Project description:Background and study aims
An advance notification letter conveying information on colorectal cancer (CRC) risk and the intervention offered in the screening programme, may increase the likelihood of a positive response to the subsequent screening invitation, as the subject will have progressed in his degree of readiness for change. In addition, it has been shown that, in particular for less educated subjects, general practitioners (GP) advice may have a strong influence on the decision to be screened. The study aims to assess the impact on the response rate to the invitation for CRC screening, in the context of population based programs, of an advance notification letter mailed to subjects eligible for invitation. The additional cost per person examined and the additional workload for the subjects GPs will be assessed.
Who can participate?
All subjects (male and female) aged 50 to 69 targeted for CRC screening in 8 population based programmes in Italy.
What does the study involve?
During the enrolment period all subjects targeted for invitation in these screening programs were randomly allocated to three groups:
A: standard personal invitation letter, signed by the GP, to undergo CRC screening
B: advance notification letter, mailed one month before the standard invitation letter (same as in A), conveying information on CRC risk, on the screening program and on the expected benefits and potential harms of screening
C: as in B, with the addition, in the advance notification letter, of the offer of a personal encounter with the subjects GP, to discuss pros and cons of screening. The response rate across the different groups will be assessed at 6 months following the invitation letter.

Project description:Participation in a primary screening colonoscopy programs remains low. Advanced notification letter has been shown to increase participation in colorectal cancer screening with fecal occult blood testing and to be cost effective compared to standard invitation. It is unknown whether advanced notification letter increases participation rate in primary colonoscopy screening program.
We hypothesize that an Advance Notification Letter will have significant influence on participation in screening colonoscopy, comparing to standard invitation procedure and will thus result in higher efficiency of the program.
This randomized controlled study aims to compare the participation rate in screening colonoscopy in response to advanced notification plus standard invitation letter and standard invitation letter alone.
Material and methods: 6800 individuals aged 55-64 years will be drawn from the Population Registry and randomly assigned in a 1:1 ratio to the group invited for screening colonoscopy with advanced notification letter (send two weeks before standard invitation) plus standard invitation (send six weeks before planned screening colonoscopy) or to standard invitation only (send six weeks before planned screening colonoscopy). The sample size was calculated to detect 3% difference in participation rate between the groups (25% vs 28%) with 80% power.

Project description:Interventions: Two alternative advance notification strategies will be trialed. Intervention 1 will consist of a simple ‘advance notification of an invitation to participate in screening for Colorectal Carcinoma (CRC)’ letter incorporating messages introducing the concept of screening for CRC, increased risk for CRC and the value of screening to reduce risk. Intervention 2 (enhanced advance notification) will include those messages plus more complex messages to reduce known barriers (lack of knowledge about CRC and its prevention, aversion for faecal sampling, low perception of the value of Fecal Occult Blood Test (FOBT) screening). The advance notification letters will be followed in 2 weeks by a standard invitation-to-screen letter. A control group will receive the standard invitation only. Invitees have 12 weeks from date of invitation-to-screen in which to participate in screening.
Primary outcome(s): Completion of faecal immunochemical test expressed as the population participation rate.[Within 12 weeks of screening invitation.]

Project description:Participants will be mailed an invitation to complete CRC screening along with a fecal immunochemical test (FIT) kit, containing a 1-sample "Polymedco Over the Counter (OC) Sensor FIT", simplified English/Spanish instructions on performing the test, educational information about colorectal cancer screening and a return mailer with prepaid postage.
Processes that will be used to promote screening completion include automated and "live" phone call reminders to encourage completion of FIT testing.
Participants will be randomly assigned to 1 of 5 interventions across 3 conditions, described below:
1. Condition 1 (Standard Intervention):
* Branch I: Participants assigned to the control condition will receive a FIT kit and the standard invitation letter to participate in free screening.
2. Condition 2 (Time Guideline):
* Branch II: Participants will receive a FIT kit and invitation letter to participate in free screening with a 1-week time restriction.
* Branch III: Participants will receive a FIT kit and invitation letter to participate in free screening with a 3-week time restriction.
3. Condition 3 (Time Guideline + Incentive):
* Branch IV: Participants will receive a FIT kit and invitation letter to participate in free screening requesting they return the kit within 1-week for a "higher" monetary incentive or within 3 weeks for a "lower" (half of the higher) monetary incentive.
* Branch V: Participants will receive a FIT kit and invitation letter to participate in free screening requesting they return the kit within 1-week for a "higher" (same as lower in Branch IV) monetary incentive or within 3 weeks for a "lower" (half of the higher) monetary incentive.
Participants with a normal test result will receive a personal letter confirming this with invitations to complete a repeat screening in subsequent year(s). Participants receiving an abnormal FIT result will be navigated to complete a diagnostic colonoscopy.
Participants continuing in the screening program in subsequent years will receive letters emphasizing the importance of repeat screening to prevent adverse CRC outcomes.

Project description:Interventions: Name of intervention: gender specific mail-administered screening invitation strategies.
All participants will receive an offer to participate in a free FOBT which will be delivered by mail. Intervention arms will differ from control according to the information included in the materials accompanying the screening offer. All study participants will receive a standard screening reminder irrespective of their intervention group. Intervention materials will target specific psychosocial variables previously identified as being related to male participation in FOBT screening.
The intervention period is 14 weeks. All participants will receive a screening offer according to the following timeline.
Baseline: Advance notification letter sent
Week two: Invitation package sent
Week eight: Reminder letters sent to those who have not completed an FOBT
Week 14: Data collection ceases.
Arm 1: Control
The control arm will receive a standard offer to participate in FOBT screening designed to mimic the strategy utilised by the Australian national bowel cancer screening program (NBCSP).
The standard offer includes an advance notification letter and a screening invitation package.
The advance notification letter is sent two weeks prior to the screening invitation package and is designed to alert participants that an invitation to screen for CRC with an FOBT will soon follow. The standard advance notification letter introduces basic information about colorectal cancer and the health benefits of screening.

Project description:Interventions: The aim is to determine the value of GP endorsement for faecal occult blood test based re-screening colorectal cancer. The interventions are conducted within the setting of a population screening program where invitees are recruited from general practice patient lists.
Intervention 1: Invitation letter to re-screen on central screening facility letterhead with statement from invitee’s general practice endorsing participation in screening, signed by screening coordinator.
Intervention 2: Invitation letter to rescreen on practice letterhead with statement from practice endorsing screening, signed by invitee’s general practitioner.
Comparison group: recruited from electoral roll receiving control invitation to rescreen.

Project description:Interventions: Group 1: Group A = Insured persons in group A receive a personal invitation letter from the AOK-BW, which describes early detection measures of colorectal cancer, invites them to carry out a quantitative FIT (fecal immunochemical tests) and additionally already includes such a test.
Group 2: Group B = Insured persons in group B receive a personal invitation letter from the AOK-BW, which describes early detection measures of colorectal cancer, invites them to carry out a quantitative FIT and also provides the option to order such a test for free (by mail, fax, internet) at the DKFZ.
Group 3: Group C (control group) = Insured persons in group C receive a personal letter from the AOK-BW, which describes early detection measures of colorectal cancer, but they are neither automatically provided a quantitative FIT nor can they order it at the DKFZ.
However, within the regular care, the insured persons can take precautionary measures if they are interested. Thus, the measures in group C (providing regular, insurance-related information, recording the use of precautionary measures in the regular care) are carried out within the framework of normal, already existing procedures and are independent of the study.
Primary outcome(s): The primary endpoint is the usage rate of the test offer after receiving an invitation letter, as well as the usage rate of the test offer after recieving an invitation letter with subsequent reminder in comparison to the one-time letter, respectively within one year after the initial letter.

Project description:Objective: To assess the effectiveness of a brief telephone intervention added to an post invitation letter in the participation rate to a colorectal cancer screening program.
Methods: Randomized controlled trial. This study is framed inside the colorectal cancer screening program of Catalonia, concretely in the region of Vallès Occidental. This program is directed to men and women from 50 to 69 years. The screening test used is the fecal immunological test (FIT) that is offered every 2 years. If the screening test is positive, a colonoscopy with sedation is offered as a diagnostic test.
