Shaking on Standing: A Critical Review.

ABSTRACT: Orthostatic tremor is a rare condition, though its exact prevalence is unknown, which is clinically characterized by a feeling of unsteadiness or being about to fall on standing and which disappears on walking, sitting, or lying down. It is generally accepted that classic orthostatic tremor manifests with a high-frequency tremor (>13 Hz) of the legs when standing. However, a number of patients initially reported as orthostatic tremor did not actually have such electrophysiological features. It is our experience that there is a clinical spectrum of different conditions presenting as shaking on standing, and this highlights the importance of the electrophysiology to aid the differential diagnosis of these disorders. Here, we provide a critical review of the clinical spectrum of shaking on standing, along with demonstrative electrophysiological recordings of some of these conditions.

Wearable Inertial Sensors to Assess Standing Balance: A Systematic Review.

Project description:Wearable sensors are de facto revolutionizing the assessment of standing balance. The aim of this work is to review the state-of-the-art literature that adopts this new posturographic paradigm, i.e., to analyse human postural sway through inertial sensors directly worn on the subject body. After a systematic search on PubMed and Scopus databases, two raters evaluated the quality of 73 full-text articles, selecting 47 high-quality contributions. A good inter-rater reliability was obtained (Cohen's kappa = 0.79). This selection of papers was used to summarize the available knowledge on the types of sensors used and their positioning, the data acquisition protocols and the main applications in this field (e.g., "active aging", biofeedback-based rehabilitation for fall prevention, and the management of Parkinson's disease and other balance-related pathologies), as well as the most adopted outcome measures. A critical discussion on the validation of wearable systems against gold standards is also presented.

Pain catastrophizing: a critical review.

Project description:Pain catastrophizing is conceptualized as a negative cognitive-affective response to anticipated or actual pain and has been associated with a number of important pain-related outcomes. In the present review, we first focus our efforts on the conceptualization of pain catastrophizing, highlighting its conceptual history and potential problem areas. We then focus our discussion on a number of theoretical mechanisms of action: appraisal theory, attention bias/information processing, communal coping, CNS pain processing mechanisms, psychophysiological pathways and neural pathways. We then offer evidence to suggest that pain catastrophizing represents an important process factor in pain treatment. We conclude by offering what we believe represents an integrated heuristic model for use by researchers over the next 5 years; a model we believe will advance the field most expediently.

Project description:The technological advances of the last twenty years together with the dramatic increase in computational power have injected new life into systems-level thinking in Medicine. This review emphasizes the close relationship of Systems Pathology to Systems Biology and delineates the differences between Systems Pathology and Clinical Systems Pathology. It also suggests an algorithm to support the application of systems-level thinking to clinical research, proposes applying systems-level thinking to the health care systems and forecasts an acceleration of preventive medicine as a result of the coupling of personal genomics with systems pathology.

The effects of standing desks within the school classroom: A systematic review.

Project description:BackgroundThe school classroom environment often dictates that pupils sit for prolonged periods which may be detrimental for children's health. Replacing traditional school desks with standing desks may reduce sitting time and provide other benefits. The aim of this systematic review was to assess the impact of standing desks within the school classroom.MethodStudies published in English up to and including June 2015 were located from online databases and manual searches. Studies implementing standing desks within the school classroom, including children and/or adolescents (aged 5-18 years) which assessed the impact of the intervention using a comparison group or pre-post design were included.ResultsEleven studies were eligible for inclusion; all were set in primary/elementary schools, and most were conducted in the USA (n = 6). Most were non-randomised controlled trials (n = 7), with durations ranging from a single time point to five months. Energy expenditure (measured over 2 h during school day mornings) was the only outcome that consistently demonstrated positive results (three out of three studies). Evidence for the impact of standing desks on sitting, standing, and step counts was mixed. Evidence suggested that implementing standing desks in the classroom environment appears to be feasible, and not detrimental to learning.ConclusionsInterventions utilising standing desks in classrooms demonstrate positive effects in some key outcomes but the evidence lacks sufficient quality and depth to make strong conclusions. Future studies using randomised control trial designs with larger samples, longer durations, with sitting, standing time and academic achievement as primary outcomes, are warranted.

Phytocannabinoids reduce inflammation of primed macrophages and enteric glial cells Critical review of the effect of phytocannabinoids on gut immune cells

Project description:In vitro study analyzed the putative-anti-inflammatory effect of nine-selected pure cannabinoids on enteric glial cells (EGC’s) triggered to undergo inflammation with lipopolysaccharide (LPS) . Our results demonstrate that THC at the lower concentrations tested exerted the most effective anti- inflammatory effect in EGC’s compared to other phytocannabinoids tested herein. We then performed RNA-seq analysis of EGC’s exposed to LPS in the presence or absence of THC or THC-COOH. Transcriptomic analysis of these EGC’s revealed 23 differentially expressed genes (DEG) compared to treatment with only LPS. Pretreatment with THC resulted in 26 DEG and THC-COOH 25 DEG. To evaluate biological pathways affected by the different phytocannabinoid treatments we used the Ingenuity platform. We show that THC treatment but not THC-COOH affected

Project description:Care of patients with sepsis has improved over the last decade. However, in the recent two years, there was no significant progress in the development of a new drug for critically ill patients. In January 2011, it was announced that the worldwide phase 3 randomized trial of a novel anti-Toll-like receptor-4 compound, eritoran tetrasodium, had failed to demonstrate an improvement in the mortality of patients with severe sepsis. In October 2011, Xigris (drotrecogin alfa, a recombinant activated protein C) was withdrawn from the market following the failure of its worldwide trial that had attempted to demonstrate improved outcome. These announcements were disappointing. The recent failure of 2 promising drugs to further reduce mortality suggests that new approaches are needed. A study was published showing that sepsis can be associated to a state of immunosuppression and loss of immune function in human. However, the timing, incidence, and nature of the immunosuppression remain poorly characterized, especially in humans. This emphasizes the need for a better understanding of sepsis as well as new therapeutic strategies. Many clinical experiences of the extracorporeal membrane oxygenator (ECMO) treatment for adult acute respiratory distress syndrome (ARDS) patients, which is caused by the H1N1 influenza A virus, were reported. The use of ECMO in severe respiratory failure, particularly in the treatment of adult ARDS, is occurring more commonly.

Non-specific Low Back Pain and Postural Control During Quiet Standing-A Systematic Review.

Project description:Background: There is a great number of people who require treatment for non-specific low back pain (LBP) yet the causes are still unclear. One proposed cause for LBP is impaired motor control and more specific an impaired postural control. Objective: The purpose of this review is to provide an overview of postural control parameter differences in persons with and without non-specific LBP during quite standing. Methods: A literature search in five databases from January 2000 until January 2018 was performed and was followed by a hand search. Twenty-one articles comparing healthy adults and adults with non-specific LBP in neuromuscular and/or biomechanical parameters during bipedal stance without external perturbation in lab studies were examined. Data extraction and quality assessment were independently performed by two persons. Factors such as study population, outcome measures, and results were extracted from the articles and included in this analysis. Results: The results show that persons with and without non-specific LBP differed in several parameters of postural control such as the center of pressure displacement, postural control strategy, and muscle activation patterns. Conclusion: While the results show that none of the parameters alone lead to significant effects, the combination of neuromuscular and biomechanical parameters was associated with the impairment of postural control in individuals with LBP during standing. Since the studies included in this analysis used different methodological procedures a replication of these studies with standardized procedures is imperative for the acquisition of more conclusive evidence on the differences in postural control during standing.
