Project description:The innate immune system plays an essential role in regulating the immune responses to kidney transplantation, but the mechanisms through which innate immune cells influence long-term graft survival are unclear. The current study highlights the vital role of trained immunity in kidney allograft survival. Trained immunity describes the epigenetic and metabolic changes that innate immune cells undergo following an initial stimulus, allowing them have a stronger inflammatory response to subsequent stimuli. We stimulated healthy peripheral blood mononuclear cells with pretransplant and posttransplant serum of kidney transplant patients and immunosuppressive drugs in an in vitro trained immunity assay and measured tumor necrosis factor and interleukin 6 cytokine levels in the supernatant as a readout for trained immunity. We show that the serum of kidney transplant recipients collected 1 week after transplantation can suppress trained immunity. Importantly, we found that kidney transplant recipients whose serum most strongly suppressed trained immunity rarely experienced graft loss. This suppressive effect of posttransplant serum is likely mediated by previously unreported effects of immunosuppressive drugs. Our findings provide mechanistic insights into the role of innate immunity in kidney allograft survival, uncovering trained immunity as a potential therapeutic target for improving graft survival.

Project description:Solid organ transplantation in the mouse is a powerful research tool that has provided a pathway to find important mechanistic insights into the regulation of allograft injury, allograft immunopathology, and transplant rejection/tolerance, and that has unique advantages over transplantation in larger species, due to the well characterization of mouse genome and availability of genetically modified animals. However, setup of mouse liver, heart and kidney transplantation is a technically demanding surgical procedure, especially the orthotopic liver transplantation in mouse. Here, we performed Microwell-based single cell RNA-seq of three mouse organ transplantation models (liver, heart and kidney). Comparison of lymphocytes, parenchyma cells and peripheral blood mononuclear cells in liver, heart and kidney revealed distinct immune microenvironment at acute rejection and tolerance state respectively. Single-cell transcriptome analysis of mouse allograft without immunosuppressive drugs provided rich resources to help understand functioning solid-organ transplantation in human.

Project description:Early development of acute rejection after kidney transplantation is associated with diminished long-term graft survival. Predicting early acute rejection (EAR) at the time of transplant is important to risk-stratify patients and titrate immunosuppression accordingly. We performed whole-blood RNA sequencing at the time of transplant in 235 kidney transplant recipients enrolled in a prospective-cohort study [one discovery set (N=81), two validation sets (N=74 and N=80)] and evaluated the relationship with EAR and graft loss. We identified a blood based 23-gene set in recipients at the time of transplant that predicts the risk of EAR and is associated with late AR and allograft loss. This gene set is an important new tool to risk-stratify recipients before kidney transplantation and help guide immunosuppressive therapy accordingly.

Project description:Prediction power of kidney allograft outcomes based on non-invasive assays is limited. Assessments of operational tolerance (OT) patients allow to identify transcriptomic signatures of true non-responders for construction of predictive models. In the discovery cohort, RNA sequencing was used to identify protective set of transcripts by comparison of 15 OT patients from TOMOGRAM Study (NCT05124444) with 14 chronic active antibody-mediated rejection (CABMR) and 23 stable graft function ≥15 years (STA). Archetypal analysis and classifier performance of RNA sequencing data showed that OT is clearly distinguishable from CABMR, but similar to STA. Selected differentially expressed transcripts between OT and CABMR were used in validation cohort (n=396) to predict 3-year kidney allograft loss at 3 time-points using RT-qPCR. Based on significant transcripts in univariable analysis, 2 multivariable Cox models were created. 3-transcripts (ADGRG3, ATG2A and GNLY) model from POD 7 predicted graft loss with C C-statistics (C) 0.727 (95% CI, 0.638-0.820). Another 3-transcript (IGHM, CD5, GNLY) model from M3 predicted graft loss with C 0.786 (95% CI, 0.785-0.865). Combining 3-transcript model with eGFR at month 3 improved C-statistics to 0.860 (95% CI, 0.778-0.944) and 0.868 (95% CI, 0.790-0.944), respectively. The model based on DGF, acute rejection, HLA mismatch and eGFR reached similar performance (C 0.846). In conclusion, identification of transcripts distinguishing OT from CABMR allowed to construct models predicting premature graft loss. Predictive statistics of these models reach same performance as well known-clinical models. Identified transcripts reflect mechanisms of injury/repair and alloimmune response when assessed at day 7 or with a loss of protective phenotype when assessed at month 3.
