Project description:To find novel circulating markers associated with prognosis of interferon therapy,We performed microarray analysis of plasma samples in 94 chronic hepatitis B patients Ninety four HBV patients, who underwent PEG-IFN or conventional interferon treatment were enrolled. Their plasma samples before treatment were collected and subject to miRNA array analysis. miRNA profiles from 13 formalin fixed formaldehyde embedded liver biopsy samples were also analyzed to evaluate the correlation between liver and plasma.

Project description:Hepatitis B virus (HBV)-associated acute on chronic liver failure (HBV-ACLF), characterized by an acute deterioration of liver function in the patients with chronic hepatitis B (CHB), is lack of predicting biomarkers for prognosis. To explore potential biomarkers of HBV-ACLF for clinical applications, immuno-depletion of high-abundance plasma proteins followed by iTRAQ-based quantitative proteomic approach was employed to analyze plasma samples from 20 healthy control people, 20 CHB patients and 20 HBV-ACLF patients, respectively. As a result, a total of 427 proteins were identified and quantified from these samples, and 42 proteins were differentially expressed in HBV-ACLF patients as compared to both CHB patients and healthy controls. According to bioinformatics analysis results, 6 proteins related to immune response (MMR), inflammatory response (OPN, HPX), blood coagulation (ATIII) and lipid metabolism (APO-CII, GP73) were selected as biomarker candidates. Further ELISA analysis confirmed the significant up-regulation of GP73, MMR, OPN and down-regulation of ATIII, HPX, APO-CII in HBV-ACLF plasma samples (p<0.01). Moreover, receiver operating characteristic (ROC) curve analysis revealed high diagnostic value of these candidates in assessing HBV-ACLF. In conclusion, present quantitative proteomic study identified 6 novel HBV-ACLF biomarker candidates and might provide fundamental information for development of HBV-ACLF biomarker.

Project description:Background: Circulating miRNAs, a new family of miRNAs presentin plasma and serum, have shown great potential as novel body fluid biomarkers for non-invasive diagnosis and prognosis of several diseases. Methods: In the present study, we employed next-generation sequencing technology to analyze the expression profiles of circulating miRNAs in the serum of chronic hepatitis B (CHB) patients presenting persistently normal alanine aminotransferase with significant histological features (SPNALT),PNALT patients with no significant histological features (NSPNALT),and healthy control patients. Results: MiRNAs were found to be the major constituents of small RNA-annotated reads by comparing the annotate draw sequence reads with miRNA databases and genome and other small RNA libraries. Our results point to a total of 2767 unique reads that can be mapped to human miRNAs or pre-miRNAs in miRbase, and the pre-miRNAs can be further mapped to the human genome. Thirty novel miRNA genes were identified in three groups. The differential expression levels of 143 miRNAs showed significant variation when the SPNALT and control group were compared. Among these, 22 miRNAs were >4-fold up-regulated (P<0.01) in SPNALT group and 4 were >2-fold down-regulated (P<0.01).When the SPNALT and NSPNALT group were compared, the differential expression levels of 84 miRNAs showed significant variation. Among these, 13 miRNAs showed a >4-fold up-regulation (P<0.01) in the SPNALT group and 21 showed a >2-fold down-regulation (P<0.01). Conclusions: Our results show that circulating miRNAs in the serum of SPNALT patients could be more comprehensively detected by the next-generation sequencing technology. Newly identified miRNAs might be used as biomarkers for diagnosis of chronic hepatitis B patients presenting persistently normal alanine aminotransferase with significant histological features.

The microarray datasets were analyzed with significance analysis of microarrays (SAM) to identify genes regulated by IFN-alpha. We identified 516 named genes up-regulated at least 2-fold, at a false discovery rate (FDR) of less than 1%. In vivo and ex vivo studies generated similar results. No genes were identified as regulated differently between these 2 experimental conditions. The up-regulated genes belonged to a broad range of functional pathways and included multiple genes thought to be involved in the direct antiviral effect of IFN-alpha. Of particular interest, 88 genes directly relating to functions of immune cells were up-regulated, including genes involved in antigen processing and presentation, T-cell activation, lymphocyte trafficking, and effector functions, suggesting that IFN-alpha up-regulates multiple genes involving different aspects of immune responses to enhance immunity against hepatitis C virus. In conclusion, IFN-alpha-inducible genes can be identified in human PBMCs in vivo as well as ex vivo. Signature changes associated with different treatment outcomes may be found among these genes. Project description:Hepatitis C virus (HCV) infection is a major cause of chronic liver disease worldwide. The current standard therapy for chronic hepatitis C (CHC) consists of a combination of pegylated IFN alpha (pegIFN-alpha) and ribavirin. It achieves a sustained viral clearance in only 50–60% of patients. To learn more about molecular mechanisms underlying treatment failure, we investigated IFN-induced signaling in paired liver biopsies collected from CHC patients before and after administration of pegIFN-alpha. In patients with a rapid virological response to treatment, pegIFN-alpha induced a strong up-regulation of IFN-stimulated genes (ISGs). As shown previously, nonresponders had high expression levels of ISGs before therapy. Analysis of posttreatment biopsies of these patients revealed that pegIFN-alpha did not induce expression of ISGs above the pretreatment levels. In accordance with ISG expression data, phosphorylation, DNA binding, and nuclear localization of STAT1 indicated that the IFN signaling pathway in nonresponsive patients is preactivated and refractory to further stimulation. Some features characteristic of nonresponders were more accentuated in patients infected with HCV genotypes 1 and 4 compared with genotypes 2 and 3, providing a possible explanation for the poor response of the former group to therapy. Taken together with previous findings, our data support the concept that activation of the endogenous IFN system in CHC not only is ineffective in clearing the infection but also may impede the response to therapy, most likely by inducing a refractory state of the IFN signaling pathway.
