Comparison of Hemoccult, Magstream and OC-Sensor Faecal Occult Blood Tests in Colorectal Cancer Screening

Project description:Colorectal cancer screening by faecal occult blood test (FOBT) is a high public health priority. The interest of guaiac tests (G-FOBT) is limited by their poor sensitivity, while the superiority of I-FOBT in comparison with G-FOBT is now established. Nevertheless automated quantitative I-FOBTs have not been compared, and the optimal number of samples and threshold is not yet fixed. The aim of this study is to compare the performances of the 2 more well-known I-FOBTs with automated analyzers (magstream by Fujirebio, and OC Sensor by Eiken) for different positivity thresholds and numbers of samples in general average risk population. Patients will performed a two samples Magstream, a two samples OC Sensor and Hemoccult II. In case of a positive test, a colonoscopy will be performed. Sensitivity and specificity for detection of cancer and advanced neoplasias will be compared between tests using ratio of sensitivities (RSN) and ratio of false positives (RFP) according to number of samples and positivity threshold.

Immunochemical FOBT screening for colorectal cancer: yield and attendance for one- versus two-sample tests, and for second round screening with various intervals.

Project description:Interventions: 1-sample or 2-samples immunochemical FOBTs. Firstly, a representative population sample of 2.500 asymptomatic persons between 50-75 years of age in the region ‘Rijnmond’ will be invited for the 1st screening round using 1-sample iFOBT in 2008 and for a 2nd screening round after one year (group A). Secondly, the 5.000 asymptomatic persons who were invited in our first trial for a 1st-round one-sample iFOBT in 2006/2007 (NTR = 1096), will be invited for 2nd round screening. They will be pre-randomized for biennial (group B) or triennial (group C) 2nd round screening. The attendance rate of these two groups will be compared with the participation of the second screening round from group A. Furthermore, 3.200 newly randomly selected persons will be invited for a single 1st round screening using 2-sample iFOBT (group D). The attendance rate of this group will be compared with the participation of the first screening round from group A. Primary outcome(s): Adherence for second round CRC screening with 1-sample iFOBT (OC-Hemodia Latex) comparing annual, biennial, and triennial screening interval. Adherence and diagnostic yield of a first screening round comparing 1-sample versus 2-sample iFOBT screening. Colorectal Cancer Screening in Average-risk Population: Immunochemical Fecal Occult Blood Testing Versus Colonoscopy

Project description:Aims: 1. To compare the efficacy of biennial immunochemical fecal occult blood test (iFOBT) versus colonoscopy every 10 years for the reduction of colorectal cancer-related mortality at 10 years in average-risk population. 2. To determine the compliance and complications associated with both strategies. Methods: Multicenter, randomized, controlled study in 8 Spanish regions (Aragón, Canarias, Catalunya, Euskadi, Galicia, Madrid, Murcia and Valencia). Study groups: * Group I: iFOBT (OC Sensor) in one stool sample, followed by colonoscopy when a positive result. * Group II: colonoscopy. Sample-size calculation: 27,749 subjects in each study group (total: 55,498).

Assessing the impact of a new fit test in the context of a population based organized screening programme for colorectal cancer

Project description:Background and study aims Colorectal, or bowel cancer, is very common. However, if detected at an early stage, it can be easier to treat and led to a much better chance of survival. The fecal immunochemical test (FIT) is a screening test for bowel cancer that tests for hidden blood in the stool. Blood in the stool can be an early sign of the disease. This study is investigating a new FIT screening test called FIT HM-Jackarc. It compares how this test performs against the test that is in routine use (FIT OC-Sensor Diana). Who can participate? Adults aged between 50-75. What does the study involve? Participants are randomly allocated to one of two groups. One group is screened for colorectal cancer using FIT HM-Jackarc. The other group is screened for colorectal cancer using FIT OC-Sensor Diana. All participants from both groups receive an invitation kit though the post. This includes a consent form for them to sign and a leaflet describing the study. They are asked to store their stool sample at 4°C and send it back to the laboratory using the pre-paid envelope provided. The analyses are performed by experienced laboratory medicine professionals. Participants found to have hidden blood in their stool undergo a colonoscopy (a procedure where a camera is used to look at the inside of the bowel).

A Universal 384-Well Proteomic Sample Preparation Platform for High-Throughput Screening and Biomarker Discovery

Project description:Major advances have been made to improve the sensitivity of mass analyzers, spectral quality, and the speed of data processing enabling more comprehensive proteome discovery and quantitation. While focus has recently begun shifting toward robust proteomic sample preparation efforts, a high throughput proteomics sample preparation is still lacking. We report the development of a highly-automated universal 384-well plate sample preparation platform with high reproducibility and adaptability for extraction of proteins from cells within a culture plate. Digestion efficiency was excellent in comparison to a commercial digest peptide standard with minimal sample loss while improving sample preparation throughput by 20- to 40-fold (<1 min/sample for entire process from cells to clean peptides). Analysis of six human cell types, which included two primary cell samples, identified and quantified approximately 4,000 proteins for each sample in a single HPLC-MS/MS injection with only 100 -10,000 cells, thus demonstrating universality of the platform.
