Minimally Invasive Total Hip Replacement in an Ipsilateral Post-traumatic above-knee Amputation: A Case Report.

ABSTRACT: Total hip replacement (THR) is a highly successful operation in alleviating pain and improving the overall function of the hip, in end-stage arthritis of the hip, in otherwise fit patients. However, THR as a surgical option in post-traumatic hip arthritis with ipsilateral above-knee amputation is rarely reported.We are presenting a case report of a 30-year-old male, who had previously underwent an above-knee amputation due to road-traffic accident, presenting 24 h after the injury with segmental fracture femur and popliteal artery laceration, for which the limb could not be salvaged. He had an impacted anteroinferior dislocation of the ipsilateral hip with significant cartilage damage of the femoral head which required open reduction. Subsequently, he developed traumatic arthritis of the involved hip which required conversion to an uncemented THR, using a minimally invasive (MIS) anterolateral approach. The preoperative management, surgical technique, and postoperative rehabilitation are described to highlight the technical challenges, these lower limb amputees may present along with review of literature of such rare cases.THR in an above-knee amputee with posttraumatic hip arthritis using MIS technique is an encouraging surgical option for early functional recovery and minimizing surgical complications.

Project description:Total hip arthroplasty through minimally invasive procedures potentially reduces operative trauma, which is expected to result in improved recovery and rehabilitation. We presently perform total hip arthroplasty using minimally invasive techniques via an anterolateral modification of the Watson-Jones approach. This approach is performed in the supine position and requires sterile draping of both legs. It also involves a modified operative technique which preserves the pelvitrochanteric muscles and extends the release of the hip capsule. In order to employ a proven implant system (BICON-PLUS threaded cup, SL-PLUS stem), it was necessary to modify the stem rasps and use manipulation rasps as trial stems. In this report, we compare 50 conventional total hip replacements with 50 procedures performed using the minimally invasive procedure in terms of blood loss and the duration of the operation. The length of the skin incision varied between 7 and 12 cm with the minimally invasive technique, compared to 15 to 22 cm with the conventional procedure. Both groups were virtually identical with respect to average blood loss (haemoglobin on 10th post-operative day: minimally invasive group, 108.0 g/L; conventional group, 112.0 g/L) and the duration of the procedure (minimally invasive group, 60 minutes; conventional group, 58 minutes). The position of the implanted components correlated with the pre-operative planning with regard to medial head offset, centre of rotation of the hip, and leg length, and was as satisfactory as that observed with the conventional procedure. The complication rate was low (2.9%). Shaft fissures occurred in the first months of use of the minimally invasive procedure before adaptation of the shaft rasps. Dislocations were attributable to improper patient behaviour. Minimally invasive surgery via the anterolateral approach in the supine position was equivalent to the conventional procedure, had a low rate of complications, and did not adversely impact the technical success of the procedure.

Project description:Reduced external knee adduction moments in the second half of stance after total hip replacement have been reported in hip osteoarthritis patients. This reduction is thought to shift the load from the medial to the lateral knee compartment and as such increase the risk for knee osteoarthritis. The knee adduction moment is a surrogate for the load distribution between the medial and lateral compartments of the knee and not a valid measure for the tibiofemoral contact forces which are the result of externally applied forces and muscle forces. The purpose of this study was to investigate whether the distribution of the tibiofemoral contact forces over the knee compartments in unilateral hip osteoarthritis patients 1 year after receiving a primary total hip replacement differs from healthy controls. Musculoskeletal modeling on gait was performed in OpenSim using the detailed knee model of Lerner et al. (2015) for 19 patients as well as for 15 healthy controls of similar age. Knee adduction moments were calculated by the inverse dynamics analysis, medial and lateral tibiofemoral contact forces with the joint reaction force analysis. Moments and contact forces of patients and controls were compared using Statistical Parametric Mapping two-sample t-tests. Knee adduction moments and medial tibiofemoral contact forces of both the ipsi- and contralateral leg were not significantly different compared to healthy controls. The contralateral leg showed 14% higher medial tibiofemoral contact forces compared to the ipsilateral (operated) leg during the second half of stance. During the first half of stance, the lateral tibiofemoral contact force of the contralateral leg was 39% lower and the ratio 32% lower compared to healthy controls. In contrast, during the second half of stance the forces were significantly higher (39 and 26%, respectively) compared to healthy controls. The higher ratio indicates a changed distribution whereas the increased lateral tibiofemoral contact forces indicate a higher lateral knee joint loading in the contralateral leg in OA patients after total hip replacement (THR). Musculoskeletal modeling using a detailed knee model can be useful to detect differences in the load distribution between the medial and lateral knee compartment which cannot be verified with the knee adduction moment.
