Project description:Interventions: Procedure/Surgery : Incision for removal or anastomosis of colon tissue is performed by extending the laparoscopic incision horizontally. A transverse incision is made in the anterior fascia of the rectus muscle. The abdominal rectus muscle is not incised, but the muscles are lateralized out to find the posterior fascia and peritoneum and incise horizontally.
Primary outcome(s): Incisional hernia incidence rate of 1 year (12 months) from the date of surgery

Project description:In this study, we examined C57BL/6J and AJ mice who received either sham surgery or cholestatic intestinal injury. Mice were anesthetized by inhaled isoflurane anesthesia. The abdomen was clipped and then prepared in sterile fashion with 70% ethyl-ethanol followed by betadine. A transverse upper abdominal incision was performed. The CBD was dissected away from the portal vein and was ligated near its junction with the duodenum using aneurysm clips engineered with a precisely standardized opening/closing mechanism. The abdominal wall was then closed in a two-layer fashion using absorbable sutures. Sham-operated mice were treated identically but without dissection or ligation of the CBD.

Project description:Nucleotide excision repair (NER) counteracts the onset of cancer and aging by removing helix-distorting DNA lesions via a ‘cut-and-patch’-type reaction. The regulatory mechanisms that drive NER through its successive damage recognition, verification, incision and gap restoration reaction steps remain elusive. Here we show that the RAD5-related translocase HLTF facilitates repair through active eviction of incised damaged DNA together with associated repair proteins. Our data shows a dual incision-dependent recruitment of HLTF to the NER incision complex, mediated by HLTF’s HIRAN domain that binds 3’-OH single-stranded DNA ends. HLTF’s translocase motor subsequently promotes dissociation of incised oligonucleotides together with members of the stably damage-bound incision complex to allow efficient PCNA loading and initiation of repair synthesis. Our findings uncover HLTF as an important NER factor that actively evicts DNA damage, thereby providing an additional quality control by coordinating the transition between the excision and DNA synthesis steps to safeguard genome integrity.

Project description:12 – 14-week-old male C57Bl6N mice were divided into 3 treatment groups (n=5). First, non-preconditioned animals received ad libitum access to food under a daily 12 hour light and 12 hour dark rhythm with similar specific pathogen free (SPF) conditions in group-cages with up to 5 mice at the same time. Calorically restricted mice obtained 66% of the usual daily intake (3 g per day per mouse) 28 days before surgery as previously described (PMID: 30522767). For the hypoxic preconditioning mice were kept in a hypoxia chamber with a reduction of oxygen to 8.3% for 2 h, 4 h and 8 h on three following days prior to surgery based on the description of Bernaudin et al. (PMID: 11919510). On the day of Ischemia-Reperfusion Injury (IRI) all mice were anesthetized by ketamin (Zoetis (Berlin, Germany))/xylazine (Bayer (Leverkusen, Germany)) and afterwards all mice received a midline abdominal incision as previously described (PMID: 27557097). After ligation of the right kidney vessels right nephrectomy was performed and the kidneys were cut into halves. One half was frozen in liquid nitrogen for proteomic analyses. Then, the left renal vessels were clamped for 40 minutes followed by a subsequent removal of the clamp for reperfusion. The abdominal incision was closed. Subsequently, the mice were placed into single cages. 4 h after reperfusion and following another anesthesia the left kidneys were taken out. Again, the kidneys were cut into two halves and one of it was preserved in liquid nitrogen for proteomic analyses.

Project description:Two female pigs weighing 40-55 kg were used. Each animal had four 4-cm midline caudal abdominal incisions created with the test devices. The test devices were monopolar electrosurgery (Electrocautery Pencil, ConMed, Utica NY) and Harmonic blade (HK105, Ethicon Endo-Surgery, Cincinnati OH). Each animal had two Harmonic Blade incisions and two Electrosurgery incisions for a total of 8 incisions. The incisions were created by marking the site on the ventral aspect of the abdomen and identified by level from cranial to caudal, as incision A, B, C and D respectively. The incision was started with a scalpel blade through the skin (epidermis and dermis). Each incision was deepened through the subcutaneous tissues down to the linea alba using the Harmonic at power level 5 for cutting tissue and power level 3 for coagulating bleeders, or electrosurgery set at 40 Watts for cutting and Blend 2/Spray for coagulation. The linea alba was not incised. Closure of the incision was accomplished by apposing the skin with 3-0 nylon in a simple interrupted pattern. Dermabond was then applied to seal the epidermis. No bandages were applied to the incisions. After three days, incision sites were removed en bloc from the abdominal wall. The tissue block was sliced perpendicular to the surgically incision to create slabs 4-6 mm thick. The slab was trimmed to remove the skin from the top and muscle from the bottom of the sample. The side walls were trimmed to approximately 4 mm from the surgical incision leaving a rectangular block of subcutaneous fatty tissue. Four to six samples were taken from each incision. Animals were then euthanized. Samples for genomics and proteomics analysis were placed in sterile DNAse-, RNAse-, protein-free Eppendorf tubes, flash frozen in liquid nitrogen, held on dry ice throughout tissue collection, and then stored at -80 C. Tissue samples were also collected in 10% neutral buffered formalin for histopathology analysis.

Project description:This study compares the gene expression changes in Sus scrofa in response to two different methods for abdominal surgical incisions ; electrosurgery and harmonic blade. As the use of laparoscopic surgery has become more widespread in recent years, the need has increased for minimally-invasive surgical devices that effectively cut and coagulate tissue with reduced tissue trauma. Although electrosurgery (ES) has been used for many generations, newly-developed ultrasonic devices (HARMONIC® Blade, HB) have been shown at a macroscopic level to offer better coagulation with less thermally-induced tissue damage. We sought to understand the differences between ES and HB at a microscopic level by comparing mRNA transcript and protein responses at the 3-day timepoint to incisions made by the devices in subcutaneous fat tissue in a porcine model This study used four 4-cm midline caudal abdominal incisions created with the test devices in two female pigs. The test devices were monopolar electrosurgery and Harmonic blade. Each animal had two Harmonic Blade incisions and two Electrosurgery incisions for a total of 8 incisions

Project description:ObjectivesThis study aimed to describe a modified, curved, deep, bidirectional, intra-umbilical, vertical incision for primary trocar insertion and prospectively compare its intraoperative and postoperative outcomes with an infra-umbilical incision in gynaecologic laparoscopy.MethodsBetween August 2019 and March 2021, 110 patients subjected to the direct trocar insertion technique for laparoscopic intervention were classified into two groups. Group A comprised 55 cases of infra-umbilical incision, whereas group B comprised 55 cases of a modified, curved, longitudinal, deep, bidirectional, intra-umbilical incision. Afterwards, intraoperative and postoperative assessments were performed.ResultsThe increase in the numbers of parity, gravidity and previous caesarean sections was found to be statistically significant; a smaller number of infertility complaints were observed in group B. Similarly, group B expressed a statistically significant less peri-trocar CO2 leakage (46 [83.6%] patients versus 28 [50.9%] patients) and more tightness of the primary portal entry (45 [81.8%] patients versus 30 [54.5%] patients) when compared to group A throughout the whole operation. After a one-month follow-up, a statistically significant (P = 0.029) decrease in the Observer Scar Assessment Scale and Patient Scar Assessment Scale scores in group B (10.4 ± 4.2 and 11.8 ± 4.3, respectively), demonstrating better cosmoses when compared to group A (13.3 ± 5.7 and 16.0 ± 6.8, respectively).ConclusionPerforming a modified, curved, deep, bidirectional, intra-umbilical, vertical incision for the insertion of a primary laparoscopic trocar (i.e. Darwish laparoscopic entry) is a simple and fast step that results in the elimination of intraoperative gas leakage and trocar slippage without the need for any additional sutures. Aesthetically, it results in a better scar with satisfactory cosmoses when compared to an infra-umbilical incision.
