Project description:As part of an investigation on contributors to offspring size following assisted reproduction, we investigated maternal mid-/late-pregnancy peripheral blood buffy coat DNA methylation at 240 CpGs previously associated with maternal pregnancy morbidity or offspring growth across various tissues. Illumina Infinium MethylationEPIC BeadChip was used to obtain DNA methylation profiles in previously frozen buffy coat samples from 70 women who conceived via assisted reproduction technologies and gave live births and 845 women who spotaneously conceived and gave live births.

Project description:ART is associated with abnormalities in DNA methylation and a higher incidence of adverse pregnancy outcomes in humans and mice. However, which specific exposure, among the interventions utilized during ART, is responsible for these outcomes remains unknown. Embryo transfer after vitrification-thaw (frozen ET) into a maternal hormonal environment more closely resembling unassisted conception is increasingly utilized clinically, but reports suggest a higher incidence of large for gestational age (LGA) infants, with male infants potentially more affected than female infants.

Health outcomes in offspring born to survivors of childhood cancers following assisted reproductive technologies.

Project description:PurposeAn increasing number of childhood cancer survivors are using assisted reproductive technologies (ART) to overcome treatment-related fertility impairment. We report perinatal and health outcomes of offspring born to survivors following ART.MethodsThe FeCt Multicenter Offspring Study surveyed the health of offspring of childhood cancer survivors. Health outcomes in offspring born to survivors following ART (n = 57, 4.6%) or after spontaneous conception (n = 1182) were assessed in the German cohort (n = 1239) using bivariate analysis. Findings were put into the context of the general German population by health outcome assessment in 1:1 matched-pair analysis (n = 2478).ResultsNearly twice the survivors used ART compared with numbers reported for the German general population (4.6% vs. 2.6%). Successful pregnancies were achieved after a median of two cycles, mainly using non-cryopreserved oocytes/sperm. Multiple sibling births (p < 0.001, 28.1% vs. 3.0%) and low birth weight (p = 0.008; OR = 2.659, 95% CI = 1.258-5.621) occurred significantly more often in offspring born to survivors who utilized ART than spontaneously conceived children, whereas similar percentages were born preterm or too small for their gestational age. ART did not increase the prevalence of childhood cancer or congenital malformations in offspring born to survivors.ConclusionART use by childhood cancer survivors was successful with both fresh and cryopreserved oocytes/sperm, and did not influence perinatal health or health outcomes when known confounders were taken into account.Implications for cancer survivorsOncofertility is an important component of patient care. Our study implicates that the utilization of ART by adult survivors of childhood cancer does not put offspring at additional risk for adverse perinatal or health outcomes.

Why we should talk about compliance with assisted reproductive technologies (ART): a systematic review and meta-analysis of ART compliance rates.

Project description:BackgroundThe goal of this systematic review and meta-analysis was to estimate the rate of compliance with assisted reproductive technologies (ART) and examine its relationship with treatment success rates.MethodsSix databases were systematically searched from 1978 to December 2011. Studies were included if they reported data on patient progression through three consecutive standard ART cycles. Compliance was estimated for the first three ART cycles (typical ART Regimen Compliance, TARC) and after the first and the second failed cycles (CAF1, CAF2). Treatment success rates for all patients who started ART and for those who fully complied with the three ART cycles were estimated.ResultsTen studies with data for 14 810 patients were included. TARC was 78.2% [95% confidence interval (CI) 68.8-85.3%], CAF1 was 81.8% (73.3-88.1%) and CAF2 was 75.3% (68.2-81.2%). The overall success rate was 42.7% (32.6-53.6%) for all patients starting ART and 57.9% (49.4-65.9%) for those who complied with three ART cycles. Compliance rates did not vary according to study quality, but TARC was higher for studies that reported data on doctor-censored patients versus those that did not (84.2% 95% CI 75.5-90.2 versus 70.6% 95% CI 58.3-80.5, P = 0.043). Analysis of funnel plots and the Egger test indicated publication bias for CAF1.ConclusionsFindings from this meta-analysis should reassure clinics and patients that most patients are able to comply with three cycles of ART. Compliers could increase their chances of success by as much as 15%. A more detailed assessment of compliance requires monitoring long-term treatment trajectories through the creation of national registries.

Epigenetic changes and assisted reproductive technologies.

Project description:Children conceived by Assisted Reproductive Technologies (ART) are at moderately increased risk for a number of undesirable outcomes, including low birth weight. Whether the additional risk is associated with specific procedures used in ART or biological factors that are intrinsic to infertility has been the subject of much debate, as has the mechanism by which ART or infertility might influence this risk. The potential effect of ART clinical and laboratory procedures on the gamete and embryo epigenomes heads the list of mechanistic candidates that might explain the association between ART and undesirable clinical outcomes. The reason for this focus is that the developmental time points at which ART clinical and laboratory procedures are implemented are precisely the time points at which large-scale reorganization of the epigenome takes place during normal development. In this manuscript, we review the many human studies comparing the epigenomes of ART children with children conceived in vivo, as well as assess the potential of individual ART clinical and laboratory procedures to alter the epigenome.
