Project description:Interventions: The intervention is based on cognitive behavior therapy (CBT) and consists of blended therapy, a combination of face-to-face CBT with a self-management interactive website. The intervention lasts four months and compromises of five individual face-to-face sessions, supplemented by three telephone contacts. Patients will have daily access to the self-management website. The CBT protocol is directed at changing cognitions and behavior related to distress. Treatment will be individually-tailored based on the presenting problems identified during clinical assessment and and the data gathered during baseline assessment. The intervention consists of one generic module and three optional modules. Three different types of distress will be addressed in the modules: 1) distress caused by physical consequences (e.g. post-cancer fatigue, gastrointestinal problems, urinary incontinence and sexual dysfunction), 2) anxiety and fear of recurrence, 3) depressive mood.
Primary outcome(s): Psychological distress as measured by the Brief Symptom Inventory (BSI-18)

Project description:Interventions: The intervention is designed as blended care, combining traditional face-to-face cognitive behavior therapy (CBT) with online activities (psycho-education, assignments, assessments and therapist contact). If access to the website is not possible a workbook will be supplied instead. The CBT lasts three months and comprises five individual hourly face to face sessions, supplemented with three (15 minute) electronic consultations (‘e-consults’) or contact by telephone. The therapy protocol is based on a theoretical model by Lee-Jones (1997) which hypothesizes that an emotional reaction (fear) can be the result of interpretations and cognitions of the threat of cancer, triggered by perceptions of internal and/or external cues, leading to dysfunctional behavior and subsequently an increased fear response. The CBT is directed at change of the cognitions and behaviors managing FCR. It contains techniques commonly used in psychotherapy, such as cognitive reframing, exposure- and response prevention, psycho-education, mindfulness and relaxation exercises.
Primary outcome(s): Fear of cancer recurrence as measured with the cancer worry scale (CWS).

1) Week 1st: Before the day of discharge
- Goal: Dietary management, symptom management, exercise
- Contents: Function of the rectum, definition of low anterior resection syndrome (bowel dysfunction), disease experience of rectal cancer patients, purpose and method of pelvic floor muscle training (PFMT), diet management, postoperative management (symptom management)
- Education method (strategy): face-to-face, tablet PC, exercise demonstration and practice, exercise video provision (mobile phone, USB), primary caregiver participation, exercise/diet diary education
- Time: 30 minutes
2) Week 2nd: First outpatient visit
- Goal: exercise check, motivation (reinforcement)
- Contents: Introduction of a successful case of PFMT, Check of suitability of PFMT
- Education Method (Strategy): face-to-face, introduction of a successful case (printed material), text messages, exercise/diet diary records
- Time: 30 minutes
3) Week 3rd: 3 weeks after discharge
- Goal: Exercise adherence check, Identification of barriers of PFMT
- Contents: Identification of difficulties in performance and maintenance of PFMT, physical/psychological state check and coaching, emotional support
- Education Method (Strategy): telephone coaching, text messages, exercise/diet diary records
- Time: 10-20 minutes
4) Week 4th: 4 weeks after discharge
- Goal: Overcoming the barriers, maintaining PFMT
- Contents: Discussion on successful PFMT experience, planning of PFMT maintenance

Project description:Analysis of transcript abundance estimates as a function of child soldier status, PTSD symptoms, and psychological resilience. Gene expression profiling was conducted on dried blood spot (DBS) samples collected from community dwelling adolescents and young adults in Nepal. Approximatley half of the sample were former child soldiers in the Nepal People's War and the other half were demographically similiar civilian non-combatants. In addition to basic demographic characteristics (age, sex, ethnic minority status, social caste status, education level), participants were also assessed on syptoms of post-traumatic stress (PTS, assessed by a culturally adapted version of The Child PTSD Symptom Scale; Kohrt BA, et al. (2011) Validation of cross-cultural child mental health and psychosocial research instruments: adapting the Depression Self-Rating Scale and Child PTSD Symptom Scale in Nepal. Project description:This study was conducted to examine the effect of the nurse navigation program (NNP) applied to patients with colorectal cancer on symptom management and psychosocial adjustment to the disease. The design type was determined as a randomized controlled experimental study with a pretest-posttest control group, with repeated measurements. For this purpose, individuals were divided into two groups as experimental (n=31) and control (n=31) groups. The study was carried out in Istanbul Lütfi Kırdar City Hospital Oncology Clinic between July 2021 and February 2022, after the necessary permissions were obtained. While individualized colorectal cancer education, psycho-oncological counseling and telephone support services were provided to the individuals in the experimental group under the guidance of nurse within the scope of NNP, no intervention was applied to the control group. In the study, data were collected with three different data collection tools: "Information Form", "Nightingale Symptom Assessment Scale (N-SAS)", "Psychosocial Adjustment To Illness Scale (PAIS/PAIS-SR)". Data were collected before NNP (once in the first week after chemotherapy), during NNP (once in the second week after chemotherapy, once in the first week after the next chemotherapy), after NNP (once in the second week after the next chemotherapy). The duration of the interventions performed via the WhatsApp application varied between 45-60 minutes between individuals. The research was completed with a total of 60 individuals, 30 in the experimental group and 30 in the control group. The data obtained in the study were analyzed using the SPSS (Statistical Package for Social Sciences) for Windows 22.0 program.

Health awareness is a more advanced knowledge achievement; it is the ability to make judgments and decisions in daily life related to health care, disease prevention and health promotion, and is the basic element of achieving self-management of health. Dietary knowledge is also a higher-level knowledge, which reflects dietary knowledge in behavior, food choices and criticism, and reflects on food choices and health decisions. Insufficient dietary knowledge will affect food choices, judgments and decisions, resulting in insufficient diet-related knowledge.Studies have confirmed that the health-related knowledge of cancer patients is significantly related to disease self-management. Only when patients understand their own diseases can they actively participate in self-health management. Nursing staff play a pivotal role in the care of patients with colorectal cancer. Through the knowledge transfer of nursing staff and guiding patients to take care of themselves, they can help improve postoperative dietary intake behaviors, thereby enhancing dietary awareness. Literature review of chronic disease self-management, nursing staff provide patients with sufficient relevant knowledge education, which can enhance patients’ self-efficacy and enhance disease self-management ability.

Project description:Here we tested a hypothesis that epileptogenesis influences expression pattern of genes in the basolateral amygdala that are critical for fear conditioning. Whole genome molecular profiling of basolateral rat amygdala was performed to compare the transcriptome changes underlying fear learning in epileptogenic and control animals. Our analysis revealed that after acquisition of fear conditioning 26 genes were regulated differently in the basolateral amygdala of both groups. Thus, our study provides the first evidence that not only the damage to the neuronal pathways but also altered composition or activity level of molecular machinery responsible for formation of emotional memories within surviving pathways can contribute to impairment in emotional learning in epileptogenic animals. Understanding the function of those genes in emotional learning provides an attractive avenue for identification of novel drug targets for treatment of emotional disorders after epileptogenesis-inducing insult.
