Project description:Simvastatin has been widely used for treatment of hypercholesterolemia due to its ability to inhibit HMG-CoA reductase, the rate limiting enzyme of de novo cholesterol synthesis via mevalonate pathway. Its inhibitory action causes also depletion of pathway intermediates, farnesyl pyrophosphate (FPP) and geranyl-geranyl pyrophosphate (GGPP), which are inevitable for proper targeting of small GTPases (e.g. Ras proteins) to their site of action. We profiled by array the gene expression of MIA PaCa-2 cells treated with simvastatin, FPP, GGPP and their combinations. The inhibitory effect of statins on GFP-K-Ras protein trafficking were partially prevented by addition of the mevalonate pathway intermediates. We conclude that the anticancer effect of simvastatin is to a large extent mediated through isoprenoid intermediates of the mevalonate pathway.

Conclusion: Three antimalarial classes display potent antiproliferative activity, and their potency is correlated with cancer cell gene expression patterns. Significance: We confirm and extend anticancer potential of these antimalarials and we discuss their therapeutic potential based on clinical data. Project description:Interventions: Standard of care anticancer treatment plus enoxaparin thromboprophylaxis 40mg self-administered subcutaneously once daily for minimum 3 months, or until completion of anticancer therapy, to maximum 6 months (adherence monitored by drug returns).
Primary outcome(s): Objectively confirmed symptomatic or asymptomatic thromboembolism.
All thromboembolic events must be confirmed by appropriate standard diagnostic investigations such as radiological imaging for DVT or PE and cardiac investigations for suspected myocardial infarct. Study specific imaging will not be performed. A blinded clinical review panel will directly assess and review all radiological and cardiac investigations to confirm events. [At time of any routine imagining for cancer response or at clinical suspicion of thromboembolism, until six months after randomisation.

Project description:Through transcriptome analysis, we uncovered 3 modules correlated with specific stages/time frames after photothrombotic ischemia. Then, 3 modules were identified by hub gene network analysis in combination with biological function analysis and KEGG pathway analysis. Among these 3 modules, the E-module hubs, the G-module hubs, and the I-module hubs contained genes correlated with the chemotaxis, the cell cycle and mitosis or the immune response/inflammation, respectively. Evidence shows that CCAAT/enhancer binding protein α (C/EBP-α)-induced inflammation has been linked to ischemic stroke. More interestingly, CEBPA, which encodes C/EBP-α, was shown to be one of the hub genes associated with the immune response/inflammation, which prompted us to carry out a more detailed investigation in this study. Here, we investigate the potential mechanism underlying the action of C/EBP-α in the etiology of ischemic brain injury.

Project description:This study is an open-label, multi-arm, parallel cohort, dose validation and expansion design. The study is modular in design, allowing evaluation of the safety, efficacy and pharmacokinetics (PK) of NUC-3373 in combination with other agents for the treatment of patients with different tumour types.
Each module is designed to evaluate a different NUC-3373 combination and consists of a dose-validation phase (Phase Ib) and a dose-expansion phase (Phase II).
Phase Ib of each module will determine the safety and tolerability of the combinations for further clinical evaluation in Phase II. Approximately 6-20 evaluable patients will be enrolled in the Phase Ib stage of each module to determine safety, tolerability, and preliminary efficacy of NUC-3373 in combination with other agents. Each module will then move into Phase II to enable a further assessment of safety and efficacy in approximately 20-40 patients.
Module 1 will assess NUC-3373 + leucovorin (LV) in combination with pembrolizumab for the treatment of patients with advanced/metastatic solid tumours who have progressed on ≤2 prior therapies for metastatic disease, that may have included 1 prior immunotherapy-containing regimen (either monotherapy or in combination with chemotherapy) or who have not progressed but where addition of NUC-3373 + LV to standard pembrolizumab monotherapy may be appropriate (e.g., patients who could not tolerate post- immuno-oncology (IO) standard of care therapy).
Module 2 will assess NUC-3373 + LV in combination with docetaxel for the treatment of patients with advanced/metastatic non-small cell lung cancer (NSCLC) or pleural mesothelioma who have progressed on, or were unable to tolerate, 1 or 2 prior lines of cytotoxic chemotherapy-containing regimens for advanced/metastatic disease.
The opening of each module will be at the discretion of the Sponsor. Further modules may be added as non-clinical and clinical data become available to support additional NUC-3373 combinations and tumour types.
