Project description:PurposeThere is a lack of level I evidence to guide radiation therapy recommendations for patients receiving neoadjuvant chemotherapy for breast cancer. We used 4 neoadjuvant chemotherapy trials to determine which patients benefit from regional nodal irradiation (RNI).Methods and materialsWe obtained data from the NSABP (National Surgical Adjuvant Breast and Bowel Project) B-18, B-27, B-40, and B-41 clinical trials. B-40 and B-41 allowed RNI at physician's discretion. We evaluated locoregional recurrence (LRR), distant recurrence, disease-free survival, and overall survival (OS). Kaplan-Meier, Peto-Peto, χ2, Fisher exact, and Wilcoxon rank-sum tests were used for survival estimates and comparison.ResultsMedian follow-up for B-18, B-27, B-40, and B-41 was 13.7, 9.7, 4.5, and 5.1 years, respectively, including 742, 2254, 1154, and 504 patients for analysis. On multivariable analysis, factors significantly associated with RNI included tumor size, ypN status, and tumor subtype; Hispanic patients were less likely to receive RNI. Patients with ypN+HER2+ disease who received RNI had improved OS. B-40 patients with ypN+HR+ disease had improved LRR. On multivariable analysis for the B-40 and B-41 study population, RNI was not associated with significantly improved OS, disease-free survival, distant recurrence, or LRR.ConclusionsRNI was associated with a clinical benefit for patients with ypN+HER2+ and ypN+HR+ disease. RNI was not significantly associated with a clinically beneficial outcome for the entire cohort. Prospective phase 3 clinical trials are needed to establish guidelines for patients who should receive RNI after neoadjuvant treatment, and action is necessary to eliminate the disparity in care delivery shown for Hispanic women.

Project description:The objective of this study was to analyze the temporal trend of physical activity and bodymass index in young adults aged 18-30 in Spain and to ascertain their relationship withsociodemographic and psychosocial variables in the period of 2009-2017. METHODS:A descriptivestudy with a sample of 10,061 young adults aged 18-30 years was performed. The data wereobtained from the European Health Survey in Spain in 2009 and 2014 and the National HealthSurvey in 2011/2012 and 2017. The chi-square test was used for qualitative variables, and multiplelinear regression analysis was performed for physical activity. RESULTS:Sedentary levels haddecreased in 2017 as compared to 2011/2012 (p < 0.001); smokers were more sedentary than nonsmokers(p < 0.001); men were more active than women (p < 0.001); and the year with the highestphysical activity was 2014. Body mass index in the total sample increased from 2009 to 2017 (p <0.01), showing a significant increase in obesity in women (p < 0.05) and no difference in men (p ?0.05). CONCLUSIONS:In the period 2011/2012-2017, the sedentary lifestyle of young adults wasreduced and physical activity was increased, with men being more active than women.

Project description:Background:Serum/plasma YKL-40 can be a useful index that is associated with tumor development. However, the prognostic value of serum/plasma YKL-40 in patients with solid tumors is still unclear. We aimed to utilize the existing literature to investigate the prognostic value of serum/plasma YKL-40 in solid tumors. Methods:An extensive literature search for relevant studies was conducted with the Embase, Medline and Web of Science databases. The effect on survival was measured with the hazard ratio (HR). Then, pooled HRs and 95% confidence intervals (CIs) were calculated using the random and fixed-effects models according to the heterogeneity of the included studies. Results:This meta-analysis was based on 41 publications and comprised a total of 7762 patients with solid tumors. The pooled HR showed that elevated serum/plasma YKL-40 was significantly associated with poor OS (HR, 1.44; 95% CI 1.33-1.56). We also found that elevated serum/plasma YKL-40 had significant prognostic effects on OS in various cancer subgroups such as gastrointestinal tumors (HR, 1.37; 95% CI 1.18-1.58), ovarian cancer (HR, 2.27; 95% CI 1.69-3.06), melanoma (HR, 1.77; 95% CI 1.18-2.67), lung cancer (HR, 1.73; 95% CI 1.35-2.23), urologic neoplasms (HR, 1.61; 95% CI 1.08-2.40) and glioblastoma (HR, 1.23; 95% CI 1.07-1.42); in contrast, the prognostic effect of serum/plasma YKL-40 was not statistically significant in breast cancer (HR, 1.07; 95% CI 0.98-1.17). Conclusions:The available evidence supports the hypothesis that elevated serum/plasma YKL-40 is associated with poor survival in patients with solid tumors and that serum/plasma YKL-40 may serve as a novel prognostic biomarker.

Project description:The snowpack is an important source of water for many Andean communities. Because of its importance, elemental and mineralogical composition analysis of the Andean snow is a worthwhile effort. In this study, we conducted a chemical composition analysis (major and trace elements, mineralogy, and chemical enrichment) of surface snow sampled at 21 sites across a transect of about 2,500 km in the Chilean Andes (18-41°S). Our results enabled us to identify five depositional environments: (i) sites 1-3 (in the Atacama Desert, 18-26°S) with relatively high concentrations of metals, high abundance of quartz and low presence of arsenates, (ii) sites 4-8 (in northern Chile, 29-32°S) with relatively high abundance of quartz and low presence of metals and arsenates, (iii) sites 9-12 (in central Chile, 33-35°S) with anthropogenic enrichment of metals, relatively high values of quartz and low abundance of arsenates, (iv) sites 13-14 (also in central Chile, 35-37°S) with relatively high values of quartz and low presence of metals and arsenates, and v) sites 15-21 (in southern Chile, 37-41°S) with relatively high abundance of arsenates and low presence of metals and quartz. We found significant anthropogenic enrichment at sites close to Santiago (a major city of 6 million inhabitants) and in the Atacama Desert (that hosts several major copper mines).
