Abstract:
We present and analyze a model for the interaction of human immunodeficiency
virus type 1 (HIV-1) with target cells that includes a time delay between
initial infection and the formation of productively infected cells. Assuming
that the variation among cells with respect to this 'intracellular' delay can be
approximated by a gamma distribution, a high flexible distribution that can
mimic a variety of biologically plausible delays, we provide analytical
solutions for the expected decline in plasma virus concentration after the
initiation of antiretroviral therapy with one or more protease inhibitors. We
then use the model to investigate whether the parameters that characterize viral
dynamics can be identified from biological data. Using non-linear least-squares
regression to fit the model to simulated data in which the delays conform to a
gamma distribution, we show that good estimates for free viral clearance rates,
infected cell death rates, and parameters characterizing the gamma distribution
can be obtained. For simulated data sets in which the delays were generated
using other biologically plausible distributions, reasonably good estimates for
viral clearance rates, infected cell death rates, and mean delay times can be
obtained using the gamma-delay model. For simulated data sets that include added
simulated noise, viral clearance rate estimates are not as reliable. If the mean
intracellular delay is known, however, we show that reasonable estimates for the
viral clearance rate can be obtained by taking the harmonic mean of viral
clearance rate estimates from a group of patients. These results demonstrate
that it is possible to incorporate distributed intracellular delays into
existing models for HIV dynamics and to use these refined models to estimate the
half-life of free virus from data on the decline in HIV-1 RNA following
treatment.

Project description:Accurate diagnosis of Alzheimer’s disease (AD) in its earliest stage can prevent the disease and delay the onset of symptoms by maintaining a healthy lifestyle and controlling modifiable risk factors. Therefore, more sensitive, relatively inexpensive, non-invasive, and simple screening tools are required for the early diagnosis and progression monitoring of AD. Here, we designed a self assembled nanoparticle-mediated amplified fluorogenic immunoassay (SNAFIA) consisting of magnetic and fluorophore-loaded polymeric nanoparticles. Multiple simultaneous captures and magnetic separation of biomarkers and target-specific fluorophore emission induced a one-tomulti signal, resulting in a low detection limit (236 aM), good reliability (R2 = 0.991), and wide analytical range (0.320–1000 fM) for APOE and CAP1 biomarkers in human serum and tear fluid. More importantly, SNAFIA successfully differentiated AD patients from healthy controls with 90% sensitivity and 100% specificity in less than 1 h in 39 clinical tear samples. As an easily accessible diagnostic tool with fast and accurate results, SNAFIA can help diagnose AD early to aid in long-term care planning, increase the efficiency of clinical trials, and dramatically accelerate therapeutic development.

Project description:Individuals infected with SARS-CoV-2 vary greatly in their symptomatology and disease progression, likely as a result of numerous genetic, biological and environmental factors and their complex interactions. Meanwhile, the potential roles of microRNAs (miRNAs) in SARS-CoV-2 infection have not been fully described. MiRNAs have emerged as key post-transcriptional regulators of gene expression, and their dysregulation can be indicative of aberrant immune function. In this study, we characterize the potential roles of mIRNAs in early COVID-19 disease progression. We studied a diverse cohort of 259 patients admitted to hospitals in Abu Dhabi, United Arab Emirates to understand the clinical and biological factors associated with ICU admission during COVID-19 treatment, integrating electronic health records (EHR), global miRNA and RNA expression, and genotyping data. Using EHR, we identified 26 factors correlated with ICU admission, including 8 blood phenotypes such as neutrophil-to-lymphocyte ratio, Interleukin-6, and C-reactive protein levels. Using genome-wide miRNA expression data for a subset of 96 individuals from Southeast Asia and the Middle East and North Africa, we identified 27 miRNAs significantly associated with ICU admission (p < 0.01), and 97 miRNAs associated with at least one of the 8 blood phenotypes. [cross-cor] Integrating expression data for 632 miRNAs and genotyping data for ~260,000 SNPs, we identified 168 significant cis-expression quantitative trait loci (cis-eQTLs), of which 59 were associated with either ICU admission or one of the 8 blood phentoypes. Overall, our findings characterize the miRNA architecture of blood phenotypes during the early stages of COVID-19 infection, identify miRNAs associated with ICU admission and therefore COVID-19 disease severity, and suggest a potential genetic control of miRNA expression during early COVID-19 disease progression.

Project description:<p>Rheumatoid arthritis (RA) is the most common autoimmune inflammatory arthritis worldwide and affects 1.3 million adults in the USA. It has previously been studied using phenotype algorithms to identify electronic health records (EHR) case cohorts. Early genetic studies of EHR-linked cohorts of RA patients have been replicated in known associations. Further development of collections of EHR-linked cohorts for RA and other phenotypes may enable not only enhanced understanding of disease risks but also the investigation of outcomes and treatment responses.</p>

Project description:Colorectal cancer (CRC) is diagnosed in one million patients each year worldwide and is the 2nd leading cause of cancer death. Peritoneal metastasis (MP) is seen in 10% of CRC patients and is the metastatic site with the worst prognosis.
Complete cytoreduction surgery (CCS) is the only treatment that allows for prolonged survivals. Five-year overall survival (OS) after CCS ranges from 30% to 60% compared with 0 to 5% with exclusive medical treatment.
Chemotherapy (CT) with fluoropyrimidine and oxaliplatin and/or irinotecan 3 months pre-operatively and 3 months adjuvant is widely used. The benefits of perioperative CT have been demonstrated in another resectable metastatic site, the liver and has become by extension a therapeutic standard in France for CRC MPs. However, the impact of delay in the initiation of surgery and adjuvant or neoadjuvant CT is unknown for CRC MPs.
Several deleterious oncologic effects are related to a long period without treatment between

* Surgery and adjuvant chemotherapy.
Several meta-analyses have demonstrated, for at least 13 different cancers, a continuous association between delays in treatment (CT, radiotherapy, or surgery) and cancer mortality. For CRC, Hanna’s meta-analysis showed that for every 4-week delay in adjuvant surgery or CT, the risk of cancer death increased by 6 and 13%, respectively.
These long delays between CT and excisional surgery also decrease survival in patients with liver metastases from colorectal origins and MPs from ovarian origins but this has never been evaluated in patients with MPs from colorectal origins.
Demonstrating an oncologic impact of therapeutic delays would have several strategic practical impacts such as:
* Promoting pre- and post-operative rehabilitation programs to facilitate recovery and reduce time to retreatment.
* To use more easily techniques (protective stoma, multi-stage surgery) limiting the risk of complications and therefore the delays in treatment.
* Propose clinical research protocols aimed at reducing these delays with knowledge of plausible statistical hypotheses.
A therapeutic strategy of shortening the time between each treatment therefore deserves to be evaluated in metastatic forms of colorectal cancer.
The investigators would like to evaluate the hypothesis that shortened time between treatments could have a prognostic impact on recurrence-free survival.

Project description:Obesity is well recognized as a risk factor for coronary heart disease and mortality. The relationship between abdominal obesity and ischemic stroke remains less clear. Previous publication showed the obesity is an independent, potent risk factor for ischemic stroke in all race-ethnic groups. It is a stronger risk factor than BMI and has a greater effect among younger persons. The goal of this experiment was to compare genome wide enrichment of H3K9ac histone mark profile of white blood cells of healthy controls, patients with obesity and/or stroke in order to understand the histone modifications differences behind the different phenotypes. There were 3 subjects in each group.
