Study With Dendritic Cell Immunotherapy in Resected Hepatic Metastasis of Colorectal Carcinoma

ABSTRACT: In this randomized multicentric phase II study, patients with colorectal carcinoma with resectable hepatic metastasis will be randomized to treatment with dendritic cells or to observation, following conventional treatment with surgery and chemotherapy.

LV-Induced Dendritic Cells for AML Immunotherapy

Project description:WilmsM-bM-^@M-^Y tumor 1 antigen (WT1) is overexpressed in acute myeloid leukemia (AML). Unfortunately, clinical immunotherapeutic use of WT1 peptides against AML has been inconclusive. A tricistronic lentiviral vector co-expressing a truncated form of WT1 (lacking the DNA-binding domain), GM-CSF and IL-4 was used to transduce human monocytes. The overnight transduction procedure induced the self-differentiation of monocytes into highly viable and immunophenotypically stable M-bM-^@M-^\SmartDC/tWT1M-bM-^@M-^] (GM-CSF+, IL-4+, WT1+, IL-6+, IL-8+, TNF-?+, MCP-1+, HLA-DR+, CD86+, CCR2+, CCR5+). RNA expression pattern analyses revealed up-regulation of several toll-like receptors, interferons and interferon-stimulated genes in lentivirally-reprogrammed SmartDC/tWT1 in comparison with conventional DCs. Remission samples from a FLT3-ITD+ AML patient and surplus material from the donor lymphocyte infusion (DLI) obtained from the matched related donor were used to produce SmartDC/tWT1. In our donor-patient model, donor T cells expanded with SmartDC/tWT1 showed high reactivity against WT1 and against primary AML. Intracellular processing of WT1 peptides by SmartDC/tWT1 resulted in superior WT1-specific CD8+ cytotoxicity against the primary leukemia blasts than exogenous peptide pulsing. Therefore, SmartDC/tWT1 produced in a single day of ex vivo culture with broad homeostatic, innate immunity, antigenic presentation and cytotoxic T cell stimulation properties can be clinically developed in combination with DLI for boosting anti-leukemia responses. Seeking to better characterize possible effects of truncated WT1 over-expression in the transcriptional activity of dendritic cells, we compared the gene expression pattern of SmartDC/tWT1 with those of SmartDC and Conventional DC.

In vivo dendritic cell reprogramming for cancer immunotherapy [organoid scRNA]

Project description:Immunotherapy can lead to long-term survival for some cancer patients, yet generalized success has been hampered by insufficient antigen presentation and exclusion of immunogenic cells from the tumor microenvironment. Here, we developed an approach to reprogram tumor cells in vivo by adenoviral delivery of the transcription factors PU.1, IRF8, and BATF3, which enabled them to present antigens as type 1 conventional dendritic cells. Reprogrammed tumor cells remodeled their tumor microenvironment, recruited, and expanded polyclonal cytotoxic T cells, induced tumor regressions, and established long-term systemic immunity in multiple mouse melanoma models. In human tumor spheroids and xenografts, reprogramming to immunogenic dendritic-like cells progressed independently of immunosuppression, which usually limits immunotherapy. Our study paves the way for human clinical trials of in vivo immune cell reprogramming for cancer immunotherapy.

Treatment Outcomes of Hepatic Metastasis After FOLFOX-4 Therapy

Project description:The role of surgical resection in the subset of patients with resectable hepatic metastases converted from initially non-resectable liver metastasis was still not clearly established. To further explore the oncologic results of surgical versus non-surgical methods for the treatment of this subset of patients, we designed and conducted the present randomized prospective study beginning in 2002. The present study was based on the following arguments against the predominant survival benefits of surgical resection in previous reported series: (1) The initially non-resectable liver metastasis was basically a disseminated disease, even though some metastases were highly responsive to chemotherapy and become resectable; (2) Since the evaluation of resectability was based on the imaging studies, it was difficult to consider the surgical resection as "curative" for the resectable hepatic metastases converted from non-resectable ones, given the limitation of the current imaging stools of high-technology; (3) The resectable hepatic metastases after neoadjuvant chemotherapy might represent a subset of hepatic metastases biologically highly responsive to chemotherapy and the time-to-progression for these metastases might be fairly long after a response. Additionally, these metastases might be also biologically responsive to other cytotoxic or targeted therapies that justified the patients’ continuous adoption of non-surgical treatment.

CD5 expression by dendritic cells directs T cell immunity and sustains immunotherapy responses

Project description:The induction of proinflammatory T cells by dendritic cell (DC) subtypes is critical for anti-tumor responses and effective immune checkpoint blockade (ICB) therapy. Here we show that human CD1c+CD5+ dendritic cells are reduced in melanoma-affected lymph nodes, with CD5 expression on DCs correlating with patient survival. Activating CD5 on DCs enhanced T-cell priming and improved survival after immune checkpoint blockade therapy. CD5+ DC numbers increased during ICB treatment, and low IL-6 levels promoted their de novo differentiation. Mechanistically, CD5 expression by DCs was required to generate optimally protective CD5hi T helper and CD8+ T cells; further, deletion of CD5 from T cells dampened tumor elimination in response to ICB therapy in vivo. Thus, CD5+ dendritic cells are an essential component of optimal immune checkpoint blockade therapy.
