Injury Occurrence and Return to Dance in Professional Ballet: Prospective Analysis of Specific Correlates.

ABSTRACT: Professional ballet is a highly challenging art, but studies have rarely examined factors associated with injury status in ballet professionals. This study aimed to prospectively examine gender-specific correlates of injury occurrence and time-off from injury in professional ballet dancers over a one-year period. The participants were 99 professional ballet dancers (41 males and 58 females). Variables included: (i) predictors: sociodemographic data (age, educational status), ballet-related factors (i.e., experience in ballet, ballet status), cigarette smoking, alcohol drinking, and consumption of illicit drugs; and (ii) outcomes: injury occurrence and time-off from injury. Participants were questioned on predictors at the beginning of the season, while data on outcomes were collected continuously once per month over the study period. Dancers reported total of 196 injuries (1.9 injuries (95% CI: 1.6⁻2.3) per dancer in average), corresponding to 1.4 injuries per 1000 dance-hours (95% CI: 1.1⁻1.7). In females, cigarette smoking was a predictor of injury occurrence in females (OR: 4.33, 95% CI: 1.05⁻17.85). Alcohol drinking was a risk factor for absence from dance in females (OR: 1.29, 95% CI: 1.01⁻4.21) and males (OR: 1.21, 95% CI: 1.05⁻3.41). Less experienced dancers were more absent from dance as a result of injury than their more experienced peers (Mann-Whitney Z: 2.02, p < 0.04). Ballet dancers and their managers should be aware of the findings of this study to make informed decisions on their behavior (dancers) or to initiate specific programs aimed at the prevention of substance use and misuse in this profession (managers).

Project description:BackgroundHigh-speed running is commonly implicated in the genesis of hamstring injury. The success of hamstring injury management is typically quantified by the duration of time loss or reinjury rate. These metrics do not consider any loss in performance after returning to play from hamstring injury. It is not known to what extent high-speed running is altered on return to play after such injury.HypothesisMatch high-speed running distance will change after returning from hamstring injury.Study designNon-randomized cohort.Level of evidenceLevel 3.MethodsMatch high-speed running distance in highest level professional football (soccer, Rugby League, Rugby Union, and Australian Rules) were examined for a minimum of 5 games prior and subsequent to hamstring strain injury for individual differences using a linear regression models approach. A total of 22 injuries in 15 players were available for analysis.ResultsPreinjury cumulative high-speed running distances were strongly correlated for each individual (r2 = 0.92-1.0; P < 0.0001). Pre- and postinjury high-speed running data were available for a median of 15 matches (range, 6-15). Variance from the preinjury high-speed running distance was significantly less (P = 0.0005) than the post injury values suggesting a suppression of high-speed running distance after returning from injury. On return to play, 7 of the 15 players showed a sustained absolute reduction in preinjury high-speed running distance, 7 showed no change, and 1 player (only) showed an increase. Analysis of subsequent (second and third injury) return to play showed no differences to return from the index injury.ConclusionReturn to play was not associated with return to high-speed running performance for nearly half of the players examined, although the same number showed no difference. Persisting deficits in match high-speed running may exist for many players after hamstring strain injury.Clinical relevanceReturning to play does not mean returning to (high-speed running) performance for nearly half of the high-level professional football players examined in this study. This suggests that successful return to play metrics should be expanded from simple time taken and recurrence to include performance.

Project description:BackgroundDespite being a common overuse entity in youth soccer, scientific data on risk factors, rehabilitation and return to play for long-standing pubic-related groin pain is still rare. The current prospective cohort study aims to evaluate potential risk-factors, propose a criteria-based conservative rehabilitation protocol and assess return-to-play outcomes among professional youth soccer players suffering from long-standing pubic-related groin pain.MethodsMale soccer players with long-standing (> 6 weeks) pubic-related groin pain from a professional soccer club's youth academy were analyzed for possible risk factors such as age, team (U12 - U23), younger/older age group within the team, position and preinjury Functional movement score. All injured players received a conservative, standardized, supervised, criteria-based, 6-level rehabilitation program. Outcome measures included time to return to play, recurrent groin pain in the follow-up period and clinical results at final follow-up two years after their return to play.ResultsA total of 14 out of 189 players developed long-standing pubic-related groin pain in the 2017/2018 season (incidence 7.4%). The average age of the players at the time of the injury was 16.1 ± 1.9 years. Risk factor analysis revealed a significant influence of the age group within the team (p = .007). Only players in the younger age group were affected by long-standing pubic-related groin pain, mainly in the first part of the season. Injured players successfully returned to play after an average period of 135.3 ± 83.9 days. Only one player experienced a recurrence of nonspecific symptoms (7.1%) within the follow-up period. The outcome at the 24-month follow-up was excellent for all 14 players.ConclusionsLong-standing pubic-related groin pain is an overuse entity with a markedly high prevalence in youth soccer players, resulting in a relevant loss of time in training and match play. In particular, the youngest players in each team are at an elevated risk. Applying a criteria-based rehabilitation protocol resulted in an excellent return-to-play rate, with a very low probability of recurrence.Trial registrationThe trial was retrospectively registered under DRKS00016510 in the German Clinical Trials Register on 19.04.2021.

Project description:BackgroundThere is no doubt that good bimanual performance is very important for skilled handball playing. The control of the non-dominant hand is especially demanding since efficient catching and throwing needs both hands.Methodology/hypothesesWe investigated training-induced structural neuroplasticity in professional handball players using several structural neuroimaging techniques and analytic approaches and also provide a review of the literature about sport-induced structural neuroplastic alterations. Structural brain adaptations were expected in regions relevant for motor and somatosensory processing such as the grey matter (GM) of the primary/secondary motor (MI/supplementary motor area, SMA) and somatosensory cortex (SI/SII), basal ganglia, thalamus, and cerebellum and in the white matter (WM) of the corticospinal tract (CST) and corpus callosum, stronger in brain regions controlling the non-dominant left hand.ResultsIncreased GM volume in handball players compared with control subjects were found in the right MI/SI, bilateral SMA/cingulate motor area, and left intraparietal sulcus. Fractional anisotropy (FA) and axial diffusivity were increased within the right CST in handball players compared with control women. Age of handball training commencement correlated inversely with GM volume in the right and left MI/SI and years of handball training experience correlated inversely with radial diffusivity in the right CST. Subcortical structures tended to be larger in handball players. The anatomical measures of the brain regions associated with handball playing were positively correlated in handball players, but not interrelated in control women.Discussion/conclusionTraining-induced structural alterations were found in the somatosensory-motor network of handball players, more pronounced in the right hemisphere controlling the non-dominant left hand. Correlations between handball training-related measures and anatomical differences suggest neuroplastic adaptations rather than a genetic predisposition for a ball playing affinity. Investigations of neuroplasticity specifically in sportsmen might help to understand the neural mechanisms of expertise in general.

Project description:PurposeTo determine whether differences in return-to-play (RTP) rate and performance-based outcomes exist following shoulder arthroscopy in athletes from the National Football League (NFL), National Basketball League (NBA), Major League Baseball (MLB), and National Hockey League (NHL).MethodsProfessional athletes from the NFL, NBA, MLB, and NHL who underwent shoulder arthroscopy between January 1998 and December 2016 were identified through an established review of injury reports and public archives. Sport-specific statistics were collected before and after shoulder arthroscopy for each athlete, providing a performance score. RTP was defined as the first game played postsurgery.ResultsOf the 208 professional athletes who met the inclusion criteria, 167 (80.3%) returned to play following shoulder arthroscopy, with MLB players returning at a significantly lower rate than those of other sports (P < .0001). NBA players had significantly shorter recovery times (201 days; P < .01) and MLB players had significantly longer recovery times (413 days; P < .001) when compared with athletes in other sports. The mean number of seasons played after shoulder arthroscopy was 3.7, 4.7, 4.8, and 5.8 for MLB, NFL, NHL, and NBA, respectively. NBA players performed worse in their first season postoperative compared with their preoperative performance (P = .0017), but their postoperative season 2 performance returned to their preoperative performance level (P = .1893). Similarly, NHL players performed worse in their postoperative season 1 compared with preoperative performance (P = .0274), but their postoperative season 2 performance improved upon their preinjury performance level (P = .0861).ConclusionsThere is a modest RTP rate among professional athletes following shoulder arthroscopy. MLB players have the longest average time to RTP and the shortest postinjury careers following shoulder arthroscopy. However, they demonstrate no significant decline in performance following their injury. Conversely, NBA players had the shortest average time to RTP and the longest post-injury career. Both NBA and NHL athletes experienced a decrease in performance in post-operative season one.Level of evidenceLevel III, retrospective comparative study.
