Project description:The BCN02 (NCT02616874) was a pilot study to evaluate the kick and kill therapy on 15 early-treated individuals that were rolled-over from the BCN01 Study. The participants in the study received two MVA.HIVconsv vaccination before and after the 3-cycle infusion of Romidepsin, an inhibitor of histone deacetylases that act as a latency reversing agent. Additionally, the inclusion of a monitored antiretroviral pause (MAP) allowed tthe identification of 8 individuals with an Early Rebound (pVL > 2,000 HIV RNA copies/mL, < 4 weeks of MAP initiation) and 4, with a Late Rebound (pVL > 2,000 HIV RNA copies/mL, > 4 weeks of MAP initiation). In the present study we studied gene expression at 3 different timepoints: w0 (Bsl), w1 (Vacc), w6 (Vacc+RMD) and applied pairwise comparisons.

Project description:The RV144 clinical trial evaluated the efficacy of a vaccine regimen that included ALVAC-HIV prime and AIDSVAX boost in preventing HIV-1 acquisition. The vaccine reduced the risk of HIV-1 acquisition by 31.2%; however the mechanisms that led to the protection induced by this vaccine remain poorly understood. Our objectives were to identify transcriptional correlates and mechanisms that could explain the reduced acquisition conferred by the vaccine. We assessed the transcriptomic profile of HIV Env stimulated peripheral blood mononuclear cells collected from 223 participants two weeks after vaccination and from 40 placebo recipients.

A Clinical trial ready proteomics workflow to support large-scale analysis of Human Plasma: application to the Fenofibrate Intervention and Event Lowering in Diabetes (FIELD) trial

Project description:Background: Proteomic mass spectrometry is widely used to quantify proteins in biological specimens for biomarker discovery. Clinical studies requiring proteomic analysis of plasma are more complicated due to the high dynamic range and abundance of major plasma proteins. There is an unmet need to develop robust, rapid and cost-effective workflows able to support large-scale clinical studies of thousands of plasma specimens. We set about developing a suitable system to be used for the analysis of plasma collected from FIELD trial participants which investigated five years oral fenofibrate on coronary heart disease and microvascular complications.

Verification of Molecular Subtyping of Bladder Cancer in the GUSTO Clinical Trial

Project description:The GUSTO clinical trial (Gene expression subtypes of Urothelial carcinoma: Stratified Treatment and Oncological outcomes) uses molecular subtypes to guide neoadjuvant therapies in participants with muscle-invasive bladder cancer (MIBC). Before commencing the GUSTO trial, we needed to determine the reliability of a commercial subtyping platform (Decipher Bladder; Veracyte) when performed in an external trial laboratory as this has not been done previously. Here we report our pre-trial verification of the TCGA molecular subtyping model using gene expression profiling. Formalin fixed paraffin embedded tissue blocks of MIBC were used for gene expression subtyping by gene expression microarrays. Intra- and inter-laboratory technical reproducibility, together with quality control of laboratory and bioinformatics processes were assessed. Eighteen samples underwent analysis. RNA of sufficient quality and quantity was successfully extracted from all samples. All subtypes were represented in the cohort. Each sample was subtyped twice in our laboratory and once in a separate reference laboratory. No clinically significant discordance in subtype occurred between intra- or inter-laboratory replicates. Examination of sample histopathology showed variability of morphological appearances within and between subtypes. Overall, these results show that molecular subtyping by gene expression profiling is reproducible, robust, and suitable for use in the GUSTO clinical trial.
