Project description:The purpose of this study is to assess if follow-up telephone calls after colorectal surgery affects a patient’s satisfaction, the outcome of their surgery, and their quality of life. In addition, readmissions, complications and emergency room visits can be tracked via these telephone calls, ensuring optimal communication between patients and the surgical office.

Project description:Interventions: The recruitment process will follow 2 ways: 1.Based on the characterization, the target population recorded in the database of the insurance health company will be invited to participate until the estimated sample size is completed. 2. During the hospitalization the patients can be detected and invited to participate into the study. In both, the informed consent will be a prior and required procedure.
The study will assess the outcomes in the same group of patients before and after intervention. The intervention follows the next steps:
1. Case detection: Identifying if the patient suffers from multiple diseases, if there are readmissions and high demand for health services during hospital stay or according to the reports of the health promoting entity (EPS in Spanish).
2. Case screening: assessing complexity INTERMED scale (European Consultation-Liaison Workgroup for General Hospital Psychiatry and Psychosomatic), medium-high or high).
3. To establish the baseline measurement of indicators to comparate during post-intervention.
4. Comprehensive and multidimensional assessment of the patient: identifying care needs.
5. Assessment of caregiver burden: identifying fatigue and general health status.
6. Personalized care plan: a comprehensive and individual plan is made for each case according to the needs detected.
7. Coordination between providers: contacts and procedures to secure referral to service providers as required by the patient and the family caregiver.

Project description:Sepsis is a syndromic response to infection and is frequently a final common pathway to death from many infectious diseases worldwide. The global burden of sepsis is difficult to ascertain, although a recent scientific publication estimated that there were 48.9 million cases and 11 million sepsis-related deaths worldwide, which accounted for almost 20% of all global deaths. Here we analyzed the whole blood transcriptome of 5 healthy controls and 5 patients with confirmed sepsis from the Department of Emergency, Division of Surgical critical care, Tongji Trauma Center, Tongji Hospital, Tongji Medical College. This research aims to understand the expression of immune cells in sepsis patients and to construct an immune cell landscape of sepsis.

Project description:Disease diagnoses of GCAT Cohort participants obtained from electronic health records (EHR), mainly including the time period from 2012 to 2017. Disease diagnoses are codified in ICD-9, and the position of diagnosis refers to primary/secondary diagnoses (up to 14 secondary diagnoses per visit). The date and origin of the visit are also specified (AP: primary care, UGR: emergency, AH: hospital care, SMA: outpatient medical service, SMH: hospital medical service).

Project description:This was a prospective observational cohort study involving a convenience sample of previously healthy children <2 years of age with acute RSV infection as well as healthy non-infected age matched controls. The study was conducted at Nationwide Children’s Hospital (NCH; Columbus, OH) during four respiratory seasons, from 2011 to 2015. Children were enrolled at the NCH urgent care clinics, the emergency department or in the inpatient ward or intensive care unit (PICU). For the inpatients enrolled at ward or PICU the median time from admission to sample collection was 21.3h (interquartile range [IQR] 16.7 -37.6h). The majority of children had a confirmatory RSV test (either rapid antigen testing or PCR based) at enrollment per standard of care. In addition, the presence of RSV was confirmed by quantitative real time (qRT)-PCR in all study subjects. Healthy age-matched controls were enrolled during well-child visits or minor elective surgical procedures not involving the respiratory tract as described [Mejias 2013]. Exclusion criteria were as follows: documented bacterial co-infections, premature birth (<36 weeks of gestation), chronic or congenital medical conditions, and immunodeficiency. For healthy controls, additional exclusion criteria included: presence of fever or symptoms of respiratory tract infection within two weeks of enrollment.

Project description:Epigenetic modifications must underlie lineage-specific differentiation since terminally differentiated cells express tissue-specific genes, but their DNA sequence is unchanged. Hematopoiesis provides a well-defined model of progressive differentiation in which to study the role of epigenetic modifications in cell fate decisions. Multi-potent progenitors (MPPs) can differentiate into all blood cell lineages, while downstream progenitors commit to either myeloerythroid or lymphoid lineages. While DNA methylation is critical for myeloid versus lymphoid differentiation, as demonstrated by the myeloerythroid bias in Dnmt1 hypomorphs {Broske, 2009 #6}, a comprehensive DNA methylation map of hematopoietic progenitors, or of any cell lineage, does not exist. Here we have generated a mouse DNA methylation map, examining 4.6 million CpG sites throughout the genome including all CpG islands and shores, examining MPPs, all lymphoid progenitors (ALPs), common myeloid progenitors (CMPs), granulocyte/macrophage progenitors (GMPs), and thymocyte progenitors (DN1, DN2, DN3). Interestingly, differentiation towards the myeloid lineage corresponds with a net decrease in DNA methylation, while lymphoid commitment involves a net increase in DNA methylation, but both show substantial dynamic changes consistent with epigenetic plasticity during development. By comparing lineage-specific DNA methylation to gene expression array data, we find many examples of genes and pathways not previously known to be involved in lymphoid/myeloid differentiation, such as Gcnt2, Arl4c, Gadd45α, and Jdp2. Several transcription factors, including Meis1 and Prdm16 were methylated and silenced during differentiation, suggesting a role in maintaining an undifferentiated state. Additionally, epigenetic modification of modifiers of the epigenome appears to be important in hematopoietic differentiation. Our results directly demonstrate that modulation of DNA methylation occurs during lineage-specific differentiation, often correlating with gene expression changes, and define a comprehensive map of the methylation and transcriptional changes that accompany myeloid versus lymphoid fate decisions. mRNA expression of 8 hematopoietic progenitor populations [MPPFL-(5), MPPFL+(3), CMP(3), GMP(3), CLP(3), DN1(3), DN2(3), DN3(3)] were compared

Receiving telemedicine and remote monitoring may help patients have better outcomes (such as fewer avoidable emergency room visits and hospitalizations, better quality of life, fewer symptoms, and fewer treatment delays) than patients who receive usual care.
