A Prospective Observational Cohort Study Evaluating Resection Rate in Patients With Metastatic Colorectal Cancer Treated With Aflibercept in Combination With FOLFIRI - Observatoire résection

ABSTRACT: Primary Objective: -Evaluate the resection rate (R0/R1) in current practice for aflibercept in combination with irinotecan plus 5-fluorouracil (FOLFIRI) in patients treated for metastatic colorectal cancer (mCRC) after failure of an oxaliplatin-containing regimen Secondary Objectives: * Evaluate overall survival, progression-free survival and relapse-free survival. * Evaluate conversion rates (initially unresectable disease which became resectable after chemotherapy, with R0/R1 resection). * Evaluate histological and radiological response rates. * Evaluate rate of postoperative complications. * Evaluate safety.

Chemotherapies Associated With Targeted Therapies on the Resection Rate of Hepatic Metastases

Project description:The main objective is to compare resection rates (R0 or R1) for hepatic metastases in the experimental arm (tri chemotherapy plus targeted therapy) versus the control arm (bi chemotherapy plus targeted therapy); in both arms the targeted therapy is selected according to K-Ras status of the patient’s tumor. The secondary objectives are to evaluate the objective response rate (CR and PR) after 4 cycles of treatment, according the RECIST V1.1 evaluation scale. * the rate of complete remission (CR) at 6 months after the last study treatment (hepatic surgery or last chemotherapy cycle). * the specific rates of resection R0, R1, R2. * the complete pathological response Rate, * the relapse-free survival rate in (R0 or R1) resected patients, * the response duration in non-resected patients, * the toxicity according to CTC AE V4 scale except for the neurotoxicity that will be evaluated with the Levi scale, * the post operative complications using the DINDO classification, * the progression-free survival (PFS) and overall survival (OS). The objectives of the biological study are: * to evaluate tumor-related predictive factors such as somatic mutations (KRAS, BRAF, TP53) and genetic amplification related factors (EGFR), * to evaluate patient-related predictive factors in connection with genetic polymorphisms (Fc gamma and VEGF receptors), * to evaluate ADCC activity via immunohistochemistry in order to analyze the lympho free and progression-free survival, * to study circulating of tumor cells as prognostic factor for metastatic colorectal cancer, non- resectable at presentation.

Proteomic Alterations Associated with Residual Disease in Neoadjuvant Chemotherapy Treated Ovarian Cancer Tissues

Project description:Optimal cytoreduction to no residual disease (R0) correlates with improved disease outcome in the management of high-grade serous ovarian cancer (HGSOC) patients. Neoadjuvant chemotherapy (NACT) followed by interval debulking surgery (IDS) offers an alternate approach to management of HGSOC patients to achieve complete resection. This study assessed proteomic alterations in matched, chemotherapy naïve and NACT-treated patients tumors obtained from HGSOC patients with suboptimal (R1) versus optimal (R0) debulking at IDS. We describe distinct proteome profiles in pre- and post-NACT HGSOC tumors correlating with residual disease status providing prognostic biomarkers for residual disease at IDS as well as candidate proteins associated with NACT resistance warranting further pre-clinical investigation.

Plasma small extracellular vesicles microRNAs profile in advanced ovarian cancer

Project description:No residual disease after debulking Surgery (R0 resection) is the most critical independent prognostic factor for advanced ovarian cancer (AOC). Therefore, it is of paramount importance to preoperative estimate the likelihood of R0 resection for choosing the best therapeutic strategy. Our study aimed to develop a non-invasive and reliable detection method for AOC patients with a high risk of residual disease. An integrated plasma small extracellular vesicles (sEVs) microRNA profiling was generated by RNA sequencing in AOC patients with no residual disease patients (R0) and residual disease(non-R0). We identified and validated a logistic model based on plasma sEVs miRNAs to predict residual disease in AOC patients.

Short Running Head Obesity in Rectal Cancer Patients

Project description:There is a controversy regarding the effect of the two Body Mass Index (BMI) extremes on the oncological outcome of rectal cancer. The obesity paradox appears to exist in rectal cancer patients treated with nCRT and surgery, as it was associated with significantly higher rates of pathological complete response and R0 resection. Underweight patients were at higher risk for anastomotic leak and R1 resection.

Whole exome sequencing identifies clinically relevant mutational signatures in resected hepatocellular carcinoma

Project description:We conducted a multicentre pilot study on patients with localized HCC following complete radiological response to surgical resection. Patients received 250 mg oral gefitinib once daily as adjuvant therapy, up to a maximum of 6 months. The clinical endpoints included recurrence-free survival (RFS) and overall survival (OS), and correlative genetic analyses were conducted from whole exome sequencing data (n=33). All statistical analyses were carried out on an intention-to-treat basis. The study is registered with ClinicalTrials.gov (NCT00282100).
