Intravenous Lidocaine for Cystectomy Procedures

ABSTRACT: Patients scheduled for a cystectomy at Charleston Area Medical Center (CAMC) will be enrolled into a randomized, double blinded clinical trial to assess if postoperative pain and ileus are reduced by perioperative intravenous lidocaine infusion versus placebo. The hypothesis is that patients receiving the lidocaine infusion will experience better pain control and ileus resolution. It is believed that lidocaine patients will also have less opioid requirement, less mortality, a shorter length of stay, less nausea /vomiting, and fewer adverse events.

Morphine alleviates pain after heart cryonjury in zebrafish without impeding regeneration

Project description:Nociceptive response belongs to a basic animal behavior facilitating adaptability and survival upon external or internal stimuli. Fish, similarly to higher vertebrates, also possess nociceptive machinery. Current protocols involving procedures performed on adult zebrafish including heart cryoinjury do not, however, take into account the adverse effects including pain that may potentially arise from these methodologies. Here, we assess the effect of two analgesics, lidocaine and morphine, followed after the heart cryoinjury in zebrafish. Monitoring swimming behaviour together with histology and gene expression analysis at the single cell level using scRNA sequencing and RNAscope fluorescent in situ hybridization technology, we show morphine, but not lidocaine, significantly improves animal welfare 6 hours post-cryoinjury, without impairing heart regeneration process. Altogether, morphine should be considered as the analgesic of choice to reduce post-surgical pain in adult zebrafish.

Intra-op Lidocaine and Ketamine Effect on Postoperative Bowel Function

Project description:Bowel function after bowel surgery is delayed (postoperative ileus)by both opiates and the surgery itself. We hypothesized that decreasing opiate use by other analgesics will speed the return of bowel function after surgery. Lidocaine and Ketamine are drugs that appear to be synergistic and do not slow peristalsis. This study is a Randomised Controlled Trial of Lidocaine Infusion Plus Ketamine Injection versus Placebo to to determine whether they will decrease opiate use and then whether decreased opiate use will speed the return of bowel function.

Lidocaine and bupivacaine downregulate MYB protein and LncRNA DANCR through interaction with miR-187-5p in MCF7 cells

Project description:In order to identify microRNAs (miRNAs) that were involved in the effects of anesthetics on breast cancer cells, Illumina next generation miRNA sequencing was performed to search for any differentially expressed miRNAs in responding to lidocaine and bupivacaine treatments in MCF7 breast cancer cells. The filtering criteria for identifying differentially expressed miRNAs in the lidocaine and bupivacaine groups were fold change ≥ 2.0X and P-value ≤ 0.05. In total, there were 1,228 significantly upregulated miRNAs in lidocaine group and 133 miRNAs in bupivacaine group. On the contrary, there were 25 significantly downregulated miRNAs in lidocaine group and 36 miRNAs in bupivacaine group.

NK Activity Modulation Induced by Intravenous Lidocaine During Colorectal Laparoscopic Surgery

Project description:Surgical resection is the best treatment option for colorectal cancer. Despite this radical approach, recurrences within five years are still common. Several authors have proposed that the immunosuppressive state surrounding the perioperative period was a key element of cancer cells spread. A particular subtype of T lymphocytes, the Natural Killer cells (NKs), is the main actor of the innate immune system. Several factors of the perioperative period can reduce activity of NKs such as stress, pain, opioids and general anaesthetics. Lidocaine is a local anaesthetic that has been widely used intravenously for abdominal surgeries. Intravenous lidocaine has been shown to reduce pain scores, morphine consumption, ileus time and length of stay in major colorectal surgeries. It reduced markers of systemic inflammation as well. The authors hypothesize that the use of intravenous lidocaine during laparoscopic surgeries for colorectal cancer resection will preserve NKs activity.

Effects of lidocaine on the expression of rat spinal cord

Project description:This study aimed to explore the possible mechanism of lidocaine-induced neurotoxicity. A total of 40 rats were randomly assigned to 4 groups (n = 10 per group): control group; sham group, received only intrathecal catheter insertion; saline group, received an intrathecal injection of saline; and lidocaine group, received an intrathecal injection of 10% lidocaine. Neurobehavioral tests were performed on rats in each group before injection and 1, 2, 3, and 4 days after the intrathecal injection. On the fourth day, after completing neurobehavioral tests, the spinal cords of all rats from L2 to L6 were removed and immediately stored in liquid nitrogen. In the control and lidocaine groups, parts of the spinal cord tissues of three rats were selected for gene microarray analysis.Based on the results of the gene chip analysis,Five rats in each group underwent real-time quantitative polymerase chain reaction (PCR) and five Western blot analysis to verify Differentially expressed genes at the mRNA transcription and protein levels. The aim of gene chip analysis is to screen potential genes involved in lidocaine-induced neurotoxicity.

RNA Expression Profiling of spinal cord tissues in a Rat Model of Neuropathic Pain

Project description:Neuropathic pain (NP), defined as a complex chronic pain state caused by somatosensory nervous system lesions or diseases, is currently the most challenging clinical neurological disorder in the world. The main signs and symptoms of NP include spontaneous pain, abnormal pain and hyperalgesia. It is generally believed that the specific pathological causes of NP include spinal cord or peripheral spinal cord injury, postherpetic neuralgia, trigeminal neuralgia, and tumor invasion. While the critical role of mRNA in NP is well recognized, the involvement of mRNA in the development of NP remains to be elucidated. Therefore, in this study, we explored changes in mRNA expression profiles through transcriptome sequencing to provide a landscape of mRNA dysregulation in the NP spinal cord.

Analgesic Efficacy of Free-opioid Anesthesia for Colorectal Surgery

Project description:Objectives: To compare free-opioid anesthesia (the combination of epidural anesthesia, intravenous lidocaine, ketamine, propofol, and sevoflurane) and opioid anesthesia (fentanyl, propofol and sevoflurane) regarding intraoperative analgesic efficacy in colectomies and rectal resections at Viet Tiep Friendship Hospital. Methods: A prospective, randomized controlled clinical trial was performed on 98 patients who were anesthetized for colorectal surgery from December 2019 to November 2021. Patients were randomized into 2 groups: Group OA - Opioid anesthesia (n = 49): Intraoperative pain control by fentanyl; FOA group - Free-opioid anesthesia (n = 49): Intraoperative pain control by continuous infusion of lidocaine, bolus doses of ketamine combined with epidural levobupivacaine.
