.96 versus 1.85 events/1000 patient-days, respectively). After adjustment for age, sex, Charlson score, admitted from care facility, previous hospitalization within six months, isolation requirement and the duration on antibiotics, the incidence rate ratio was 1.44 (95% CI 0.71 to 2.94) for the new design versus the historic design wards. A restricted analysis on the numbers of events occurring in single-bed versus multibed wings within the new design ward revealed an event incidence density of 1.89 versus 3.47 events/1000 patient-days, respectively (P=0.18), and an incidence rate ratio of 0.54 (95% CI 0.15 to 1.30). CONCLUSIONS:No difference in the incidence density of hospital-acquired infections or colonizations was observed for medical patients admitted to a new design ward versus historic design wards. A restricted analysis of events occurring in single-bed versus multibed wings suggests that ward design warrants further study.

Project description:An acute mental health triage ward at Oxleas NHS Foundation Trust was attracting high levels of formal service user and family complaints. The Trust used experience based co-design to examine the issues and redesign procedures. This resulted in an immediate eradication of formal complaints for a period of 23 months. This paper describes two outcomes: firstly, the successful adaptations made to the experience based co-design methodology from its origins in physical care, in order to ensure it was safe and effective in an acute mental health setting; and, secondly, the changes made to the ward as a result of this quality improvement intervention.

Project description:Higher demand for placements, due to Covid-19 training delays and increased student numbers resulted in placement demand exceeding opportunities. Therefore, a student simulated ward (Cub Ward) was created. A disused ward was transformed into a functioning ward with high and medium fidelity mannequins as patients. The ward was completely led by student nurses. A part 3 student was Nurse-in-Charge while remaining students took on the role of Staff Nurses, and part 1 students remained as “student nurses”. Learning outcomes were:• Develop an understanding of how clinical areas function• Discuss prioritisation in clinical care• Understand patient management throughout a shift• Explore management of workload and resources• Analyse effective communication with MDT A real-time simulated shift included medication administration, patient cares, admissions, discharges, recognition and escalation of deteriorating children. Students also had to manage parents, ward visitors and various ward rounds. Pre and post confidence scales showed improvement for all student groups. Some unintended benefits included involvement of the MDT in the student experience. Peer support and teaching was evident throughout the day, as was resolution for issues encountered for example identifying poor allocation and reallocating accordingly.

Project description:Work on synthetic biology has largely used a component-based metaphor for system construction. While this paradigm has been successful for the construction of numerous systems, the incorporation of contextual design issues-either compositional, host or environmental-will be key to realising more complex applications. Here, we present a design framework that radically steps away from a purely parts-based paradigm by using aspect-oriented software engineering concepts. We believe that the notion of concerns is a powerful and biologically credible way of thinking about system synthesis. By adopting this approach, we can separate core concerns, which represent modular aims of the design, from cross-cutting concerns, which represent system-wide attributes. The explicit handling of cross-cutting concerns allows for contextual information to enter the design process in a modular way. As a proof-of-principle, we implemented the aspect-oriented approach in the Python tool, SynBioWeaver, which enables the combination, or weaving, of core and cross-cutting concerns. The power and flexibility of this framework is demonstrated through a number of examples covering the inclusion of part context, combining circuit designs in a context dependent manner, and the generation of rule, logic and reaction models from synthetic circuit designs.

Project description:Instead of using diversity oriented syntheses (DOS) to obtain compounds with biological activities, we employed the DOS method to efficiently obtain multifunctional single attachment point (MSAP) reagents for the conjugation to proteins. Acid insensitive functional groups (chelators, fluorochromes) were attached to Lys-Cys-NH(2) or Lys-Lys-betaAla-Cys-NH(2) peptide scaffolds. After cleavage from solid supports, the modified peptide intermediates were split and further modified by two solution phase, chemoselective reactions employing the single amine and single thiol presented on the intermediates. MSAP-based fluorochrome-chelates were obtained, some possessing a third functional group like a polyethylene glycol (PEG) polymer or "click chemistry" reactive alkynes and azides. The DOS of MSAP reagents permitted the efficient generation of panels of MSAP reagents that can be used to obtain multifunctional proteins with a single modified amino acid (a single attachment point).

Project description:Random peptide libraries that cover large search spaces are often used for the discovery of new binders, even when the target is unknown. To ensure an accurate population representation, there is a tendency to use large libraries. However, parameters such as the synthesis scale, the number of library members, the sequence deconvolution and peptide structure elucidation, are challenging when increasing the library size. To tackle these challenges, we propose an algorithm-supported approach to peptide library design based on molecular mass and amino acid diversity. The aim is to simplify the tedious permutation identification in complex mixtures, when mass spectrometry is used, by avoiding mass redundancy. For this purpose, we applied multi (two- and three-)-objective genetic algorithms to discriminate between library members based on defined parameters. The optimizations led to diverse random libraries by maximizing the number of amino acid permutations and minimizing the mass and/or sequence overlapping. The algorithm-suggested designs offer to the user a choice of appropriate compromise solutions depending on the experimental needs. This implies that diversity rather than library size is the key element when designing peptide libraries for the discovery of potential novel biologically active peptides.

Project description:Excellent braking performance is the premise of safe driving, and improve the braking performance by upgrading structures and optimizing parameters of braking systems has become the pursuit of engineers. With the development of autonomous driving and intelligent connected vehicle, new structural schemes such as electro-mechanical brakes (EMBs) have become the future of vehicle braking systems. Meanwhile, many scholars have dedicated to the research on the parameters optimization of braking systems. While, most of the studies focus on reducing the brake size and weight, improving the brake responses by optimizing the parameters, almost not involving the braking performance, and the optimization variables are relatively single. On these foundations, a multi-objective optimal design of EMB parameters is proposed to enhance the vehicle's braking performance. Its objectives and constraints were defined based on relevant standards and regulations. Subsequently, the decision variables were set, and optimal math model was established. Furthermore, the co-simulation platform was constructed, and the optimal design and simulation analyses factoring in the crucial structural and control parameters were performed. The results confirmed that the maximum braking pressure response time of the EMB is decreased by approximately 0.3 s, the stopping distance (SD) of 90 km/h-0 is shortened by about 3.44 m. Moreover, the mean fully developed deceleration (MFDD) is increased by 0.002 g, and the lateral displacement of the body (LD) is reduced by about 0.037 m. Hence, the vehicle braking performance is improved.
