Project description:The chemotherapy resistance of esophageal adenocarcinomas (EAC) is underpinned by cancer cell extrinsic mechanisms of the tumor microenvironment (TME). We demonstrate that by targeting the tumor-promoting functions of the predominant TME cell type, cancer associated fibroblasts (CAF) with phosphodiesterase type 5 inhibitors (PDE5i) we can enhance the efficacy of standard-of-care chemotherapy. These findings demonstrate that CAF drive chemotherapy resistance in EAC and can be targeted by repurposing PDE5i, a safe and well tolerated class of drug administered to millions of patients world-wide to treat erectile dysfunction.

Phosphodiesterase type 5 inhibitors enhance chemotherapy in preclinical models of esophageal adenocarcinoma by targeting cancer-associated fibroblasts.

Project description:The chemotherapy resistance of esophageal adenocarcinomas (EACs) is underpinned by cancer cell extrinsic mechanisms of the tumor microenvironment (TME). We demonstrate that, by targeting the tumor-promoting functions of the predominant TME cell type, cancer-associated fibroblasts (CAFs) with phosphodiesterase type 5 inhibitors (PDE5i), we can enhance the efficacy of standard-of-care chemotherapy. In ex vivo conditions, PDE5i prevent the transdifferentiation of normal fibroblasts to CAF and abolish the tumor-promoting function of established EAC CAFs. Using shotgun proteomics and single-cell RNA-seq, we reveal PDE5i-specific regulation of pathways related to fibroblast activation and tumor promotion. Finally, we confirm the efficacy of PDE5i in combination with chemotherapy in close-to-patient and in vivo PDX-based model systems. These findings demonstrate that CAFs drive chemotherapy resistance in EACs and can be targeted by repurposing PDE5i, a safe and well-tolerated class of drug administered to millions of patients world-wide to treat erectile dysfunction.

Methiothepin Increases Chemotherapy Efficacy against Resistant Melanoma Cells.

Project description:We previously reported that methiothepin, a small molecule known as a nonselective serotonin 5-HT receptor antagonist, inhibited the doxorubicin efflux activity of the Hedgehog receptor Ptch1 and enhanced the cytotoxic, pro-apoptotic, anti-proliferative, and anti-clonogenic effects of doxorubicin on adrenocortical carcinoma cells. Here, we show that methiothepin also inhibits doxorubicin efflux and increases doxorubicin cytotoxicity in melanoma cells which endogenously overexpress Ptch1. Melanoma patients having the BRAFV600E mutation are treated with vemurafenib, an inhibitor of BRAFV600E, often in combination with trametinib, an inhibitor of MEK. Almost all patients ultimately acquire resistance to the treatment leading to disease progression. Here, we report that methiothepin overcomes the resistance of BRAFV600E melanoma cells by enhancing the cytotoxicity of vemurafenib and trametinib on these cells leading to melanoma cells death. We observe that the addition of methiothepin to vemurafenib prevents migration of resistant melanoma cells more efficiently than vemurafenib alone. Our results provide an additional proof that Ptch1 drug efflux inhibition increases the effectiveness of anti-cancer treatments and overcomes resistance of melanoma cells expressing Ptch1.

Preclinical efficacy of the MTH1 inhibitor karonudib in B-cell lymphoma

Project description:Although chemo-immunotherapy has greatly improved survival of B-cell lymphoma patients, treatment resistant and refractory disease represent a major challenge that call for development of new treatment options. Karonudib (TH1579) is a new drug developed to inhibit MTH1, an enzyme which prevents oxidized dNTPs from being incorporated into DNA. Here, we tested the efficacy of karonudib in vitro and in preclinical models of B-cell lymphoma. Karonudib reduced viability in a wide range of B-cell lymphoma cell lines at concentrations that were well tolerated by activated normal B cells. Induction of mitotic arrest was seen as early as 6 h after karonudib treatment, with induction of apoptosis detected after 12 h. These effects were observed independent of TP53 mutational status. Increased incorporation of 8-oxo-dGTP into DNA, in addition to arrest in prometaphase due to failure in spindle assembly were detected in cells exposed to karonudib, suggesting a dual inhibitory mechanism. Karonudib inhibited tumor growth, led to complete remission in the majority of cases and prolonged survival in two different xenograft mouse models of aggressive B-cell lymphoma, including an ABC DLBCL patient-derived xenograft model. Karonudib was well tolerated in vivo, and no weight loss was observed in treated animals. NUDT1, the gene encoding MTH1, was upregulated in tumor biopsies from ABC and GCB DLBCL and BL patients, as compared to B cells from healthy donors. Together, our preclinical findings provide a rational for further clinical testing of karonudib in aggressive B-cell lymphoma.
