Project description:Study hypothesis: A decreased faecal load in the surgical wound diminished the incidence of surgical site infection.
Primary outcome(s): Incidence of surgical site infection. The criteria to define surgical site infection are described by Disease Control and Prevention National Healthcare Safety Network (Spain). For the purposes of this study superficial and deep wound infections would be considered as a unique entity condition. This will be measured 30 days after the operation.

Project description:Wound healing is impaired by infection; however, how microbe-induced inflammation modulates tissue repair remains unclear. We took advantage of the optical transparency of zebrafish and a genetically tractable microbe, Listeria monocytogenes, to probe the role of infection and inflammation in wound healing. We found a critical window of microbial clearance necessary to limit persistent inflammation and enable efficient wound repair. Infection with bacteria engineered to activate the inflammasome, Lm-Pyro, induced persistent inflammation and impaired healing despite low bacterial burden. In contrast, infection with an anti-inflammatory, apoptosis inducing strain, Lm-Apo, had similar infectious burden but was associated with rapid wound repair. Inflammatory infections induced il-1b expression and blocking IL-1R signaling partially rescued wound healing in the presence of persistent infection. Taken together, our findings suggest that the dynamics of microbe-induced tissue inflammation impacts repair in complex tissue damage independent of bacterial load, with a critical early window for efficient tissue repair.

Project description:It is becoming increasingly apparent that commensal skin bacteria have an important role in wound healing and infection progression. However, the precise mechanisms underpinning many of these probiotic interactions remain to be fully uncovered. In this work, we demonstrate that the common skin commensal Cutibacterium acnes can limit the pathogenicity of the prevalent wound pathogen Pseudomonas aeruginosa in vivo. We show that this impact on pathogenicity is independent of any effect on growth, but occurs through a significant down regulation of the Type Three Secretion System (T3SS), the primary toxin secretion system utilized by P. aeruginosa in eukaryotic infection. We also show a down regulation in glucose acquisition systems, a known regulator of the T3SS, suggesting that glucose availability in a wound can influence infection progression. This suggests that introducing carbon source competition within the wound microenvironment may be an effective way to prevent or limit wound infection.

Project description:Wound infections are traditionally thought to occur when microbial burden exceeds the innate clearance capacity of host immune system. Here we introduce the idea that the wound environment itself plays a significant contributory role to wound infection. We developed a clinically relevant murine model of soft tissue infection to explore the role of activation of microbial virulence in response to tissue factors as a mechanism by which pathogenic bacteria cause wound infections. Mice underwent abdominal skin incision and light muscle injury with a crushing forceps versus skin incision alone followed by topical inoculation of Pseudomonas aeruginosa. Pseudomonas aeruginosa whole genome transcriptional profiling demonstrated that fascia induced the activation of multiple genes responsible for the synthesis of the iron scavenging protein pyochelin. Ex-vivo murine fascia homogenates were prepared and Pseudomonas aeruginosa MPAO1 was incubated with an inoculum of the fascia homogenate solution. Pseudomonas aeruginosa MPAO1 incubated under the same condtions without the homogenate was used as the control group. Three biological replicates in each group was used.

Project description:Infections of burn wounds, especially those caused by Pseudomonas aeruginosa, could trigger sepsis or septic shock, which is the main cause of death after burn injury. Compared with traditional saline-wet-to-dry dressings, negative pressure wound therapy (NPWT) is more effective for the prevention and treatment of wound infections. However, the mechanism by which NPWT controls infection and accelerates wound healing remains unclear. Accordingly, in this study, the molecular mechanisms underlying the effects of NPWT were explored using a murine model of P. aeruginosa-infected burn wounds. NPWT significantly reduced P. aeruginosa levels in wounds, enhanced blood flow, and promoted wound healing. Additionally, NPWT markedly alleviated wound inflammation and increased the expression of wound healing–related molecules. Recent evidence points to a role of circular RNAs (circRNAs) in wound healing; hence, whole-transcriptome sequencing of wound tissues from NPWT and control groups was performed to evaluate circRNA expression profiles.

Project description:Infected wounds pose a major mortality risk in animals. Injuries are particularly common in the ant Megaponera analis which raids pugnacious prey. Here we show that M. analis can determine when wounds are infected and treat them specifically. By applying a variety of antimicrobial compounds and proteins secreted from the metapleural gland to infected wounds workers reduce the mortality of infected individuals by 90%. Chemical analyses showed that wound infection is associated with specific changes in the cuticular hydrocarbon profile thereby likely allowing nestmates to diagnose the infection state of injured individuals and apply the appropriate antimicrobial treatment. This study demonstrates that the targeted use of antimicrobials to treat infected wounds, previously thought to be a uniquely human behavior, has evolved in insect societies as well.

Project description:The clinical assessment of wound infection and related complications is challenging and the development of objective methods to measure wound status and predict healing outcomes is therefore essential. As bacterial protease activities play important roles in infection, analysis of changes in the wound peptidome by individual bacterial enzymes could provide insight into proteolytic events occurring in infected wounds, and may aid in the discovery of biomarkers. Wound infection was mimicked by incubating sterile acute wound fluids ex vivo with the bacterial proteases Pseudomonas aeruginosa LasB and Staphyloccocus aureus V8. Using a peptidomics approach, we characterized the low-molecular-weight peptidome of the digested samples and identified over 100 protein targets for each enzyme. Additionally, we found enzyme-specific peptides and digestion patterns.
