Project description:<p>Thoroughbred (TB) racehorses are subjected to rigorous conditioning programs involving exercise sessions of various types, distances and intensities. These programs aim to optimize both their physical and mental capabilities, which are essential to achieve maximal performance. Conventional investigations of exercise-induced metabolic responses typically focus on a limited set of targeted hematological and biochemical parameters. This field study was conducted to comprehensively assess the metabolic responses triggered by exercise sessions routinely used in TB conditioning. To accomplish this, we employed an untargeted metabolomics approach. Blood samples were collected from 10 racehorses at rest and after completing a routine exercise session. The population was subsequently divided into two groups based on the intensity of the exercise performed: a high-intensity group (n = 6, gallop at ± 13.38 m/s, 1400 m) and a moderate-intensity group (n = 4, soft canter at ± 7.63 m/s, 2500 m). The exercise intensity was evaluated through monitoring of speed, heart rate, and targeted hematological and biochemical parameters (e.g., lactatemia, hematocrit, etc). Resting and 30 min post-exercise plasma samples were analyzed using an untargeted metabolomics approach based on ultra-performance liquid chromatography coupled with high-resolution mass spectrometry. Unsupervised principal component analysis revealed changes in the metabolome from pre- to post-exercise samples with greater alterations following high-intensity exercise. 54 metabolites, mainly related to amino acid, fatty acid, nucleic acid and vitamin metabolism, were altered following high-intensity exercise versus 23 metabolites, mainly related to fatty acid and amino acid metabolism, following moderate-intensity exercise. Untargeted metabolomics complemented the routine biochemical analyses, allowing a more comprehensive exploration of the exercise-induced response to exercises used in routine conditioning. As previously reported by traditional biochemistry, exercise-induced changes in energy metabolism in TB racehorses have been confirmed. Through metabolomics, the involvement of lipid and amino acid metabolism during routine exercise and recovery has been highlighted, aspects that had previously been largely overlooked in Thoroughbred racehorses.</p>

Project description:This study aimed to characterize the effects of laparotomy on post-operative physical function and skeletal muscle gene expression in C57BL/6N mice at 3, 20 and 24 months of age to investigate late-life vulnerability and resiliency to acute surgical stress. Pre- and post-operative physical functioning were assessed by forelimb grip strength and motor coordination on the accelerating rotarod. RNA sequencing of soleus muscles collected on post-operative day 5 (POD 5) showed that laparotomy induced age-associated differential gene expression and canonical pathway activation with the greatest effects in the youngest mice. The 24-month-old mice were stratified into low functioning (LF) and high functioning (HF) groups based on their baseline motor performance on the accelerating rotarod. RNA-sequencing showed 224 and 228 differentially expressed genes (DEGs) for HF and LF, respectively, compared to their respective controls. Only 21 DEGs were observed in both groups. We conclude that pre-operative performance on the accelerating rotarod correlates with differences in skeletal muscle gene expression, which may contribute to the differences in functional outcomes post-operatively in HF and LF mice. These findings demonstrate a mouse model of laparotomy which recapitulated some features of post-operative skeletal muscle decline in older adults following surgery, and identified age-associated, laparotomy-induced molecular signatures in skeletal muscles.

Project description:Interventions: The general rehabilitation program will consist of a multidisciplinary education and exercise program, which will be provided as group sessions twice weekly for 8 weeks. The education component will include 1-hour nutritional advice and management provided by a dietitian; 1-hour exercise education regarding the importance of exercise to well-being, bladder and bowel advice, and adherence to exercise provided by a physiotherapist; 1-hour fatigue management and return to work strategies provided by an occupational therapist; and 5-hours health behavior change and self-management strategies provided by a psychologist. The exercise component will consist of a moderate intensity (Borg Rating of Perceived Exertion (RPE) Scale 12-14), physiotherapist-supervised, combined aerobic and resistance training program. The exercise program will be individualized based on the results from baseline outcome measures. Participants will be required to attend an one hour education session per week and an one hour exercise session per week over the 8-week intervention period. The home-based walking program requires participants to perform daily walking with duration and intensity individually prescribed based on the results of baseline walking tests. To encourage adherence to the home-based walking program, 6 telephone motivational coaching sessions (1 to 2 calls per month) will be provided from a physiotherapist, between data collection time-points 2 (immediately post-intervention) and 3 (6 months following baseline) . Daily feedback for participants regarding distance and number of steps taken will be encouraged with a simple pedometer.
Primary outcome(s): Functional capacity as assessed using the six-minute walk test [At baseline, immediately post-intervention, and at 6 months following baseline];Feasibility as assessed by the recruitment rate, adherence, withdrawals, and adverse events. There are minimal risks associated with the rehabilitation and measurement tests. Participants may feel some general muscle discomfort after exercise, which is a normal response to exercise.[At baseline, immediately post-intervention, and at 6, 12, 24, and 60 months following baseline]

Project description:Interventions: Pre-operative exercise programme:
The exercise programme will consist of 60 minutes individualised (one-to-one) training session with a registered physiotherapist, once a week, for 2 to 6 weeks (maximal of 6 sessions) before the patient undergoes their scheduled surgery. The last day of exercise will be the day before the patient undergoes surgery. The duration (2-6 weeks), specific exercises performed and intensity of the exercise programme will all be tailored to each patient through a health assessment, taking into consideration their current health status, physical activity level, co-morbidities and medical history. Each session will consist of 10 minutes of warm-up (walking and cycling), 40 minutes of aerobic and endurance exercises (such as cycling, treadmill and rower activity), respiratory and muscle strength exercises (breathing and weights training), and 10 minutes cool down (stretches and flexibility exercises). The aerobic and endurance exercises will be performed at 40% to 60% of the maximum heart rate and the strength training will be performed at an intensity of 40% to 60% of the one repetition maximum. Participant will be given instruction and recommendations on how to complete and progress these exercises (or similar exercises) at home (4 sessions x 60 minutes at home per week, up to the scheduled date of surgery). Furthermore, apart from the prescribed exercises, participants will be recommended to engage in moderate-intensity exercise for at least 150 minutes per week (such as walking or cycling). Home exercises will be captured in a daily exercise diary that participants will complete until the end of the exercise programme.
Primary outcome(s): The feasibility of incorporating a standardised, intensive exercise programme into the pre-operative period for patients undergoing pelvic exenteration at Royal Prince Alfred Hospital.
This will be determined by the number of eligible patients recruited, retention and adherence rates to the exercise programme. The retention rate will be defined as the percentage of individuals who completed the intervention. Adherence will be defined as the percentage of exercise sessions attended by those who were randomised to the intervention group. Adherence to the exercise programme will be recorded using attendance records (recorded by the study physiotherapist) / participant exercise diaries (recorded by the participant).[Feasibility will be assessed during the week before surgery (1 week pre-operative), ];The acceptability of incorporating a standardised, intensive exercise programme into the pre-operative period for patients undergoing pelvic exenteration at Royal Prince Alfred Hospital. This will be assessed by a semi structured questionnaire pre-discharge.[Acceptability will be assessed shortly before discharge after surgery (pre-hospital discharge).];The acceptability to patients of being randomised to the exercise program or usual care (satisfaction with study intervention). Project description:The aim of this investigation was to evaluate the effect of training on the global transcriptional response of skeletal muscle to an acute bout of resistance exercise. Seven young healthy men and women underwent a 12-week supervised progressive unilateral arm resistance exercise (RE) training program. One week after the last session of training, subjects performed an acute bout of bilateral arm RE in which the trained and the untrained arm exercised at the same relative intensity. A muscle biopsy was obtained 4h post exercise from the biceps brachii of the trained and untrained arm. Trained and untrained muscle samples were analyzed for mRNA levels of over 20,000 annotated genes using Affymetrix U133 Plus 2.0 microarrays.
