The effect of probiotics supplementation on gut microbiota after Helicobacter pylori eradication: a multicenter, randomised controlled trial

Project description:IntroductionHelicobacter pylori eradication therapy may lead to the perturbation of gut microbiota. We aim to investigate the impact of probiotics on eradication rate and gut microbiota during eradication therapy.MethodsA total of 162 patients receiving bismuth quadruple therapy were enrolled and randomly assigned to groups given probiotics (n = 83) or placebo (n = 79) for 4 weeks. Fecal samples were collected before treatment and 2, 4, 6, and 8 weeks after eradication therapy. Gut microbiota was analyzed by 16S rRNA high-throughput sequencing.ResultsThe eradication rates in the placebo and probiotics group were 82.43% and 87.01%, respectively (P > 0.05). Compared with baseline, alpha and beta diversity was significantly altered 2 weeks after eradication in both groups, which was restored at week 8. There were no significant differences in diversity between the two groups. H. pylori eradication therapy resulted in enrichment of some detrimental bacteria taxa such as Shigella, Klebsiella, and Streptococcus, while probiotics supplementation could rapidly restore these taxa levels after eradication and increase the taxa of Bacillus and Lactobacillales. Functional analysis revealed that lipopolysaccharide biosynthesis and polymyxin resistance pathways were significantly enriched after eradication, while probiotics supplementation mainly enriched the cofactors and vitamins metabolism pathways. Increased relative abundances of Roseburia and Dialister were associated with the positive eradication outcome.ConclusionsProbiotics supplementation might help to construct a beneficial profile of gut microbiota after eradication therapy. Specific bacteria taxa are associated with H. pylori eradication outcome. These findings may be of value in rational use of probiotics during H. pylori eradication.Trial registrationChinese Clinical Trial Registry, ChiCTR1900022116.

Project description:In animal production the use of probiotics supplements to promote animal health is increasing. The objective of this study was to assess the impact of probiotics administration on global gene expression in dairy cows. Lactating Holstein Friesian cows (n=10) from the North Carolina Agricultural and Technical State University dairy herd were used for the study. Treatment was a 10 ml oral drench of FASTtrak microbial pack (Probiotics) (Conklin Company, Kansas City, MO) at the recommended dose in water or water only (control). This treatment was carried out for 60 days. Whole blood was collected at the beginning (Day 0) and end of the study (Day 60) for microarray analysis. We employed microarray expression profiling as a discovery platform to identify genes with potential association with probiotics supplementation in cows. Gene expression analysis identified 10,859 differentially expressed genes- 1168 upregulated genes and 9691 downregulated gene. Results for pathway analysis showed significant pathways associated with innate immunity such as the Toll-like receptor (TLR) pathway, inflammation response and Wingless (Wnt) signaling pathway. Real-time PCR was used to validate gene expression of members of the TLR and Wnt signaling pathway. Treatment affected the expression of innate and adaptive immune response, cytokine and Wnt pathway genes. Daily administration of probiotics to dairy cows impacts global gene expression and particularly the expression of innate immune genes in dairy cows. Ten animals were enrolled in the study and an initial blood sample was collected (Day 0). Animals (n=5) received either daily supplementations with FASTtrak microbial pack (Probiotics) (Conklin Company, Kansas City, MO) or water daily (control animals) for 60 days. Blood samples were collected at the end of the study from probiotics-treated and control animals for RNA extraction and microarray analysis. In vitro effect of lipopolysaccharide (LPS) endotoxin treatment was evaluated using blood samples collected from probiotics-treated animals (Day 60 samples) to serve as positive control array. A pooled sample was generated by taking equal concentration of RNA from experimental animals in each group. Pooled samples from each group was hybridized on Agilent one color bovine v2 bovine (v2) 4x44KÂ array slides.

Project description:The pharmacological significance of B vitamins, essential for various metabolic processes, and the therapeutic potential of probiotics in gastrointestinal health have been well-documented. However, the interactions between these entities remain poorly understood. In this study, we endeavored to elucidate the potential interplay between B vitamins and probiotics utilizing liquid chromatography-triple quadrupole mass spectrometry, pharmacokinetic modeling, and 16S rRNA gene sequencing. Employing healthy and pseudo-germ-free rat models, we revealed that probiotics significantly improve the absorption of B1, B3, B5, and B12, and that the gut microbiota played a mediating role in this enhanced absorption of B vitamins by probiotics. High-throughput genetic sequencing uncovered a synergistic effect of B vitamins and probiotics in modulating the gut microbiota, particularly increasing the abundance of Verrucomicrobia and Akkermansia. Furthermore, in vitro experiments demonstrated that probiotics used in this study had a relatively minor influence on the production and permeability of B vitamins, while B vitamins did not significantly contribute to the growth, auto-aggregation, and adhesion of probiotics. In summary, a complex network connection exists between B vitamins and probiotics, wherein the gut microbiota emerges as a pivotal factor that cannot be overlooked.

Project description:BackgroundPrevious meta-analyses reported that probiotics improve the effectiveness of Helicobacter pylori (H. pylori) eradication during antibiotic therapy, while results regarding a possible reduction of side effects remained inconclusive. Moreover, the effectiveness of different strains of probiotics has not been studied so far. It is further conceivable that probiotics will produce additional effects only if antibiotics are relatively ineffective.MethodsThis meta-analysis includes eligible randomized controlled trials examining effects of probiotics supplementation on eradication rates (ER) and side effects, published up to May 2014. Sub-group analysis was performed to compare different probiotic strains and antibiotic therapies with different effectiveness in controls (ER <80% vs.>80%). Publication bias was assessed with funnel plots and Harbord's test. The quality of the trials was assessed with the Cochrane risk of bias tool.ResultsThirty-three RCTs involving a total of 4459 patients met the inclusion criteria in case of eradication rates of which 20 assessed total side effects in addition. Overall, the pooled eradication rate in probiotics supplementation groups was significantly higher than in controls (ITT analysis: RR 1.122, 95% CI 1.086-1.159, PP analysis: RR 1.114, 95% CI 1.070-1.159). Sub group-analysis could, however, confirm this finding only for four individual strains (Lactobacillus acidophilus, Lactobacillus casei DN-114001, Lactobacillus gasseri, and Bifidobacterium infantis 2036) and for relatively ineffective antibiotic therapies. There was a significant difference between groups in the overall incidence of side effects (RR 0.735, 95% CI 0.598-0.902). This result was, however, only confirmed for non-blinded trials.ConclusionsThe pooled data suggest that supplementation with specific strains of probiotics compared with eradication therapy may be considered an option for increasing eradication rates, particularly when antibiotic therapies are relatively ineffective. The impact on side effects remains unclear and more high quality trials on specific probiotic strains and side effects are thus needed.

Project description:The aim of the experiment was to assign patients enrolled in the VANISH randomised trial to sepsis response signature (SRS) endotypes based on a previously published gene expression signature, in order to test for differential responses to treatment. VANISH was a double-blind randomised clinical trial in septic shock, with patients randomised to receive norepinephrine or vasopressin followed by hydrocortisone or placebo. We collected blood samples upon enrolment, extracted RNA and performed transcriptomic profiling using microarrays, allocated patients to SRS1 or SRS2 using a linear model (Davenport 2016), and tested for an association between sepsis endotype and response to either norepinephrine or vasopressin, or to corticosteroids. There was a significant interaction between treatment with hydrocortisone or placebo, and SRS endotype (p=0·02)
