Effect of gabapentin capsules on nausea and vomiting

ABSTRACT: Intervention 1: In the intervention group: gabapentin capsule of 300 milligrams three times a day from one day Chemotherapy will be used until the end of the chemotherapy period. Intervention 2: In the control group, for the reduction of nausea and vomiting after chemotherapy, a routine of three drugs (dexamethasone, ranitidine and ketril) will be used.;Treatment - Drugs;Treatment - Drugs;In the intervention group: gabapentin capsule of 300 milligrams three times a day from one day Chemotherapy will be used until the end of the chemotherapy period.;In the control group, for the reduction of nausea and vomiting after chemotherapy, a routine of three drugs (dexamethasone, ranitidine and ketril) will be used Primary outcome(s): Nausea and vomiting. Aprepitant Without Steroid in Preventing Chemotherapy-induced Nausea and Vomiting in Patients With Colorectal Cancer

Project description:Addition of aprepitant, an NK1 receptor antagonist to a 5-HT3 receptor antagonist and dexamethasone regimen was shown to be effective for prevention of chemotherapy-induced nausea and vomiting (CINV) with moderately emetogenic chemotherapy (MEC). Little is known about the efficacy of aprepitant when used without dexamethasone. Dexamethasone is widely used to prevent both acute and delayed nausea and vomiting induced by chemotherapy. However, multi-period use of dexamethasone could be associated with side effect, such as hyperglycemia, dyspepsia and insomnia. This randomized phase III trial studies antiemetic therapy with aprepitant and tropisetron to see how well they work compared to dexamethasone plus tropisetron in preventing chemotherapy-induced nausea and vomiting in patients with colorectal cancer receiving FOLFOX(oxaliplatin, leuvovorin and 5-fluorouracil) chemotherapy.

Project description:RATIONALE: Aprepitant, palonosetron, and dexamethasone may help lessen or prevent nausea and vomiting in patients receiving chemotherapy. PURPOSE: This phase II trial is studying how well giving aprepitant together with palonosetron and dexamethasone works in preventing nausea and vomiting caused by chemotherapy in patients receiving chemotherapy for metastatic colorectal cancer.

Project description:Medical cannabis has been increasingly prescribed for a range of conditions including epilepsy, chronic neuropathic pain, and chemotherapy-induced nausea and vomiting. The benefits and possible adverse events of medical-grade cannabis products vary between patients, suggesting that genetics may play a role in the pharmacokinetics of the cannabinoids, yet regulatory restrictions have led to limited clinical studies. This study is aimed at identifying a genetic signature that is predictive of the pharmacokinetics of tetrahydrocannabinol (THC), the principal intoxicating chemical compound derived from cannabis. We have identified 55 variants among 38 genes that were overrepresented in either the Low-THC or High-THC groups.

RNA-seq profiles between human parental and 5-FU drug resistant HCT116 and SW480 colorectal cancer cell lines

Project description:Colorectal cancer (CRC) is one of the most frequently diagnosed and lethal malignancy. Several key factors including poor dietary habits, smoking, alcohol consumption, genetic predisposition, obesity, diabetes mellitus, and sedentary lifestyle – all result in a significantly increased risk for developing CRC. Current treatment modalities for patients with CRC include surgery, which is often followed with adjuvant chemotherapy, especially in patients with a stage II and III disease. Most patients with advanced CRC are generally treated with the chemotherapeutic drug, 5-fluorouracil (5-FU) – either given alone or in combination with oxaliplatin and other molecularly-targeted drugs. While such treatment regimens are effective at improving disease outcomes, their clinical usefulness is often hampered due to the considerable toxicity associated with these treatments, which often causes severe nausea, vomiting, weight loss, and risk of infectious complications due to immunosuppression. Furthermore, their therapeutic efficacy is limited due to the emergence of chemotherapeutic drug resistance. In this study we performed genomewide transcriptomic analysis of parental and 5-FU resistant cells to identify differentially expressed genes and its associated pathways in promoting chemoresistance in CRC.

Effects of dexamethasone on human skeletal muscle and adipose tissue

Project description:Context: Glucocorticoids are frequently prescribed drugs with important side-effects such as glucose intolerance and tissue remodelling. Objective: The goal was to explore the molecular basis of the crosstalk between adipose tissue and skeletal muscle during short term glucocorticoid treatment. Design and Intervention: Healthy male subjects were assigned to a 4-d treatment with dexamethasone at 4mg/d. Main Outcome Measures: Primary outcome measures were gene expression profiling of adipose tissue and skeletal muscle. Urinary cortisol and metabolic biochemistry were also assessed. Dexamethasone was provided in 0.5 mg tablets as 4mg per day during 4 days to male healthy volunteers. Skeletal muscle and adipose tissue biopsies were obtained at the beginning and at the end of the protocol. The transcriptome analysis compared before vs. after the 4 days dexamethasone treatment using a dye swap design and whole genome 4x44k oligonucleotide arrays (Agilent Technologies).

Not surprisingly, learning plays a critical role in feeding behavior through the establishment and strengthening of food preferences and aversions. That is, the integration of taste and post ingestive visceral signals in the brain results in memorial representations about the consequences associated with ingesting a particular food. For example, when ingestion of a food is followed by negative gastrointestinal consequences (e.g. nausea, sickness, or vomiting), the animal develops a conditioned taste aversion (CTA), which produces a switch from acceptance to avoidance of that and any like tasting stimulus. Despite recent advances in understanding CTA responsive intracellular signaling pathways in the amygdala, little is known about any long-term regulation of target gene expression following CTA memory consolidation and retrieval. The present study utilized oligo-nucleotide microarray to understand the genes and networks involved in Conditional Taste Aversion Behavior.
