ERAS (Enhanced Recovery After Surgery) Protocol Implementation in Piedmont Region for Colorectal Cancer Surgery

ABSTRACT: The study assesses the impact on quality of care of implementing the ERAS (Enhanced Recovery After Surgery) protocol for colorectal cancer surgery in the network of public hospitals in the Regione Piemonte (North-West Italy). Every hospital is a cluster entering the study treating patients according to its current clinical practice. On the basis of a randomized order, each hospital switches from current clinical practice to the adoption of the ERAS protocol.

Early Discharge After Robotic Colorectal Surgery (eRCS): a Prospective Observational Study

Project description:The enhanced recovery after surgery (ERAS) scheme has improved recovery of patients by addressing the surgical stress and enabling a proactive perioperative care which has proven to decrease postoperative complications and reduce hospital stay. ERAS had a major beneficial effect in open colorectal surgery, while in laparoscopic surgery the additional benefit was somewhat less. Also, current literature is not clear if ERAS can improve operative outcomes of robotic colorectal surgery. Starting from the already proven ERAS protocol, we aim to build a selective enhanced recovery scheme and a standardized early follow-up pathway enabling early safe discharge of low-risk patients after robotic colorectal surgery. This observational study will also design and assess the feasibility and clinical outcomes of an Early discharge Colorectal Surgery (eRCS) protocol which may be used to discharge patients in postoperative day 1 after robotic colorectal resections through close virtual and outpatient follow-up.

Gene expression profiling of prostate cancer (PCa) and normal prostatic samples

Project description:Prostate cancer tissue samples containing at least 70% of neoplastic cells were obtained from patients (age range: 50–75 years) who had undergone radical prostatectomy at the Hospital of Biella (Italy) and prostate biopsies were obtained from healthy patients of the same age range, without any diagnosis of prostate tumor or BPH. Gene expression profiling was performed using Human Gene Expression 8 × 60K Microarray, Agilent Technologies.

Molecular profiling of innate immune response mechanisms in ventilator associated pneumonia (VAP)

Project description:Ventilator associated pneumonia (VAP) is the 2nd most common hospital acquired infection associated with high morbidity, mortality and increased hospitalization. Current practice of diagnosis is based on clinical symptoms and bronchoalvolar lavage (BAL) culture. The procedures of BAL collections are invasive whereas, endotracheal aspirate (ETA), a matrix of upper airway collection is minimally invasive and underexplored in VAP diagnosis. The study describes first in detail characterization of proteome of longitudinal ETA collections from 16 intubated patients including 11 VAP patients and explores potential utility of ETA in VAP diagnosis.

A specific missense mutation in GTF2I occurs at high frequency in thymic epithelial tumors

Project description:Next generation sequencing of 28 thymic epithelial tumors (TETs) revealed a high frequency of GTF2I missense mutation (chr7:74146970T/A) in A thymomas, a relatively indolent subtype. The GTF2I mutation was confirmed in 82% of A and 74% of AB thymomas in a series of 274 TETs but was rare in aggressive subtypes, where recurrent mutations of known cancer genes were identified. Therefore, GTF2I mutation correlated with a better survival. GTF2I Beta and Delta isoforms were expressed in TETs and both mutant isoforms were able to stimulate cell proliferation in vitro. Thymic carcinomas presented a higher number of mutations than thymomas (average 43.5 and 18.4, respectively). Recurrent mutations of known cancer genes, including TP53, CYLD, CDKN2A, BAP1 and PBRM1 were identified in thymic carcinomas. These findings will complement the diagnostic work up of these rare tumors, and also help the development of a molecular classification, and assessment of prognosis and treatment strategies. Tumor samples of 286 patients were collected from 4 different institutions: National Cancer Institute (Bethesda MD), Pisa University Hospital (Pisa, Italy), Padua University Hospital (Padua, Italy) and IRCCS Istituto Clinico Humanitas (Rozzano, Italy).

Transcription profiling of human peripheral blood mononuclear cells from radiation-exposed workers versus control subjects

Project description:We used peripheral blood mononuclear cells to study the gene expression profiles of radiation-exposed workers versus control subjects. Human PBMC were isolated from 56 healthy donors who were working in a local Hospital (Bologna, Italy): 28 individuals were x and g radiation-exposed workers, recruited from the Nuclear Medicine and Cardiovascular Units, while the control group consisted of 28 unexposed subjects, working at the same health care facility, matched for sex, age and smoking habits. The mean dosimeter reading, expressed as accumulated doses, during the entire time of employment (range: 1-32 years), was 19.49±37.59 mSv (mean ± SD). All procedures related to the collection of blood samples were performed with an informed consent and according to the protocol approved by the Institutional Review Board.

Effects of Implementing an ERAS Protocol

Project description:Introduction: Successful colorectal surgery is determined based on postoperative mortality and morbidity rates, complication rates, and cost-effectiveness. One of the methods to obtain an excellent postoperative outcome is the Enhanced Recovery After Surgery (ERAS) protocol. This study aims to see the effects of implementing an ERAS protocol in colorectal surgery patients. Methods: Eighty-four patients who underwent elective colorectal surgery at National Tertiary-level Hospital were included between January 2021 and July 2022. Patients were then placed into ERAS and control groups according to the criteria. The Patients in the ERAS group underwent a customized 18-component ERAS protocol and were assessed for adherence. Postoperatively, both groups were monitored for up to 30 days and assessed for complications and readmission. The investigators then analyzed the length of stay and total patient costs in both groups.
