ABSTRACT: Highlights•Extravasation of contrast is a dreaded complication of PCI.•Swift assessment for coronary perforation is key to avoid catastrophic complications.•Wire perforation of coronary arteries is a known cause of coronary-cameral fistulas.•Combined fluoroscopy and echocardiography enable more precise diagnosis.•Most coronary-cameral fistulas are benign and require no treatment.
