ABSTRACT: Highlights•Giant TAA is a life-threatening condition with a high risk for rupture.•TAA may compress the PAs and cause severe PH leading to RVF mimicking PTE.•PA rupture by an enlarged TAA is rare, and the prognosis is poor.•Inflammatory diseases are frequent causes of ascending aortic aneurysms in young patients.•Multimodality cardiovascular imaging is indispensable in assessing TAA.
