Project description:Meningiomas account for roughly one-third of all primary brain tumors. Although typically benign, about 20% of meningiomas are aggressive, and despite the rigor of the current histopathological classification system, there remains considerable uncertainty in predicting tumor behavior. Here we analyzed 160 tumors from all three WHO grades (I-III) using clinical, gene expression and sequencing data. Unsupervised clustering analysis identified three molecular groups that reliably predicted clinical severity. These groups did not directly correlate with the WHO grading system, which would classify more than half of the tumors in the most aggressive molecular group as benign. Transcriptional and biochemical analyses revealed that aggressive meningiomas involve loss of the repressor function of the DREAM complex, resulting in cell cycle activation, and only tumors in this group tend to recur after full resection. These findings should improve our ability to predict recurrence and develop targeted treatments for these clinically challenging tumors.

Project description:Variability of meningioma growth, even within benign subgroup, makes some remain unaltered, while others grow fast despite an appended radiotherapy. Biomarkers that differentiate between less and more aggressive meningiomas would therefore have a significant clinical benefit in prediction of their biological behaviour. The aim of the study was to identify new candidate miRNAs enabling better and proper prediction of meningioma recurrence. We used miRNA 4.0 array (Applied Biosystems, Foster City, CA, USA) to detect miRNA profiles in 44 primary meningioma patients (19 with subsequent recurrence) and 5 healthy controls. Moreover, miRNA profiles were analyzed in 15 secondary meningiomas.

Meningiomas tumor recurrence and metabolic aggressiveness expression profile

Project description:The aim of this study was to determine the relationship between gene expression profiles and new metabolic subgroups of benign meningioma with potential clinical relevance. 19 human meningiomas biopsies were obtained at the Department of Neurosurgery of the Clinical University Hospital of Valencia. This study was reviewed and approved by the local ethics committee. During surgery, most of the resected tissue was sent for routine histological analysis and the remainder was immediately put in cryogenic vials and snap-frozen in liquid nitrogen. All snap-frozen samples were stored in a freezer at -80 C until further analysis. Tumors were classified according to the 2007 WHO Histological Classification 1. Grade II meningioma were classified according to previously published criteria, including high mitotic index and three out of five markers of atypia.In summary, this work demonstrates gene expression differences in a new subgroup of histological benign meningiomas that exhibit aggressive metabolism and tumor recurrence.

Meningioma transcriptomic landscape demonstrates novel subtypes with regional associated biology and patient outcome

Project description:Meningiomas are the most common primary intracranial tumors in humans. While most of these tumors are benign, some are malignant, rapidly recur after multimodal treatment with surgery and radiotherapy, and can ultimately be fatal. The current WHO grade system does not always identify high risk meningiomas, therefore better characterizations of the biology of aggressive tumors are needed. In order to address these challenges, we combined 13 bulk RNA-Seq datasets, corrected for batch effects, and applied Uniform Manifold Approximation and Projection (UMAP) to create a reference landscape of ~1300 meningioma tumors. Our analyses revealed multiple distinct meningioma subtypes with specific biological signatures. Clinical metadata, mutations, copy number alterations, and gene-fusion data effectively correlated with regions of the UMAP. Notably, regional distribution of time to recurrence identified major clusters as well as intra-cluster differences of meningiomas with varying patient outcomes. The most aggressive subtype, characterized by an enrichment of higher WHO grades, frequent tumor recurrences, and shorter time to recurrence, exhibited elevated proliferation rates and RNA expression resembling muscle development. To facilitate clinical applications, we developed a cross-validated nearest-neighbors-based algorithm that accurately maps new patients onto this UMAP landscape. Our study highlights the utility of transcriptomic analysis in discerning meningioma heterogeneity as well as successful combination of multiple datasets from various sources. We provide a valuable tool for understanding the disease, predicting tumor biology and patient prognosis. This resource is accessible via the open source, interactive online tool Oncoscape, where the scientific community can explore the landscape and mine clinical and genomic metadata.

Whole genome expression in meningioma

Project description:We performed expression profiling of 24 meningioma and two dura controls analyzing 55000 transcripts including 18300 known genes. We compared expression in meningioma vs. dura, expression of low grade (WHO I) vs. higher-grade (WHO II and WHO IIII) tumors and expression of meningothelial and syncytial meningioma vs. fibroblastic meningioma. Gene expression was analysed in 24 meningioma including eight of each WHO grade and two dura controls analyzing 55000 transcripts including 18300 known genes.
