In absense of confirmation of death survival time will be censored at the earlier of the last date the patient is known to be alive and the study cut-off date.

A placebo versus study medication trial study for verification of improving survival in metastatic colorectal cancer patients.

Project description:Interventions: Clinical safety/efficacy, randomized, double-blinded, phase 3 trial of placebo versus xilonix. It will be included 800 patients of both sexes aged 18 years or older. Patients will receive 1 dose every 2 weeks of placebo (1/3 of patients) or of 7,5 mg/kg of the human monoclonal antibody anti interleukin 1a (also named MABp1 or xilonix) that will be administered intravenously for as long as the patient is beneficiating or until he/she reaches any of the protocol discontinuation criteria, i.e: pregnancy, consent withdrawal, disease progression, adverse event that justifies the patient withdrawal or study termination by the sponsor.;Drug;Biological/vaccine Primary outcome(s): Overall survival will be the primary endpoint of this study survival time will be defined as the duration from the date of randomization until death or censor date. Long noncoding RNA (lncRNA) expression data from human myelodysplastic syndromes (MDS) bone marrow mononuclear cells

Project description:lncRNAs not only participate in normal hematopoiesis but also contribute to the pathogenesis of acute leukemia. However, their clinical and prognostic relevance in MDS remains unclear to date. In this study, we profiled the lncRNA expressions in 176 adult patients with primary MDS with Affymetrix GeneChip Human Transcriptome Array 2.0, and identified the lncRNAs whose expression levels were significantly associated with overall survival (OS). We then constructed a risk scoring system with the weighted sum of these lncRNAs. Patients with higher lncRNA scores had a significantly shorter OS and a higher five-year leukemic transformation rate compared with those with lower scores.

Trial Comparing Two Two Sequences of Therapy in Colorectal Metastatic Patients

Project description:Primary Objectives: Aim of this study is to compare the efficacy and safety of two different sequences of chemotherapeutic agents in order to optimize the treatment of patients with metastatic colorectal cancer progressed to a first line chemotherapy with FOLFIRI and bevacizumab. Primary endpoint will be overall survival, defined as the time elapsed from the date of randomization to the date of patient death due to any cause, or the last date the patient was known to be alive. Secondary Objectives Progression free survival, Quality of life, Health resource utilisation and economic evaluation, Toxicity and incidence of adverse events The study regimen includes: Strategy A: FOLFOX-4 followed, after progression, by irinotecan/cetuximab Strategy B: irinotecan/cetuximab followed, after progression, by FOLFOX-4 Patients will be randomly assigned to one of the two treatment sequences (with 1:1 ratio) using a block design randomization procedure stratified according to center. The patient accrual period is planned for approximately 36 months. To assess OS, all pts will be followed for up to 18 months after the last patient is randomised. The maximum estimated study duration is approximately 54 months.All statistical analyses will be based on an intention-to-treat approach. CONSORT rules will be applied to describe study flow and protocol deviations.

Predictive biomarkers for survival benefit with ramucirumab in urothelial cancer in the RANGE trial

Project description:The RANGE study (NCT02426125) evaluated ramucirumab (an anti-VEGFR2 monoclonal antibody) in patients with platinum-refractory advanced urothelial carcinoma (UC). Here, we use programmed cell death-ligand 1 (PD-L1) immunohistochemistry (IHC) and transcriptome analysis to evaluate the association of immune and angiogenesis pathways, and molecular subtypes, with overall survival (OS) in UC. Higher PD-L1 IHC and immune pathway scores, but not angiogenesis scores, are associated with greater ramucirumab OS benefit. Additionally, Basal subtypes, which have higher PD-L1 IHC and immune/angiogenesis pathway scores, show greater ramucirumab OS benefit compared to Luminal subtypes, which have relatively lower scores. Multivariable analysis suggest patients from East Asia as having lower immune/angiogenesis signature scores, which correlates with decreased ramucirumab OS benefit. Our data highlight the utility of multiple biomarkers including PD-L1, molecular subtype, and immune phenotype in identifying patients with UC who might derive the greatest benefit from treatment with ramucirumab. Phase II Study of Immune Checkpoint Blockade with Pembrolizumab after High Dose Cytarabine in Relapsed/Refractory Acute Myeloid Leukemia

Project description:To address the role of immune dysregulation in AML, we conducted a phase II study of high-dose cytarabine followed by pembrolizumab 200 mg IV on day 14 to examine whether programmed death-1 inhibition improves clinical responses in relapsed/refractory (R/R) AML. Overall responders could receive pembrolizumab maintenance up to two years. Among thirty-seven patients enrolled, the overall response rate, composite complete remission rate (CRc; primary endpoint), and median overall survival (OS) were 46%, 38% and 11.1 months, respectively. Patients with refractory/early relapse and those receiving treatment as first salvage had encouraging outcomes (median OS 13.2 and 11.3 months, respectively). Grade >3 immune-related adverse events were rare (14%) and self-limiting. Patients who achieved CRc had a higher frequency of progenitor exhausted CD8+ T-cells in the bone marrow prior to treatment and a non-significant increase in baseline T-cell diversity.

DNA copy number gain at 1q23.3 is associated with poor survival in metastatic bladder cancer [Affymetrix]

Project description:Metastatic urothelial carcinoma (UC) of the bladder is associated with multiple somatic copy number alterations (SCNAs). We evaluated SCNAs to identify predictors of poor survival in patients with metastatic UC treated with platinum chemotherapy. We obtained overall survival (OS) and DNA copy number data from UC patients in a Spanish discovery cohort and three validation cohorts. GISTIC was used to identify altered regions, and associations between copy number gains/losses and OS were determined by a Cox proportional hazard model adjusting for performance status and visceral disease The output files of the GISTIC 2.0 software are linked as supplementary files on Series record along with readme.txt file describing the contents of each file/column.
