Project description:We exploited the extensive genomic diversity of the Leucegene cohort of primary human AML specimens to provide an overview of the human AML surfaceome. Due to high cell number requirements, surface proteomics has been underexploited in AML so far, although surface proteome analysis of AML cell lines and small cohorts of primary human AML specimens paved the way for antigen identification23-25. Herein, we compared global and surface proteomic datasets generated from primary human AML specimens and show that surfaceome analysis uniquely identifies a larger subset of cell surface proteins compared to global proteomics. We therefore built a cohort of 100 primary human AML specimens that was subjected to surface proteome analysis and served as a primary dataset for antigen identification. A significant portion of the cohort also underwent single-cell RNA sequencing, which allowed the exploration of antigen expression at the population level and the selection of AML antigens expressed by primitive blasts. These analyses led to the identification of novel AML antigens expressed by the majority of AML specimens of the cohort, of antigens overexpressed by specific AML subgroups, as well as of previously uncovered potential leukemia stem cell (LSC) markers, and represents the first large-scale surface proteomic study in AML.

Here, by employing synthetic leukemogenesis in human cells, we first show that RAS mutations are obligatory late events that need to succeed earlier cooperating mutations. We provide the mechanistic explanation for this in a requirement for mutant RAS to specifically transform committed progenitors of the myelomonocytic lineage (granulocyte-monocyte progenitors, GMPs) harboring previously acquired driver mutations, revealing that advanced leukemic clones originate from a different cell type than more ancestral clones. Furthermore, we demonstrate that RAS-mutant leukemia stem cells (LSCs) give rise to monocytic disease, as frequently observed in patients with poor responses to treatment with the BCL2 inhibitor drug Venetoclax (VEN). We show that this is because RAS-mutant LSCs, in contrast to RAS-WT LSCs, have altered BCL2 family gene expression profiles and are resistant to VEN, driving clinical resistance and relapse with monocytic features. Our findings demonstrate that a specific genetic driver by imposing a specific LSC target cell restriction shapes the non-genetic cellular hierarchy of AML and critically impacts therapeutic outcomes in patients.

Project description:RAS mutations are frequently found among AML patients, generating a constitutively active signaling protein changing cellular proliferation, differentiation and apoptosis. We previously showed that treatment of AML patients with high-dose cytarabine (HDAC) is preferentially beneficial for those with an oncogenic RAS mutation. By applying a murine AML model, we could ascribe this effect to a RAS-driven, p53-dependent induction of differentiation. Here, we sought to confirm the correlation between RAS status and differentiation of blasts obtained from AML patients. The expression signature of primary AML blasts expressing oncogenic RAS where compared to blasts with wtRAS. Blasts where obtained from peripheral blood or bone marrow samples from patients with CBFbeta-MYH11 translocations and with or without an additional NRAS point mutation. As a reference, the Kasumi-1 acute myeloid leukemia cell line was used.
