Project description:Chronic Obstructive Pulmonary Disease (COPD) is a chronic inflammatory disease, primarily affecting the airways. Stable biomarkers characterizing the inflammatory phenotype of the disease, relevant for disease activity and suited to predict disease progression are needed to monitor the efficacy and safety of drug interventions. We therefore analyzed a large panel of markers in bronchoalveolar lavage, bronchial biopsies, serum and induced sputum of 23 healthy smokers and 24 smoking COPD patients (GOLD II) matched for age and gender. Sample collection was performed twice within a period of 6 weeks. Assays for over 100 different markers were validated for the respective matrices prior to analysis. In our study, we found 51 markers with a sufficient repeatability (intraclass correlation coefficient >0.6), most of these in serum. Differences between groups were observed for markers from all compartments, which extends (von-Willebrand-factor) and confirms (e.g. C-reactive-protein, interleukin-6) previous findings. No correlations between lung and serum markers were observed, including A1AT. Airway inflammation defined by sputum neutrophils showed only a moderate repeatability. This could be improved, when a combination of neutrophils and four sputum fluid phase markers was used to define the inflammatory phenotype.In summary, our study provides comprehensive information on the repeatability and interrelationship of pulmonary and systemic COPD-related markers. These results are relevant for ongoing large clinical trials and future COPD research. While serum markers can discriminate between smokers with and without COPD, they do not seem to sufficiently reflect the disease-associated inflammatory processes within the airways.

Biomarkers in Liquid Biopsies for Prediction of Early Liver Metastases in Pancreatic Cancer.

Project description:Pancreatic ductal adenocarcinoma (PDAC) is one of the most aggressive solid malignancies with poor survival rates. Only 20% of the patients are eligible for R0-surgical resection, presenting with early relapses, mainly in the liver. PDAC patients with hepatic metastases have a worse outcome compared to patients with metastases at other sites. Early detection of hepatic spread bears the potential to improve patient outcomes. Thus, this study sought for serum-based perioperative biomarkers allowing discrimination of early (EHMS ≤ 12 months) and late hepatic metastatic spread (LHMS > 12 months). Serum samples from 83 resectable PDAC patients were divided into EHMS and LHMS and analyzed for levels of inflammatory mediators by LEGENDplexTM, which was validated and extended by Olink® analysis. CA19-9 serum levels served as control. Results were correlated with clinicopathological data. While serum CA19-9 levels were comparable, Olink® analysis confirmed distinct differences between both groups. It revealed significantly elevated levels of factors involved in chemotaxis and migration of immune cells, immune activity, and cell growth in serum of LHMS-patients. Overall, Olink® analysis identified a comprehensive biomarker panel in serum of PDAC patients that could provide the basis for predicting LHMS. However, further studies with larger cohorts are required for its clinical translation.

Identification of gene biomarkers for respiratory syncytial virus infection in a bronchial epithelial cell line.

Project description:Respiratory syncytial virus (RSV) infection involves complex virus-host interplay. In this study, we analyzed gene expression in RSV-infected BEAS-2B cells to discover novel signaling pathways and biomarkers. We hybridized RNAs from RSV- or vehicle-treated BEAS-2B to Affymetrix HU133 plus 2.0 microarrays (n = 4). At 4 and 24 h post-infection, 277 and 900 genes (RSV/control ratio >/=2.0 or </=0.5), and 1 and 12 pathways respectively were significantly altered. Twenty-three and 92 genes at 4 and 24 h respectively matched respiratory disease biomarkers with ARG2 flagged at 24 h and SCNN1G, EPB41L4B, CSF1, PTEN, TUBB1 and ESR2 at both time points. Hierachical clustering showed a cluster containing ARG2 and IL8. In human bronchial epithelial cells, RSV upregulated arginase II protein. Knockdown of ARG2 increased RSV-induced IL-8, LDH and histone release. With microarray, we identified novel proximal airway epithelial cell genes that may be tested in the sputum samples as biomarkers of RSV infection.

Associations of Prenatal Dietary Inflammatory Potential with Childhood Respiratory Outcomes in Project Viva.

Project description:BackgroundInflammation during pregnancy may be a factor in the developmental programming of asthma and wheeze in childhood.ObjectiveTo examine associations of inflammatory potential of prenatal diet with respiratory outcomes in early childhood and midchildhood.MethodsAmong 1424 mother-child pairs in Project Viva, a prebirth cohort, we examined associations of Dietary Inflammatory Index (DII®) (first trimester, second trimester, and average of first and second trimesters) scores in relation to ever asthma and wheezing in the past year (early childhood and midchildhood); current asthma and lung function (midchildhood), and wheeze trajectory during 1 to 9 years. We used multivariable linear and logistic regression modeling, adjusting for relevant confounders.ResultsIn a fully adjusted analysis, a more proinflammatory diet was associated with an early versus never wheeze trajectory (first- and second-trimester average fourth vs first quartile: odds ratio, 1.89; 95% CI, 1.14-3.13). A more proinflammatory diet during pregnancy also was associated with lower forced expiratory flow (forced expiratory flow at 25%-75%) in midchildhood (first- and second-trimester average fourth vs first quartile: ?, -132 mL; 95% CI, -249 to -14). Results were evident for first-, but not second-, trimester DII and wheeze trajectory and midchildhood forced expiratory flow at 25% to 75%. Other child respiratory outcomes, including ever asthma, were not related to any DII measure during pregnancy.ConclusionsProinflammatory diet during pregnancy is associated with wheeze trajectory during early childhood and decrements in small airways caliber in midchildhood, but not other respiratory outcomes in the offspring.
