Effects of the menstrual cycle on bispectral index and anesthetic requirement in patients with preoperative intravenous dexmedetomidine following propofol induction.

Project description:Several studies have suggested that the menstrual cycle has the impact on sedation. No previous study has evaluated the effects of the menstrual cycle on sedation level and propofol requirement with preoperative intravenous dexmedetomidine. Sixty-four adult fertile women receiving general anesthesia for elective gynecologic surgery were included in the study. Patients were classified into four groups on the basis of the phase of menstrual cycle and whether infused dexmedetomidine preoperatively: Group FS: the follicular phase (days 8-12) and preoperative intravenous normal saline; Group FD: the follicular phase (days 8-12) and preoperative intravenous dexmedetomidine; Group LS: the luteal phase (days 20-24) and normal saline; Group LD: the luteal phase (days 20-24) and dexmedetomidine. The patients were infused respectively dexmedetomidine at 0, 1.0, 0 and 1.0 μg/kg over 10 min, and then propofol TCI, which was administered with target plasma concentration at 4.0 μg/ml. BIS, heart rate, MAP were recorded until BIS to 50. In the LD group, the descent range of the BIS values was significantly sharpest among the four groups at loss of eyelash reflex. From propofol administered to loss of eyelash reflex and BIS values reach to 50, propofol requirements were significantly least and duration were shortest in the LD group among the four groups. Menstrual cycle phases affect the sedation of propofol induction with preoperative intravenous dexmedetomidine, which is deeper in the luteal phase. We should cautious of excessive sedation by propofol anesthesia with preoperative dexmedetomidine in the luteal phase.

[Sufentanil during anesthetic induction of remifentanil-based total intravenous anesthesia: a randomized controlled trial].

Project description:BackgroundPostoperative pain represents an important concern when remifentanil is used for total intravenous anesthesia because of its ultrashort half-life. Longer acting opioids, such as sufentanil, have been used during induction of remifentanil-based total intravenous anesthesia as a means to overcome this shortcoming. However, the effectiveness and safety of such strategy still lacks evidence from randomized clinical trials. Hence, we aimed to assess the postoperative analgesic efficacy and safety of a single dose of sufentanil administered during the induction of remifentanil-based total intravenous anesthesia.MethodsForty patients, scheduled for elective open abdominal surgery, were randomized to receive remifentanil-based total intravenous anesthesia with or without a single dose of sufentanil upon induction. We assessed the postoperative morphine consumption administered through a patient-controlled analgesia pump. Self-reported pain scores and the occurrence of nausea, vomiting, pruritus, agitation, somnolence and respiratory depression were also assessed up to 2 days after surgery.ResultsThe mean difference between the sufentanil and control groups regarding morphine consumption in the post-anesthetic care unit and at 12, 24 and 48h after surgery were -7.2mg (95%CI: -12.5 to -2.1, p<0.001), -3.9mg (95%CI: -11.9 to 4.7, p=0.26), -0.6mg (95%CI: (-12.7 to 12.7, p=0.80), and -1.8mg (95%CI: (-11.6 to 15.6, p=0.94), respectively. Neither self-reported pain nor the incidence of adverse events were significantly different between groups at any time point.ConclusionOur findings suggest that the administration of sufentanil during induction of remifentanil-based total intravenous anesthesia is associated with decreased early postoperative opioid consumption.

A Quantile Regression Approach to Estimating the Distribution of Anesthetic Procedure Time during Induction.

Project description:Although procedure time analyses are important for operating room management, it is not easy to extract useful information from clinical procedure time data. A novel approach was proposed to analyze procedure time during anesthetic induction. A two-step regression analysis was performed to explore influential factors of anesthetic induction time (AIT). Linear regression with stepwise model selection was used to select significant correlates of AIT and then quantile regression was employed to illustrate the dynamic relationships between AIT and selected variables at distinct quantiles. A total of 1,060 patients were analyzed. The first and second-year residents (R1-R2) required longer AIT than the third and fourth-year residents and attending anesthesiologists (p = 0.006). Factors prolonging AIT included American Society of Anesthesiologist physical status ≧ III, arterial, central venous and epidural catheterization, and use of bronchoscopy. Presence of surgeon before induction would decrease AIT (p < 0.001). Types of surgery also had significant influence on AIT. Quantile regression satisfactorily estimated extra time needed to complete induction for each influential factor at distinct quantiles. Our analysis on AIT demonstrated the benefit of quantile regression analysis to provide more comprehensive view of the relationships between procedure time and related factors. This novel two-step regression approach has potential applications to procedure time analysis in operating room management.

Project description:In order to identify microRNAs (miRNAs) that were involved in the effects of anesthetics on breast cancer cells, Illumina next generation miRNA sequencing was performed to search for any differentially expressed miRNAs in responding to lidocaine and bupivacaine treatments in MCF7 breast cancer cells. The filtering criteria for identifying differentially expressed miRNAs in the lidocaine and bupivacaine groups were fold change ≥ 2.0X and P-value ≤ 0.05. In total, there were 1,228 significantly upregulated miRNAs in lidocaine group and 133 miRNAs in bupivacaine group. On the contrary, there were 25 significantly downregulated miRNAs in lidocaine group and 36 miRNAs in bupivacaine group.
