Project description:Extracellular vesicles (EVs) are valuable sources for the discovery of useful cancer biomarkers. This study explores the potential usefulness of tumor cell-derived EV membrane proteins as plasma biomarkers for early detection of colorectal cancer (CRC). EVs were isolated from the culture supernatants of four CRC cell lines by ultracentrifugation, and their protein profiles were analyzed by LC-MS/MS. Bioinformatics analysis of identified proteins revealed 518 EV membrane proteins in common among at least three CRC cell lines. We next used accurate in-clusion mass screening (AIMS) in parallel with iTRAQ-based quantitative proteomic analysis to highlight candidate proteins and validated their presence in pooled plasma-generated EVs from 30 healthy controls and 30 CRC patients. From these, we chose 14 potential EV-derived targets for further quantification by targeted MS assay in a separate individual cohort comprising of 73 CRC and 80 healthy subjects. Quantitative analyses revealed significant increases in ADAM10, CD59 and TSPAN9 levels (2.19- to 5.26-fold, p <0.0001) in plasma EVs from CRC patients, with AUC values of 0.83, 0.95 and 0.87, respectively. Higher EV CD59 levels were significantly corre-lated with distant metastasis (p = 0.0475), and higher EV TSPAN9 levels were significantly asso-ciated with lymph node metastasis (p = 0.0011), distant metastasis at diagnosis (p = 0.0104) and higher TNM stage (p = 0.0065). A two-marker panel consisting of CD59 and TSPAN9 outper-formed the conventional marker CEA in discriminating CRC and stage I/II CRC patients from healthy controls, with AUC values of 0.98 and 0.99, respectively. Our results identify EV mem-brane proteins in common among CRC cell lines and altered plasma EV protein profiles in CRC patients, and suggest plasma EV CD59 and TSPAN9 as a novel biomarker panel for detecting early-stage CRC.

Project description:Abstract: Altered expression of microRNAs (miRNAs) is associated with development and progression of various human cancers by regulating the translation of oncogenes and tumor suppressor genes. In colorectal cancer (CRC), these regulators complement the Vogelstein multi-step model of pathogenesis and have the potential of becoming a novel class of tumor biomarkers and therapeutic targets. Using QRT-PCR, we measured the expression of 621 mature miRNAs in 40 CRCs and their paired normal tissues and identified 23 significantly deregulated miRNAs. We subsequently evaluated their association with clinical characteristics of the samples and presence of alterations in the molecular markers of CRC progression. Expression levels of miR-31 were correlated with CA19-9 and miR-18a, miR-21 and miR-31 were associated with mutations in APC gene. To investigate the downstream regulation of the differentially expressed miRNAs identified we integrated putative mRNA target predictions with the results of a meta-analysis of seven public gene expression datasets of normal and tumor samples of CRC patients. Many of the CRC deregulated miRNAs computationally map to targets involved in pathways related to progression. Here one promising candidate pair (miR-1 and MET) was studied and functionally validated. We show that miR-1 can have a tumor suppressor function in CRC by directly down-regulating MET oncogene both at RNA and protein level and that re-expression of miR-1 leads to MET driven reduction of cell proliferation and motility, identifying the miR-1 down-modulation as one of the events that could enhance CRC progression. Project description:CA19-9 is a glycan elevated in human gastrointestinal pathologies, but absent from rodents. We humanized the mouse glycome and found that CA19-9 is not only a correlative biomarker of pancreatic disease, but also plays a causative role in the initiation of pancreatitis. Elevation of CA19-9 results in the activation of EGFR signaling in the normal pancreatic ductal compartment through secretion of Fibulin 3. These signaling changes can be blocked by blocking either CA19-9 or Fibulin 3.

Project description:This study enrolled patients who underwent R0 resection of tumor and had elevated tumor biomarkers (CEA, CA19-9). After enroll the study, a CTC test will performed and patients who had positive CTC will be randomly assigned to two groups. The control group will continue follow-up until radiological recurrence appear, the treatment group will start treatment or change the current adjuvant regimen. First endpoint is OS. The secondary endpoint is DFS, adverse event.

Site-specific glycoforms of plasma CEA differed in patients with different cancers and the progression of CRC.

Project description:Colorectal cancer is the 3rd commonly diagnosed cancer (10.2%) and 2nd cause of cancer death (9.2%) in all cancers.There will be estimated 1.8 million new colorectal cancer cases and 0.9 million colorectal cancer deaths in 20181. As the 5th incidence rates and mortality rates cancer in china, colorectal cancer incidence rates has increased from 1990 to 20112,3. CRC is concidered as a muti-step progress diseases4, patients with precursors lesions and early stage CRC can get great benefit from surgical treatment, colonoscope can detect visiable early dieases but not suitable for large-scale screening5, because of the poor sensitivity or specificity, blood carcinoembryonic antigen(CEA)6,7 and fecal blood testing(FOBT)8 provide limited help. There is no standard biomarker predicting CRC lymph node and liver metastasis4, although It’s very valuable for patients and docter before and after surgery9. Liver is the main site for colorectal cancer metastasis10, hepatic resection is the only treatment and successful for CRC-LM11, Unfortunately, over 50% of patients who get hepatic resection develop recurrences11, since then it’s hlepful to find biomarkers for monitoring CRC metastasis, blood CEA has been accepted as indicator of CRC progress12, in current study, we aim to explore a more convient and presion way for monitoring CRC metastasis.
