The Bowel Research Collaborative Data Tracking System Implementation Study

ABSTRACT: The project’s primary purpose is to establish a network of Hospice Palliative Care settings using a common and consistent method of assessing and documenting bowel functioning in order to be able to carry out future collaborative studies of constipation treatments. The secondary goal is to gather normative data on current bowel care function and outcomes of current treatments which can be used to determine sample size calculations for future controlled trials of bowel management protocols. In order to complete this goal the project requires a thorough assessment of current nursing practice in regard to bowel care.

Breakthrough Improvement Collaborative for ColoRectal Cancer (BIC4CRC)

Project description:Colorectal cancer is the most common cancer in Europe and the third worldwide. Approximately 1 in 20 men and 1 in 35 women will develop colorectal cancer at some moment in their life. In Flanders, in 2014, there was an increase in the detection of colorectal cancer with 21% compared to 2013. Early detection improves the prognosis for the patient. In this early stage, colorectal surgery is one of the most important treatments, but it is also complex and has a high complication rate. However, over the last decade, surgical care for patients with colorectal cancer has become more standardized. The use of structured care methods, such as care pathways and protocols, has helped in standardizing care processes. Specifically for patients with colorectal cancer, perioperative care has shifted with the implementation of Enhanced Recovery After Surgery (ERAS) programs. The goal of ERAS- protocols is to optimize the interventions during the perioperative hospitalization period and reduce postoperative complications. Despite the increasing evidence in favor of the use of these standardized protocols, adherence and implementation in daily practice remains challenging. The primary goal of this quality improvement project is to enhance the standardization of key interventions in the ERAS care process for patients undergoing colorectal surgery. Therefore, adherence to the ERAS-guidelines will be investigated and hospitals will receive feedback to set up improvement initiatives. Moreover, interactive group sessions and on-site training activities will stimulate knowledge sharing and define best practices.

Transcriptomic changes induced by a new Src Family Kinase Inhibitor and proton radiation in glioblastoma cells

Project description:Glioblastoma multiforme (GBM) is a central nervous system tumour. The current standard care for GMB is surgical resection, followed by conventional radiotherapy (RT) often combined with daily drug (temozolomide) administration. However, these treatment modalities are not currently curative and the resistance to both chemotherapy and RT plans is the main cause of GBM care failures. In this scenario, proton therapy (PT) could be used as a successful strategy for GBM treatment, being able to regulate the balance between tumour control and the normal tissue tolerance. In addition, new drugs could be used as radiosensitizing agents combined with PT in order to optimize GBM care. In this work we analyzed the GEP of the U87 MG human glioblastoma cell line, after treatment with PT alone or in combination with a new targeted compound, named Si306 (property of Lead Discovery Siena, Siena, Italy). We described GEP induced by both treatments, highlighting for the first time, the cell pathways induced by Si306. Summarizing, our results suggesting this compound as a novel possible candidate to treat GBM in combination with PT.

Project description:Extramural venous invasion (EMVI) is the spread of microscopic tumour cells into the veins around the tumour. Rectal cancer treatment has improved greatly over recent years. However, it is important for us to learn as much about the tumours as possible in order to develop newer therapies. Current treatments may benefit from new genetic information relating to the cancer. We hope to identify genetic differences in certain types of rectal cancer which will allow future treatments.

Transcriptomic changes induced by Curcumin loaded solid nanoparticles and radiation in breast cells

Project description:In breast cancer (BC) care, radiotherapy is considered an efficient treatment. However, approximately 90% of BC-related deaths are due to the metastatic tumor progression. Then, it is strongly desirable to improve tumors radiosensitivity using molecules with synergistic action. In this study, we developed curcumin-loaded solid nanoparticles (Cur-SLN) in order to increase curcumin bioavailability and evaluated their radiosensitizing ability in comparison to curcumin free (Cur), by using an in vitro approach on BC models. Precisely, here we analyzed the transcriptomic profiles induced by Cur-SLN treatments, in non tumorigenic MCF10A and tumorigenic MCF7 and MDA-MB-231 BC cell lines, highlighting the networks responsible of its radiosensitization ability. Curcumin loaded- SLN can be suggested in future preclinical and clinical studies to test its concomitant use during radiotherapy treatments with the double implications of being a radiosensitizing molecule against cancer cells, with a protective role against IR side effects.

Project description:Inflammatory bowel diseases are highly debilitating conditions that require constant monitoring and life-long medication. Current treatments are focused on systemic administration of immunomodulatory drugs, but they have a broad range of undesirable side-effects. The RNA interference is a highly specific endogenous mechanism that regulates the expression of the gene at the transcript level, which can be repurposed using exogenous short interfering RNA (siRNA) in order to repress the target gene’s expression. While siRNA therapeutics can offer an alternative to existing therapies, with a high specificity critical for chronically administrated drugs, evidence of their potency compared to chemical kinase inhibitors used in clinics is still lacking in alleviating an adverse inflammatory response. In this experiment, we compared the transcriptomic profile of cells transfected 48hv by siRNA targeting JAK1 and JAK3 (100 pM) to those of cells treated 48h with the most recent and promising kinase inhibitors for Janus kinases (Jakinibs), Tofacitinib (1 µM) and Filgotinib (5 µM).

Exosomes from mesenchymal stromal cells reduce murine colonic inflammation via a macrophage-dependent mechanism

Project description:Conventional treatments for inflammatory bowel disease (IBD) have multiple potential side effects. Therefore, alternative treatments are desperately needed. In the present study, we proposed a new therapeutic tool for the treatment of IBD in murine pre-clinical models by using MSC-Exos. We demonstrated that the infused MSC-Exos are immunotolerated by the host, which is convenient for a future clinical application of MSC-Exos in IBD. To understand the molecular basis mediating the anticolitic benefit of MSC-Exos, we performed proteomic analysis using iTRAQ technology to detect the protein expression profiles in MSC-Exos and the corresponding supernatants from their parent cell MSCs. Proteomic analysis revealed that MSC-Exos were enriched in proteins involved in regulating multiple biological processes associated with the anti-colitic benefit of MSC-Exos. Particularly, metallothionein-2 in MSC-Exos was required for the suppression of inflammatory responses in macrophages. Taken together, MSC-Exos are critical regulators of inflammatory responses and may be promising candidates for IBD treatment.

Methylation Profiling of Medulloblastoma in a Clinical Setting Permits Sub-Classification and Reveals New Outcome Predictions

Project description:In this study, we report the first examination of Medulloblastoma (MB) methylation patterns in Arab world, using an extensive primary tumor cohort in tertiray care center. We assessed DNA methylation patterns to sub-classify clinical MB cases and their amenability to clinical applications from archival biopsy material (FPPE). We herein establish methylation events as clinically useful biomarkers and demonstrate how their incorporation into current risk-stratification schemes could significantly improve the accuracy of survival predictions in clinical setting. This holds potential for future precision therapeutic approaches aimed at improving the outcomes of afflicted MB patients.
