NONMEM Tutorial Part II: Estimation Methods and Advanced Examples.

ABSTRACT: In this second tutorial on NONMEM, examples of typical PK/PD modeling problems that occur in the pharmaceutical field will be presented, and which the reader can utilize as a template for his own modeling endeavors. Each of the problems presented are challenging in some way, and the logic behind setting up each problem is discussed. Logical concepts of the problem itself as well as the technical aspect of how to set it up in NONMEM are described and demonstrated. The concepts behind the various estimation algorithms will first be described to allow the user a better understanding of how to use them. This article is protected by copyright. Comparison of Parameter Estimation Methods in Stochastic Chemical Kinetic Models: Examples in Systems Biology.

Project description:Stochastic chemical kinetics has become a staple for mechanistically modeling various phenomena in systems biology. These models, even more so than their deterministic counterparts, pose a challenging problem in the estimation of kinetic parameters from experimental data. As a result of the inherent randomness involved in stochastic chemical kinetic models, the estimation methods tend to be statistical in nature. Three classes of estimation methods are implemented and compared in this paper. The first is the exact method, which uses the continuous-time Markov chain representation of stochastic chemical kinetics and is tractable only for a very restricted class of problems. The next class of methods is based on Markov chain Monte Carlo (MCMC) techniques. The third method, termed conditional density importance sampling (CDIS), is a new method introduced in this paper. The use of these methods is demonstrated on two examples taken from systems biology, one of which is a new model of single-cell viral infection. The applicability, strengths and weaknesses of the three classes of estimation methods are discussed. Using simulated data for the two examples, some guidelines are provided on experimental design to obtain more information from a limited number of measurements.

Nanopharmaceutics: Part II-Production Scales and Clinically Compliant Production Methods.

Project description:Due the implementation of nanotechnologies in the pharmaceutical industry over the last few decades, new type of cutting-edge formulations-nanopharmaceutics-have been proposed. These comprise pharmaceutical products at the nanoscale, developed from different types of materials with the purpose to, e.g., overcome solubility problems of poorly water-soluble drugs, the pharmacokinetic and pharmacodynamic profiles of known drugs but also of new biomolecules, to modify the release profile of loaded compounds, or to decrease the risk of toxicity by providing site-specific delivery reducing the systemic distribution and thus adverse side effects. To succeed with the development of a nanopharmaceutical formulation, it is first necessary to analyze the type of drug which is to be encapsulated, select the type matrix to load it (e.g., polymers, lipids, polysaccharides, proteins, metals), followed by the production procedure. Together these elements have to be compatible with the administration route. To be launched onto the market, the selected production method has to be scaled-up, and quality assurance implemented for the product to reach clinical trials, during which in vivo performance is evaluated. Regulatory issues concerning nanopharmaceutics still require expertise for harmonizing legislation and a clear understanding of clinically compliant production methods. The first part of this study addressing "Nanopharmaceutics: Part I-Clinical trials legislation and Good Manufacturing Practices (GMP) of nanotherapeutics in the EU" has been published in Pharmaceutics. This second part complements the study with the discussion about the production scales and clinically compliant production methods of nanopharmaceutics.

Targeted maximum likelihood estimation for a binary treatment: A tutorial.

Project description:When estimating the average effect of a binary treatment (or exposure) on an outcome, methods that incorporate propensity scores, the G-formula, or targeted maximum likelihood estimation (TMLE) are preferred over naïve regression approaches, which are biased under misspecification of a parametric outcome model. In contrast propensity score methods require the correct specification of an exposure model. Double-robust methods only require correct specification of either the outcome or the exposure model. Targeted maximum likelihood estimation is a semiparametric double-robust method that improves the chances of correct model specification by allowing for flexible estimation using (nonparametric) machine-learning methods. It therefore requires weaker assumptions than its competitors. We provide a step-by-step guided implementation of TMLE and illustrate it in a realistic scenario based on cancer epidemiology where assumptions about correct model specification and positivity (ie, when a study participant had 0 probability of receiving the treatment) are nearly violated. This article provides a concise and reproducible educational introduction to TMLE for a binary outcome and exposure. The reader should gain sufficient understanding of TMLE from this introductory tutorial to be able to apply the method in practice. Extensive R-code is provided in easy-to-read boxes throughout the article for replicability. Bayesian Methods for Calibrating Health Policy Models: A Tutorial.

Project description:Mathematical simulation models are commonly used to inform health policy decisions. These health policy models represent the social and biological mechanisms that determine health and economic outcomes, combine multiple sources of evidence about how policy alternatives will impact those outcomes, and synthesize outcomes into summary measures salient for the policy decision. Calibrating these health policy models to fit empirical data can provide face validity and improve the quality of model predictions. Bayesian methods provide powerful tools for model calibration. These methods summarize information relevant to a particular policy decision into (1) prior distributions for model parameters, (2) structural assumptions of the model, and (3) a likelihood function created from the calibration data, combining these different sources of evidence via Bayes' theorem. This article provides a tutorial on Bayesian approaches for model calibration, describing the theoretical basis for Bayesian calibration approaches as well as pragmatic considerations that arise in the tasks of creating calibration targets, estimating the posterior distribution, and obtaining results to inform the policy decision. These considerations, as well as the specific steps for implementing the calibration, are described in the context of an extended worked example about the policy choice to provide (or not provide) treatment for a hypothetical infectious disease. Given the many simplifications and subjective decisions required to create prior distributions, model structure, and likelihood, calibration should be considered an exercise in creating a reasonable model that produces valid evidence for policy, rather than as a technique for identifying a unique theoretically optimal summary of the evidence.

A Tutorial on Multilevel Survival Analysis: Methods, Models and Applications.

Project description:Data that have a multilevel structure occur frequently across a range of disciplines, including epidemiology, health services research, public health, education and sociology. We describe three families of regression models for the analysis of multilevel survival data. First, Cox proportional hazards models with mixed effects incorporate cluster-specific random effects that modify the baseline hazard function. Second, piecewise exponential survival models partition the duration of follow-up into mutually exclusive intervals and fit a model that assumes that the hazard function is constant within each interval. This is equivalent to a Poisson regression model that incorporates the duration of exposure within each interval. By incorporating cluster-specific random effects, generalised linear mixed models can be used to analyse these data. Third, after partitioning the duration of follow-up into mutually exclusive intervals, one can use discrete time survival models that use a complementary log-log generalised linear model to model the occurrence of the outcome of interest within each interval. Random effects can be incorporated to account for within-cluster homogeneity in outcomes. We illustrate the application of these methods using data consisting of patients hospitalised with a heart attack. We illustrate the application of these methods using three statistical programming languages (R, SAS and Stata).

The risk of bias in denoising methods: Examples from neuroimaging.

Project description:Experimental datasets are growing rapidly in size, scope, and detail, but the value of these datasets is limited by unwanted measurement noise. It is therefore tempting to apply analysis techniques that attempt to reduce noise and enhance signals of interest. In this paper, we draw attention to the possibility that denoising methods may introduce bias and lead to incorrect scientific inferences. To present our case, we first review the basic statistical concepts of bias and variance. Denoising techniques typically reduce variance observed across repeated measurements, but this can come at the expense of introducing bias to the average expected outcome. We then conduct three simple simulations that provide concrete examples of how bias may manifest in everyday situations. These simulations reveal several findings that may be surprising and counterintuitive: (i) different methods can be equally effective at reducing variance but some incur bias while others do not, (ii) identifying methods that better recover ground truth does not guarantee the absence of bias, (iii) bias can arise even if one has specific knowledge of properties of the signal of interest. We suggest that researchers should consider and possibly quantify bias before deploying denoising methods on important research data.

A tutorial on propensity score estimation for multiple treatments using generalized boosted models.

Project description:The use of propensity scores to control for pretreatment imbalances on observed variables in non-randomized or observational studies examining the causal effects of treatments or interventions has become widespread over the past decade. For settings with two conditions of interest such as a treatment and a control, inverse probability of treatment weighted estimation with propensity scores estimated via boosted models has been shown in simulation studies to yield causal effect estimates with desirable properties. There are tools (e.g., the twang package in R) and guidance for implementing this method with two treatments. However, there is not such guidance for analyses of three or more treatments. The goals of this paper are twofold: (1) to provide step-by-step guidance for researchers who want to implement propensity score weighting for multiple treatments and (2) to propose the use of generalized boosted models (GBM) for estimation of the necessary propensity score weights. We define the causal quantities that may be of interest to studies of multiple treatments and derive weighted estimators of those quantities. We present a detailed plan for using GBM to estimate propensity scores and using those scores to estimate weights and causal effects. We also provide tools for assessing balance and overlap of pretreatment variables among treatment groups in the context of multiple treatments. A case study examining the effects of three treatment programs for adolescent substance abuse demonstrates the methods.

Permafrost Dynamics Observatory-Part I: Postprocessing and Calibration Methods of UAVSAR L-Band InSAR Data for Seasonal Subsidence Estimation.

Project description:Interferometric synthetic aperture radar (InSAR) has been used to quantify a range of surface and near surface physical properties in permafrost landscapes. Most previous InSAR studies have utilized spaceborne InSAR platforms, but InSAR datasets over permafrost landscapes collected from airborne platforms have been steadily growing in recent years. Most existing algorithms dedicated toward retrieval of permafrost physical properties were originally developed for spaceborne InSAR platforms. In this study, which is the first in a two part series, we introduce a series of calibration techniques developed to apply a novel joint retrieval algorithm for permafrost active layer thickness retrieval to an airborne InSAR dataset acquired in 2017 by NASA's Uninhabited Aerial Vehicle Synthetic Aperture Radar over Alaska and Western Canada. We demonstrate how InSAR measurement uncertainties are mitigated by these calibration methods and quantify remaining measurement uncertainties with a novel method of modeling interferometric phase uncertainty using a Gaussian mixture model. Finally, we discuss the impact of native SAR resolution on InSAR measurements, the limitation of using few interferograms per retrieval, and the implications of our findings for cross-comparison of airborne and spaceborne InSAR datasets acquired over Arctic regions underlain by permafrost.
