ABSTRACT: This study will randomise patients undergoing screening colonoscopy to receive Entonox either continuously or as required. Both these methods are used in the published studies of Entonox and in clinical practice. Distinct microglial transcriptional responses during opioid exposure or neuropathic pain [mouse]

Project description:In settings of heightened pain sensitivity, such as following peripheral nerve injury (PNI) or opioid-induced hyperalgesia (OIH), microglia take on an activated phenotype. Functional studies have suggested that microglia activated by PNI or chronic opioids then engage common mechanisms to facilitate pain. Here we conducted RNA sequencing of acutely isolated spinal cord microglia to comprehensively interrogate commonality between PNI and OIH. By combining our results with meta-analysis of published datasets, we identify transcriptional signatures of microglial reactivity that differ between PNI models over time, opioid exposure, or CNS pathology, despite similar histological outcomes. Collectively, these results reveal a discrepancy between histological markers of activation and transcriptional response, and provide a resource of pain-associated microglial transcriptomes that caution against a universal signature of microglia activation.

Project description:Nociception is protective and prevents tissue damage but can also facilitate chronic pain. If a general principle governs these two types of pain is unknown. Here, we show that both basal mechanical and neuropathic pain are controlled by microRNA-183 cluster in mice. This single cluster controls more than 80% of neuropathic pain-regulated genes and scales basal mechanical sensitivity and mechanical allodynia by regulating auxiliary voltage-gated calcium channel subunits a2d. Basal sensitivity is controlled in nociceptors and allodynia involves TrkB+ light-touch mechanoreceptors. These light-touch sensitive neurons that normally do not elicit pain produce pain during neuropathy that is reversed by gabapentin. Thus, a single miRNA cluster continuously scales acute noxious mechanical sensitivity in nociceptive neurons and suppresses neuropathic pain transduction in a specific, light-touch sensitive neuronal type recruited during mechanical allodynia.

A randomised controlled study evaluating the use of water during a colonoscopy in patients to facilitate the performance of the colonoscopic procedure

Project description:Interventions: The use of room temperature water as an adjunctive method for distension of colon lumen during colonoscopy. Approximately 300ml of sterile water will be instilled into the colon during the colonoscopy procedure. The addition of water will take approximately 2-3 minutes to the overall time of the procedure. Additional gas insufflation (the normal standard of care) will also be used in addition to the water to achieve adequate colon distension to facilitate the procedure. Patients will be randomised to either the water assisted group or the standard of care. patients will be blinded to which arm they have been allocated to, but the operator will be unblinded. Standard parameters will be monitored and recorded during the procedure in both arms to assess the effect/difference between the two groups Primary outcome(s): Time to caecal intubation[When the caecal pole is reached];Patient analgesic scores. Pain will be assessed by using a visual analog scale of 1-10 (Wong-Baker Faces Pain rating scale) [At conclusion of procedure and if further analgesia is requested by the patient during the procedure.];Sedation requirements. The required quantity of midazolam and fentanyl will be collated and recorded at the conclusion of the procedure. Any extra analgesia during the course of the procedure will be noted in addition to the induction analgesia. Differentially expressed long non-coding RNAs and mRNAs in DRG of bone cancer pain and sham group rats based on microarray and bioinformatic analysis

Project description:Objective: The diagnostic roles of long non-coding RNAs (lncRNAs) and mRNAs in bone cancer pain are still not well defined. We aimed to identify differentially expressed DRG mRNAs in bone cancer pain and sham group rats systematically. Methods: Expression profile of mRNAs in DRG from bone cancer pain and sham group rats was analyzed by microarray assay. Gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway based approaches were used to investigate biological functions and signaling pathways affected by the differentially expressed mRNAs. GPR160 was selected for validation using Real-Time PCR. Results: Compared with sham group, a total of 807 differentially expressed mRNAs were revealed by heatmap and volcano-plot, including up-regulation in 720 and down-regulation in 87 (fold change ≥ 2 and P < 0.05), respectively. Pathways analysis based on upregulated mRNAs were mainly involved in antigen processing and presentation and phagosome pathway (P < 0.05). We first reported differentially expressed mRNAs in bone cancer pain and sham group rats systemically. Combined with the published dataset, we identified several differentially expressed mRNAs which might to be diagnostic or prognostic markers. The biological functions of those mRNAs should be further validated. Taken together, this study provided the basis for future treatment of bone cancer pain.

Bowel Preparation for elective Left-Sided Colonic Surgery

Project description:Study hypothesis: The objective of this study is to ascertain the best method of bowel preparation, prior to elective left-sided colonic surgery in terms of patient comfort and surgical efficacy. Sodium phosphate enemas (Colex) and bisacodyl (tablet and suppository) are to be compared. The occurrence of infections (wound, peritonitis) and anastomotic leaks will be monitored. Null hypotheses are: 1. Patients in both groups experience pain and discomfort equally 2. The surgeon finds no difference in the condition of the left hemicolon intraoperatively 3. No difference in incidence of infection is found 4. No difference in incidence of anastomotic leaks is found Primary outcome(s): Questionnaires will be used to assess the opinions of patients and surgeons. The parameters are recorded in a five point scale.
