Project description:Severe fever with thrombocytopenia syndrome banyangvirus (SFTSV) induces immunopathogenic disease with high fatality rate. Despite serious health concern, the mechanisms underlying clinical manifestations are largely unknown. Here, we applied targeted proteomics and single-cell transcriptomics analyses to examine the differential immune landscape of SFTS patient blood. Serum immunoprofiling distinguished Low-risk and High-risk clusters of SFTS patients with differential inflammatory cytokine levels, which matched with disease severity. Single-cell transcriptomics analysis of SFTS patient blood at different infection state showed pronounced expansion of B cells with the alteration of B-cell subset populations, specifically in fatal patients. Furthermore, plasma cells were the primary reservoir of SFTSV replication with the downregulation of IFN pathway. This study identifies not only the molecular signatures of serum inflammatory profile and B cell lineage population, but also plasma cells as the viral reservoir in SFTSV-induced fatal patients, suggesting the alteration of B cell function linked to SFTS lethal infection.

Monocytes are known to play an important role in the pathogenesis of SFTS, but the distinct subpopulations of monocytes associated with the disease progression and their roles in inflammatory responses remain poorly elucidated. In this study, we used single-cell RNA sequencing (scRNA-seq) to explore the specific monocyte subpopulations correlated with the progression of SFTS and their involvement in inflammatory processes.

Single-cell landscape of peripheral immune responses to fatal SFTS

Project description:We performed scRNA-seq on 27 PBMC samples from 15 SFTS patients (including four deceased and 11 surviving) and four healthy controls. We found that the immune cell landscape was reprogrammed in SFTS and characterized by monocyte shift to intermediate type along with complement activation, perturbation of plasmablasts composition, and highly exhausted T cells, all were correlated with lethal consequences. We identified the overexpression of interferon-stimulated genes across most immune cell types post SFTSV infection, which were simultaneously related to older age, high viremia and hyper-inflammation. These data collectively supported the intermediate monocytes and IFN-I-inducible plasmablasts to be major targets for SFTSV infection, and proposed the pivotal role of IFN-I response in exacerbating hyper-inflammation and lethal SFTS, which might help to identify therapeutic strategies for SFTS.

Serum Proteomics of Severe Fever with Thrombocytopenia Syndrome Patients

Project description:Dabie bandavirus, also termed as severe fever with thrombocytopenia syndrome virus (SFTSV), was first isolated in China in 2010. At this time, the virus was found to have spread to South Korea, Japan, and other countries. A high case fatality rate is reported for SFTS, ranging from 12–50% within various sources. Several omics for clinical studies among SFTS patients as well as studies of cultured SFTSV have attempted to characterize the relevant molecular biology and epidemiology of the disease. However, a global serum proteomics analysis among SFTS patients has not yet been reported to date. Thrombocytopenia and multiple organ failure are the major immediate causes of death among SFTS patients. In this study, serum proteomic changes related to thrombocytopenia, abnormal immune response, and inflammatory activation were documented in SFTS patients. These findings provide useful information forunderstanding the clinical manifestations of SFTS.

Molecular characterization of systemic sclerosis esophageal pathology identifies inflammatory and proliferative signatures

Project description:Esophageal involvement in patients with systemic sclerosis (SSc) is common, but tissue-specific pathological mechanisms are poorly understood. Fibrosis in the esophagus is thought to disrupt smooth muscle function and lead to esophageal dilatation, but autopsy studies demonstrate esophageal smooth muscle atrophy and the absence of fibrosis in the majority of SSc cases. Molecular characterization of SSc esophageal pathology is lacking. Herein, we perform a detailed characterization of SSc esophageal histopathology and molecular signatures at the level of gene expression. Esophageal biopsies were prospectively obtained during esophagogastroduodenoscopy in 16 consecutive SSc patients and 7 subjects without SSc. Upper and lower esophageal biopsies were evaluated for histopathology and gene expression. Individual patient’s upper and lower esophageal biopsies showed nearly identical patterns of gene expression. Similar to skin, inflammatory and proliferative gene expression signatures were identified suggesting that molecular subsets are a universal feature of SSc end-target organ pathology. The inflammatory signature was present in biopsies without high numbers of infiltrating lymphocytes. Molecular classification of esophageal biopsies was independent of SSc skin subtype, serum autoantibodies and esophagitis. Proliferative and inflammatory molecular gene expression subsets in tissues from patients with SSc may be a conserved, reproducible component of SSc pathogenesis. The inflammatory signature is observed in biopsies that lack large inflammatory infiltrates suggesting that immune activation is a major driver of SSc esophageal pathogenesis.

Inflammatory and interferon gene expression signatures in patients with mitochondrial disease

Project description:People with mitochondrial disease are susceptible to metabolic decompensation and neurological symptom progression in response to an infection. Increasing evidence suggests that mitochondrial dysfunction may cause chronic inflammation, which may promote hyperresponsiveness to pathogens and neurodegeneration. We collected whole blood and isolated peripheral blood mononuclear cells from mitochondrial disease patients and healthy controls and examined transcriptional changes to identify common gene signatures of immune dysfunction in mitochondrial disease. We performed GSEA analyses and identified a positive enrichment of inflammatory and type I interferon gene sets in mitochondrial disease patients, coincident with a negative enrichment of T cell and B cell gene sets.
