Project description:Background: Myocardial infarction (MI) is the single most critical event in coronary disease. Platelets partecipate in processes that precipitate acute MI, lack nuclear DNA but retain megakaryocyte mRNAs, thus their transcriptome could provide information preceding the coronary event. Our aim was to obtain a gene expression atlas of platelets from patients with their very first acute MI (FAMI) in order to identify potential markers of precipitating factors. Methods and results: Platelets from seventeen FAMI patients and matched controls were collected and RNA and protein were purified. Expression profiles were obtained with genome-wide microarrays. Platelet-specific proteomic analysis was also performed with fluorescent DIGE separation in the same samples. Strong transcriptional down-regulation was observed in patient platelets, whereas more balanced numbers of up and down-represented proteins were found by proteomic analysis. Conclusions: Platelet transcriptome revealed quantitative differences between FAMI patients and controls. The functional analysis deregulated mRNAs and the integration with proteomic data in platelets may suggest pre-activation of platelets and/or megakaryocytes in FAMI patients before the acute event.

Project description:Twenty-three AMI patients and 23 non-AMI healthy controls were included in this pilot study. Tissue samples were cut at 10 μm from formalin fixed paraffin-embedded tissue blocks using a microtome. Six to eight 10-μm sections were used for the isolation procedure. Total RNA isolation was performed using a miRNeasy FFPE kit (Qiagen) according to the manufacturer’s protocol. RNA concentration and purity was determined and before gene expression profiling (Affymetrix Human Exon 1.0ST Array). The microarray labeling, hybridization and processing was performed according to the manufacturer’s protocol at the Microarray Core Facility of Chinese National Human Genome Center, Shanghai, China. The raw data of microarray were quantile-normalized over all samples, summarized with the robust multi-array average (RMA) algorithm and log2 transformed with a median polish for ~22,000 transcript clusters (gene-level). Significance Analysis of Microarrays (SAM) was used to identify differential genes between AMI patients and non-AMI controls. Project description:ObjectivesTo study clinical characteristics, mortality, and secondary prevention, after a first incident acute myocardial infarction (AMI) in patients with ankylosing spondylitis (AS) compared with the general population.MethodsIn total, 292 subjects with AS and a first AMI between Jan 2006 and Dec 2014 were identified using the Swedish national patient register. Each subject was matched with up to 5 general population comparators per AS-patient (n = 1276). Follow-up started at the date of admission for AMI and extended until death or 365 days of follow-up. Cox regression was used to assess mortality in two time intervals: days 0-30 and days 31-365. For a subgroup with available data, clinical presentation at admission, course, treatment for AMI, and secondary prevention were compared.ResultsDuring the 365-day follow-up, 56/292 (19%) AS patients and 184/1276 (14%) comparators died. There were no difference in mortality due to cardiovascular-related causes, although the overall mortality day 31-365 was increased among patients with AS compared with comparators (HR [95% CI] = 2.0 [1.3;3.0]). At admission, AS patients had a higher prevalence of cardiovascular comorbidities compared with comparators. At discharge, patients with AS were less often prescribed lipid-lowering drugs and non-aspirin antiplatelet therapy.ConclusionsPatients with AS tend to have a higher comorbidity burden at admission for first AMI. The mortality after a first AMI due to cardiovascular-related causes does not seem to be elevated, despite an increased overall mortality during days 31-365 among patients with AS compared with the general population. Key Points • The all-cause mortality after a first AMI was higher in patients with AS. • Mortality after a first AMI due to CVD-related causes does not seem to be elevated for patients with AS. • In patients with AS suffering a first AMI, more attention should be given to other comorbidities causing an excess in mortality.

Project description:Studies of miRNA profiling in the plasma of AMI patients with Left ventricular end diastolic volume (LVEDV) increase or LVEDV decrease between discharge and follow-up Venous blood was collected in citrated tubes prior discharge. Plasma was harvested by centrifugation and stored at -80°C until assayed. Identical volumes of plasma from the 30 patients of a group (LVEDV increase or LVEDV decrease) were pooled to reach a final volume of 400µL for each group. The two pools were processed conjointly. Total RNA was extracted using miRVana isolation kit (Applied Biosystems), dephosphorylated and labeled using miRNA Complete Labeling and hybridization kit (Agilent). Hybridization was performed on miRNA Human Microarray Release 12.0 slides (Agilent). 4 arrays per pool were hybridized. Scanning was achieved with the Genepix 4000B Scanner (Molecular Devices, Sunnyvale, USA). Raw data were acquired with the Genepix Pro software (Molecular Devices).

Project description:Acute myocardial infarction (AMI) is primarily due to coronary atherosclerotic plaque rupture and subsequent thrombus formation. Platelets play a key role in the genesis and progression of both atherosclerosis and thrombosis. Since platelets are anuclear cells that inherit their mRNA from megakaryocyte precursors and maintain it unchanged during their life span, gene expression (GE) profiling at the time of an AMI provides information concerning the platelet GE preceding the coronary event. In ST-segment elevation myocardial infarction (STEMI), a gene-by-gene analysis of the platelet GE identified five differentially expressed genes (DEGs): FKBP5, S100P, SAMSN1, CLEC4E and S100A12. The logistic regression model used to combine the GE in a STEMI vs healthy donors score showed an AUC of 0.95. The same five DEGs were externally validated using platelet GE data from patients with coronary atherosclerosis but without thrombosis. Early signals of an imminent AMI are likely to be found by platelet GE profiling before the infarction occurs.
