Project description:Research on the Transcendental Meditation (TM) technique suggests it is a powerful intervention for reducing the unwanted effects of stress. Investigations of the TM program as a therapeutic intervention include cardiovascular disease (CVD), post-traumatic stress disorder (PTSD), and anxiety disorders. Healthcare utilization studies show reductions in diseases after regular practice of the program. This study compared long-term practitioners of TM (38 ± 4 years) with non-practitioner controls. We used DNA microarray global transcriptomics technologies in Peripheral Mononuclear Blood Cells (PMBCs) to compare a non-practitioner control group (Group III) and a TM-practitioner group (Group IV). Demographically-matched groups of 6 each were studied. These results are expected to be relevant to stress and stress-related diseases such as CVD, diabetes, and PTSD.

Project description:Interventions: The aim is to determine the value of GP endorsement for faecal occult blood test based re-screening colorectal cancer. The interventions are conducted within the setting of a population screening program where invitees are recruited from general practice patient lists.
Intervention 1: Invitation letter to re-screen on central screening facility letterhead with statement from invitee’s general practice endorsing participation in screening, signed by screening coordinator.
Intervention 2: Invitation letter to rescreen on practice letterhead with statement from practice endorsing screening, signed by invitee’s general practitioner.
Comparison group: recruited from electoral roll receiving control invitation to rescreen.
Duration: Four annual rounds of screening offers.
All people received the same invitation type over the 4 rounds of screening invitations.
Primary outcome(s): Paricipation in screening defined as return of a screening kit 12 weeks from offer.[]

Project description:Background and study aims
We carried out a study on subjects with a positive fecal occult blood test performed in the local colorectal cancer screening programme. Our goal was to find the best way to invite these subjects to undergo a total colonoscopy (TC).
Who can participate?
3800 subjects aged 50-69 years old, who attended nine colorectal cancer screening programmes in Italy.
What does the study involve?
The screening programmes invited subjects to undergo a fecal occult blood test (FOBT) every two years. Subjects with a positive FOBT (FOBT+) were invited to undergo a TC. During the study, FOBT+ subjects were randomly allocated to be invited to a TC in different ways:
1. Both first invitation and recall by mail
2. First invitation by phone, recall to non-compliers by mail
3. First invitation by phone, recall by face-to-face counseling with the General Practitioner
4. First invitation by phone, recall by an appointment with a specialist screening practitioner (nurse, healthcare assistant)
At the end of the study, we compared the uptake of TC of the groups of subjects who had received the different types of invitation.

Project description:Interventions: Group 1: Focus of the letter/writing addressed to the 50 year old group (two intervention groups of the birth year 1962) is testing for occult blood in the stool, which is paid by the health insurance from that age (and up to 55 years) annually. Beside the information about this offer and the invitation to its perception, the letter/writing should contain an understandable and clear information booklet about the test and its application and significance. In one of the two intervention groups, a stool test kit for carrying out the tests for blood in the stool should be additionally attached (test cards for each two samples from three consecutive bowel movements). It should be checked whether this service offer further increases participation rate in Germany, as shown in other countries. People in the intervention group 2 (cover letter with the stool test kit) will be asked to apply the test cards and send them to the DKFZ. There, the tests will be analysed and the result will be reported directly by mail to the persons and, if desired, additionally to the family doctor of whom the address is reported on a request form. In case of a positive test result, a meeting is recommended with the family doctor to discuss the possibility of a colonoscopic investigation. The test results are pseudonymised forwarded to the study center in the Saarland, where they can be clearly assigned to the investigation group 2 based on the communication numbers (which are noted on the materials sent to them). To check the benefits of repeated letters/writing of the same birth cohort, people are contacted again on the following 51th birthday (regardless of participation in the previous year). - To what extend can the sending of a stool test kit together with the personal invitation letter further increase participation?
- To what extend is a colonoscopy performed in case of a positive test result?
- To what extend can the use of screening colonoscopy be increased through a personal invitation letter with an information booklet?
- What effects can be achieved in terms of participation at initial cover letter and follow-up letter in one year distance?

Project description:Interventions: 1. Intervention name
Shared model of cancer follow-up care between general practitioners and radiation oncologists, for patients with breast, prostate or colorectal cancer.
2. Why
The rising incidence of cancer and increasing number of cancer survivors has resulted in the need to find alternative models of care for cancer follow-up care. The acceptability for follow-up care in general practice is growing, and acceptance increases with shared-care models where oncologists are still overseeing care. However, a major barrier to this model is the effective two-way exchange of information in real-time between oncologists and general practitioners. Improved communication technology plays an important role in the acceptability and feasibility of cancer shared follow-up care.
3. What
The intervention is additional radiation oncology follow-up appointments for the patient with their general practitioner; whilst the radiation oncologist is informed in real-time via the hospital oncology information system (electronic record).
Digital health technology (PROsaiq) will be used to transfer the review information from the general practitioner directly into the hospital system. The radiation oncologist will oversee the care by accessing the review information directly, and identify if the patient needs any further medical intervention or support.
4. Who provided
The general practitioner will complete a two hour radiation oncology course online (eviQ). A 30-minute training session will be provided by a radiation oncologist directly to the general practitioner on how to access the clinical assessment link, and what to do during the follow-up review by a radiation oncologist (review sleep, weight, skin reactions, bladder/bowel, prostate blood leve

Project description:Screening for ColoRectal Cancer (CRC) is widely recommended because of compelling evidence that it reduces mortality from CRC and that it’s cost-effective. In France, there is a national CRC screening programme inviting people aged 50-74 years by mail to consult their General Practitioner (GP) for a free on charge Faecal Occult Blood test deliverance every two years. Participation is a key factor for the screening programs success. High levels of screening uptake and the continued participation to regular screening must be achieved and maintained in the targeted population if the enormous potential of CRC screening is to be realised. Screening rates of various international programs vary widely (20-52%) and the participation is insufficient in the most settings. Therefore, a better understanding of facilitating and hindering factors of patient participation can guide interventions to improve CRC screening rates. They include patients and physicians’ factors. One issue is about GPs practice. GP involvement has been shown to improve the participation in the CRC screening. This study aims to assess the effectiveness of an intervention targeting GPs to increase patient participation in the CRC screening. Volunteer GPs will be randomly assigned to either the intervention group or to usual care (control group) and they will be followed one year. The intervention consists in providing every four months to the experimental group a list of their patients who are eligible but did not proceed yet to the screening. In both groups, patient participation in the screening will be measured at the end of the study.

Project description:Background and study aims
An advance notification letter conveying information on colorectal cancer (CRC) risk and the intervention offered in the screening programme, may increase the likelihood of a positive response to the subsequent screening invitation, as the subject will have progressed in his degree of readiness for change. In addition, it has been shown that, in particular for less educated subjects, general practitioners (GP) advice may have a strong influence on the decision to be screened. The study aims to assess the impact on the response rate to the invitation for CRC screening, in the context of population based programs, of an advance notification letter mailed to subjects eligible for invitation. The additional cost per person examined and the additional workload for the subjects GPs will be assessed.
Who can participate?
All subjects (male and female) aged 50 to 69 targeted for CRC screening in 8 population based programmes in Italy.
What does the study involve?
During the enrolment period all subjects targeted for invitation in these screening programs were randomly allocated to three groups:
A: standard personal invitation letter, signed by the GP, to undergo CRC screening
B: advance notification letter, mailed one month before the standard invitation letter (same as in A), conveying information on CRC risk, on the screening program and on the expected benefits and potential harms of screening
C: as in B, with the addition, in the advance notification letter, of the offer of a personal encounter with the subjects GP, to discuss pros and cons of screening. The response rate across the different groups will be assessed at 6 months following the invitation letter.

Project description:Using a mouse model of paternal exposure to traumatic stress, we identify circulating factors involving peroxisome proliferator-activated receptor (PPAR) pathways in the effects of exposure to the germline. Chronic injection of serum from trauma-exposed males into controls recapitulates metabolic phenotypes in the offspring. Pharmacological PPAR activation in vivo reproduces metabolic dysfunctions in the offspring and grand-offspring of injected males, and affects the sperm transcriptome in fathers and sons. In germ-like cells in vitro, both serum and PPAR agonist induce PPAR activation. Together, these results highlight the role of circulating factors as potential communication vectors between the periphery and the germline.

Project description:Interventions: Two alternative advance notification strategies will be trialed. Intervention 1 will consist of a simple ‘advance notification of an invitation to participate in screening for Colorectal Carcinoma (CRC)’ letter incorporating messages introducing the concept of screening for CRC, increased risk for CRC and the value of screening to reduce risk. Intervention 2 (enhanced advance notification) will include those messages plus more complex messages to reduce known barriers (lack of knowledge about CRC and its prevention, aversion for faecal sampling, low perception of the value of Fecal Occult Blood Test (FOBT) screening). The advance notification letters will be followed in 2 weeks by a standard invitation-to-screen letter. A control group will receive the standard invitation only. Invitees have 12 weeks from date of invitation-to-screen in which to participate in screening.
Primary outcome(s): Completion of faecal immunochemical test expressed as the population participation rate.[Within 12 weeks of screening invitation.]
